The influence of the cysteine protease inhibitor L-trans-epoxysuccinyl-leucyl amido(4-guanidio)butane (E64) on photobiological effects of tetra(4-sulfonatophenyl)porphine.

Human cervix carcinoma cells of the line NHIK 3025 were exposed to light after 18 h incubation with tetra(4-sulfonatophenyl)porphine (TPPS4) in the absence or presence of the cysteine protease inhibitor L-trans-epoxysuccinyl-leucyl amido(4-guanidino)butane (E64) followed by 1 h in sensitizer-free medium. E64 changed the photochemical properties of TPPS4 in NHIK 3025 cells, i.e., TPPS4 fluorescence yield was enhanced 2.5-fold and photochemically induced lysosomal rupture and loss of cell bound TPPS4 were inhibited. Additionally, E64 slightly (10%) reduced the sensitivity of the NHIK 3025 cells to photoinactivation. This is not likely to be due to its inhibitory effect on protease activity, but correlates with its inhibition of lysosomal rupture. The present results indicate that the release of lysosomal cysteine proteases from the lysosomal compartments are of little or no importance in the photochemical inactivation of NHIK 3025 cells when TPPS4 is used as photosensitizer.

Carcinoma in Situ

Intraepithelial neoplasia of the vulva.

Intraepithelial neoplasia of the vulva is being seen with increasing frequency. Awareness of this should prompt the clinician to carefully inspect the vulva on all patients. Reports strongly suggest a relationship between infection with human papillomavirus and vulvar intraepithelial neoplasia. The frequency with which this disease progresses to invasive carcinoma is unknown at present. However, it is obvious that it does occur. Both the warty and basaloid types of vulvar intraepithelial neoplasia are associated with HPV infection and are often associated with invasive squamous cell carcinoma of the vulva demonstrating similar morphologic characteristics. These changes are seen more often in younger women who smoke than in the older nonsmoking woman whose lesions do not appear to be HPV related. Both local excision and the carbon dioxide laser are effective for treating vulvar intraepithelial neoplasia. The choice of which approach to take depends upon the location and size of the lesion or lesions. Whichever approach is utilized, preservation of the normal vulvar anatomy and function are of paramount importance.

Carcinoma in Situ

Expression of the glycolipid globotriaosylceramide (Gb3) in testicular carcinoma in situ.

Changes in the cell membrane glycolipid composition and metabolism are frequently associated with carcinogenesis. The accumulation of globo-series glycolipids is the most notable change of the germ cell glycolipid composition observed in testicular tumours. In this study, the expression of the globo-series core-structure, globotriaosylceramide (Gb3) was investigated in the preinvasive stage of testicular germ cell tumours, carcinoma in situ (CIS). Seventeen tissue specimens with CIS and 12 samples of overt testicular tumours were immunostained with anti-Gb3 monoclonal antibody 38-13. The accumulation of Gb3 was detected in 12 CIS samples (70.6%) and in 8 invasive tumour samples (66.7%), including seminoma, non-seminoma and a combined germ cell tumour. Our findings indicate that the composition of glycolipids shift at the common preinvasive stage of testicular germ cell tumours and confirm that Gb3 is a tumour-associated antigen of testicular germ cell neoplasia.
