Primary hyperparathyroidism and hypertension.

Abstract

A causal relationship between hyperparathyroidism and hypertension has been presumed to exist. In order to determine the nature of any such relationship, 50 patients with primary hyperparathyroidism and diastolic hypertension undergoing surgical correction of hypercalcemia (study group) were compared to 50 matched eucalcemic patients with diastolic hypertension of similar magnitude undergoing equivalent elective surgery (control group). There were no significant differences in average preoperative diastolic pressures, average postoperative diastolic pressures, or the magnitude of early postoperative reduction in diastolic pressure between the study and control groups (P greater than 0.05). Bed rest may play a significant role in the early postoperative reduction in blood pressure frequently observed in patients undergoing correction of hyperparathyroidism.

Central venous pulse pressure analysis using an R-synchronized pressure measurement system.

OBJECTIVE: The information derived from central venous catheters is underused. We developed an EKG-R synchronization and averaging system to obtained distinct CVP waveforms and analyzed components of these. METHODS: Twenty-five paralyzed surgical patients undergoing CVP monitoring under mechanical ventilation were studied. CVP and EKG signals were analyzed employing our system, the mean CVP and CVP at end-diastole during expiration were compared, and CVP waveform components were measured using this system. RESULTS: CVP waveforms were clearly visualized in all patients. They showed the a peak to be 1.8+/- 0.7 mmHg, which was the highest of three peaks, and the x trough to be lower than the y trough (-1.6+/- 0.7 mmHg and -0.9+/- 0.5 mmHg, respectively), with a mean pulse pressure of 3.4 mmHg. The difference between the mean CVP and CVP at end-diastole during expiration was 0.58+/- 0.81 mmHg. CONCLUSIONS: The mean CVP can be used as an index of right ventricular preload in patients under mechanical ventilation with regular sinus rhythm. Our newly developed system is useful for clinical monitoring and for education in circulatory physiology.

Beyond inpatient and outpatient care: alternative model for hypertension management.

Hypertension is a major contributor to worldwide cardiovascular mortality, however, only one-third of patients with hypertension have their blood pressure treated to guideline recommended levels. To improve hypertension control, there may need to be a fundamental shift in care delivery, one that is population-based and simultaneously addresses patient, provider and system barriers. One potential approach is home-based disease management, based on the triad of home monitoring, team care, and patient self-care. Although there may be challenges to achieving the vision of home-based disease management, there are tremendous potential benefits of such an approach for reducing the global burden of cardiovascular disease.
