Reconstructive procedures for extremities with peripheral nerve defects.

Abstract

Surgeons who undertake reconstruction of extremities with peripheral nerve loss should be experienced enough to select the appropriate procedures for the individual patient. To gain adequate strength for a specific function, a new muscle-tendon unit usually is added to the power-train rather than redistribute muscles that have related function. Precise sensibility required precise motion, which is very difficult to achieve following peripheral nerve loss. The anticipated result should be a balanced simplification of motor and sensory function.

The effect of regional trauma care systems on costs.

OBJECTIVE: To assess cost savings from regional trauma care systems. DESIGN: Multivariate regression analysis is used to isolate the effects of regional trauma care systems on medical costs while controlling for personal and injury characteristics and other factors likely to influence medical costs. Percentage reductions in costs are translated into dollar cost savings with corrections for excluded costs and losses from premature death. SETTING: Injuries to workers filing workers' compensation lost workday claims. PARTICIPANTS: Randomly sampled workers' compensation claims from 17 states filed between 1979 and 1988 (N = 217,000). MAIN OUTCOME MEASURE: Medical payments per episode of four injury groups: lower-extremity fractures and dislocations, upper-extremity fractures and dislocations, other upper-extremity injuries, and back strains and sprains. We distinguish hospitalized from nonhospitalized claims. RESULTS: Statistical analyses reveal that states with trauma care systems have 15.5% lower costs per hospitalized injury episode. Savings average $1025 per case in 1988 dollars. Costs per episode for disabling nonhospitalized injury are 10% lower in states with trauma care systems, with savings averaging $75 per case. The largest savings are for back injuries. CONCLUSIONS: Extending trauma care systems nationwide could lower annual medical care payments by $3.2 billion. Including productivity losses due to premature death, the savings could total $10.3 billion, 5.9% of national injury costs.

[Muscle incoordination phenomena after surgical management of proximal rupture of the biceps tendon].

A significant atrophy of the biceps brachii muscle without decreasing maximal force during flexion in the elbow joint was shown at least 1.5 years after surgical refixation in 6 patients with a tendon-rupture of the long head of the right biceps. A decreased myoelectrical activity was found in the caput longum region of the muscle. The decrease correlates with an increase of spectral myoelectrical power in the region of caput breve or brachioradialis muscle. In addition compensatory recruitment processes can be supposed because of changed spectral myoelectrical power in the low-frequency bands. Changes of the morphological structure of the muscle were only found in 1 patient by sonography.

External fixation as a primary and definitive treatment of open limb fractures.

This paper analyses the medical records of 5024 injured persons treated and operated on at the Surgical Clinic of the Clinical Hospital in Osijek, during the war against Croatia (1991 and 1992). There were 1653 (33.0 per cent) admitted, while the rest were treated as out-patients. Of the admitted patients 82.7 per cent (1372) had limb injuries. In 760 (15.0 per cent) of the wounded, 1320 limb fractures caused by explosives were diagnosed, out of which 856 were of the lower and 464 of the upper limbs. One hundred and ninety-two (3.9 per cent) were treated by external fixation. External fixators were applied to 147 lower and 68 upper open long bones, respectively. Adequate reconstructive operations were performed on 106 (2.1 per cent) patients with associated injuries to important blood vessels. The stabilization of the open and unstable external fractures was achieved by external fixation, mostly at one level, regardless of the wound size. The importance of good wound cleaning has been particularly stressed as well as bone fragment reposition. Special attention has been given to the preoperative and intra-operative assessment of the limbs. In 20 patients (9.3 per cent) treated with external fixation, osteitis occurred. Twenty-one more patients (9.7 per cent), because of bad fracture healing after external fixation, were submitted to secondary internal bone fixation combined with bone grafts. Out of these, nine (42.8 per cent) developed osteitis as a consequence of the additional treatment.
