The policy response to the problem of developing a geographical resource allocation formula sensitive to relative population needs for hospital and community health services resources in the National Health Service demonstrates a continuing tension between normative and empirical solutions. Since 1988, the balance has shifted in favour of a more empirical approach to identifying and weighting population needs indicators in response to concerns about the theoretically justified, but essentially approximate, nature of the Resource Allocation Working Party formula introduced in 1977-1978. However, judgements and assumptions about the nature of 'need' have still to be made in order to construct a usable resource allocation formula since empirical data on what is cannot provide a complete guide to what ought to be a fair distribution of resources in relation to need.
PURPOSE: To determine whether soft-copy interpretation of computed radiography (CR) in neonatal and pediatric intensive care units (ICUs) can result in a cost savings compared with screen-film radiography for the radiology department. MATERIALS AND METHODS: The actual cost of CR equipment, maintenance contracts, and the estimated cost for data storage were compared with the potential savings that could result from eliminating film, decreasing the number of librarians and lost radiographs, and improving technologists' efficiency. RESULTS: After the 1st year, net cash flow from use of soft-copy CR interpretation is projected to result in a slight savings for the radiology department but not enough to offset the capital equipment purchase price. CONCLUSION: Soft-copy CR ICU imaging will not result in a cost savings. To have a balanced net cash flow would require either more than doubling potential savings or decreasing capital equipment cost by greater than one-half. The justification for soft-copy CR needs to be judged by its effect on increasing physician efficiency and timely decisions on patient care.
In the process of reshaping the entire nation's health care delivery system, it is only natural that financing vehicles which we once considered "standard" are evolving as well. The purpose of this article is to explore some of the creative and nontraditional vehicles available to emerging health care organizations and integrated systems to access capital. This brief overview focuses on the organizational structures, financial characteristics, and financial structures of emerging health care delivery networks and providers. Select case studies provide examples of the more creative financial structures currently being employed to meet rapidly growing fund needs.
