A resource centre for practice based research.

Abstract

Academic family medicine units can provide an important service to practice-based researchers. A pilot programme was set up to test the feasibility of a practice-based Research Resource Centre in the Duke-Watts Family Medicine Program to provide support and expertise to practitioners who wish to carry out research projects in their practices. Physicians gained research experience and knowledge while actually performing the research in their own practice settings. They were able to proceed with projects at their own pace and use our resources as needed. At the same time, the interest and availability of our staff provided them with the necessary momentum to see their projects to completion.

[Study of short-term unfitness for work because of common illness among primary care management employees].

OBJECTIVES: To find the level of absenteeism among workers in the primary care management of Murcia, along with their distribution by professional category, health area, age and gender; and (2) find the causes of absenteeism and the relationship between these causes and the number of days lost. DESIGN: A descriptive, retrospective, population-based study. SETTING: Primary care management in Murcia. POPULATION: The management staff in Murcia consisted of 1,225 employees, who overall had 610 short-term periods of unfitness for work (SUW) due to common illness and accidents not at work, between October 1st, 1992 and October 1st, 1993. The variables studied were: age, gender, professional category, area where they worked, reason for and length of SUW and type of care. MAIN RESULTS: Index of absenteeism (IA) = 0.50, index of Seriousness (IS) = 40.75, Index of Incapacity (IL) = 18.12 and overall rate (OR) = 4.96. We found a large amount of SUW "without diagnosis": 104 cases (17.04%), which was the second most frequent cause behind respiratory complaints (23.77%). The IS were high in pathologies of the circulatory system, mental illness and neoplasias. CONCLUSIONS: The length of SUW (IS) is not influenced by any of the variables studied, excepting the specific cause of the SUW. II and IA are affected by the model of care (greater in the traditional type, p < 0.05), the health area where they work (p < 0.001) and professional category (p < 0.001). The filling out the sickness note must be improved in order to lessen the number of SUWs "without diagnosis".

[Diabetic night clinic. Advantages for insured patients and insurance carriers].

The night clinic for diabetics is a new, partially inpatient supplement of the present medical treatment. It represents a modern cooperative model which provides treatment in the time period that is not taken care of by either out-patient or normal in-patient service. The effectivity in the education and metabolic adjustment of the patients is as good as in a normal clinic. The night clinic permits the insured patient to have equal opportunities at work and a better quality of life. It thus fulfills the social and medical demands in the St.-Vincent declaration. The diabetic can no longer be discriminated against due to missing work in order to be instructed. By remaining employed the patients can be more realistically adapted through cooperation with the company doctor. The social and medical subsequent costs can thus be clearly reduced for the payer. By this cost reduction and by lower operating costs while simultaneously being a more efficient way of treatment, the night clinic conception fulfills modern medical, economic and legal standards.

[Transient work disability throughout a year in the IV Area of Asturias].

OBJECTIVE: We try to determinate the epidemiological distribution of the different pathologies which produce laboral absenteeism. DESIGN: Descriptive study. SETTING: IV Health Area, Asturias. PARTICIPANTS: Parts of discharge laboral transitory incapacity P9. INTERVENTION AND MAIN RESULTS: The gross rate of laboral transitory incapacity is 16.8%. Sex: 67.3% male, 32.7% female. The mean days for episode is 37.42 (DE = 60.47). Female average duration is 10 days more than male. For each year the mean duration increased 1.3 days (r = 0.25; p < 0.05). The 50% episodes has a duration less than two weeks. The three main illness groups are the breathing diseases (VIII), locomotor diseases (XIII) and accidents and traumatism (XVII). Many pathologies are associated with sex and age. Many pathologies have a stational distribution: VIII (p < 0.001), digestive diseases (p < 0.001), mental diseases (p < 0.05) and XVII (p < 0.05). CONCLUSIONS: 1) Is interesting the exploration of the information contents in P9 parts. 2) The aged is relationed with during of absenteeism. 3) Many pathologies were associated with the sex and aged of patients. 4) Many pathologies have a stational distribution.
