Variable diastolic rumbling murmur caused by floating left atrial thrombus.

Abstract

A 53 year old man with a history of rheumatic mitral stenosis was admitted to hospital because of recurrent fainting spells. Auscultation showed a variable diastolic rumbling murmur. Simultaneous echo- and phonocardiograms demonstrated a floating left atrial thrombus; when this passed into the left ventricular inflow tract and obstructed the mitral valve orifice the diastolic murmur disappeared.

Relation of left ventricular lead placement in cardiac resynchronization therapy to left ventricular reverse remodeling and to diastolic dyssynchrony.

The effects of left ventricular (LV) lead placement for cardiac resynchronization therapy (CRT) on LV remodeling and dyssynchrony are not well defined. Sixty-one patients (age 60 +/- 11 years, 76% men) were evaluated by echocardiography before and 4 +/- 2 months after CRT and grouped by the LV lead placement (lateral, posterolateral, or anterolateral). Echocardiographic measurements included LV volumes and LV ejection fraction. Tissue Doppler imaging was used to assess for inter- and intraventricular systolic and diastolic dyssynchrony. Analysis of variance was used to determine the effect of the LV lead placement on echocardiographic variables after CRT. The LV lead was placed in a lateral cardiac vein in 33 patients (54%), posterolateral in 15 (25%), and anterior in 13 (21%). Lateral LV lead placement was associated with significantly smaller LV volumes compared with the posterolateral lead placement (p <0.01). Diastolic dyssynchrony improved significantly with lateral lead placement compared with the anterior lead location (p <0.05). Improvement in LV ejection fraction and inter- and intraventricular systolic dyssynchrony was similar among the 3 groups. In conclusion, in patients undergoing CRT, a lateral lead location resulted in greater reverse LV remodeling and improved diastolic dyssynchrony compared with other lead placement locations.

Combined experimental and numerical analysis of the flow structure into the left ventricle.

Fluid dynamics is used for diagnosis in cardiology only to a partial extent. Indeed several aspects of cardiac flows and their relation with pathophysiology are unknown. The flow that develops into the left ventricle is here studied by using a combination of numerical and experimental models. The former allows a detailed three-dimensional analysis, the latter can be used in conditions, like in presence of turbulence, that are out of reach of the current computational power. The three-dimensional flow dynamics is analyzed in terms of its vortical structure. The study, within its limitations, provides further physical understanding about the intraventricular flow structure. This could eventually support the development of cardiac diagnostic indicators based on fluid dynamics.

Left ventricular diastolic function in hemodialysis patients: role of preload increase maneuver on tissue Doppler imaging evaluation.

OBJECTIVE: Tissue Doppler imaging (TDI) has recently been proposed as a relatively preload-independent method to evaluate left ventricular diastolic function. We sought to investigate the higher-accuracy of TDI to assess diastolic function in end-stage renal disease (ESRD) patients on hemodialysis (HD) associated with a preload increase maneuver. METHODS: Thirty-two consecutiveESRD patients (16 female, ages 48.8 +/- 17.5 years, 14 45 years old) were evaluated. Measurements of E, A velocities and the E/A ratio from transmitral inflow pulsed wave Doppler, and E', A' velocities and the E'/A' ratio from TDI were obtained 1 h before and 1 h after HD at baseline and with a preload increase maneuver. RESULTS: The E/A ratio changed significantly in all patients aged >45 before and after HD with the preload increase maneuver. The E'/A' ratio increased in all subjects with the preload increase maneuver before HD but did not change with the maneuver after HD in the euvolemic state in all patients. CONCLUSION: In ESRD patients on routine HD, TDI evaluation associated with a preload increase maneuver proved to be a more accurate method to identify diastolic dysfunction when the evaluation is performed in euvolemic patients after HD.
