Interlaboratory bilirubin variability.

Abstract

Stabilized liquid quality control sera, some with elevated bilirubin concentrations, were distributed to laboratories for total and conjugated (direct) bilirubin analyses. Interlaboratory variability was high; for example, results ranged from 10.9 to 24.0 mg/100 ml for a serum with a mean bilirubin concentration of 18.1 mg/100 ml. Coefficients of variation were typically 10% to 12% for total bilirubin ad approximately 24% for conjugated bilirubin analyses. It is believed that these stabilized sera, when used as quality control materials, can help reduce the unacceptably large interlaboratory variability found in this study.

[Vertical transmission causing hepatitis C in a newborn infant].

There is a case report of an intrauterine growth retarded infant who was suffering of sepsis caused by Klebsiella pneumoniae. An exchange transfusion was carried out because of hyperbilirubinaemia on the 5th day of life. 10 days later high bilirubin concentration was observed again parallel to the liver enzyme abnormalities. HCV-related antibodies were detectable in the sera of the newborn and his mother as well. The results of analysis of HCV RNA with the polymerase chain reaction in the infant and his mother suggest vertical transmission of HCV infection. This is the first case of HCV infection in the newborn caused by vertical transmission in Hungary.

Binding of lithocholate and its glucuronide and sulfate by human serum albumin.

In the present study, the binding affinities of lithocholate sulfate and glucuronide by human serum albumin were compared to that of lithocholate by equilibrium dialysis, and the binding sites of these bile acids on those of various fluorescent probes and bilirubin were also studied. The dissociation constants for the primary binding sites for lithocholate sulfate and glucuronide on human serum albumin were 0.057 and 0.24 microM, respectively, which were lower than that for lithocholate (0.82 microM). Lithocholate sulfate and glucuronide, as well as lithocholate, did not simply inhibit the binding of the site II and III fluorescent probes or bilirubin to albumin. Inhibition by these bile acids of the site I fluorescent probe binding to albumin suggested that the secondary binding sites of these bile acids are equal to site I. The results of simultaneous equilibrium dialysis using [3H]lithocholate and [14C]lithocholate sulfate indicated that these compounds have the same primary binding site. In conclusion, sulfation and glucuronidation may increase the binding affinities of bile acids to human serum albumin without changing their binding site.
