[Muscle function tests in practice].

Abstract

Tests about muscle-function include clinical-physical investigation and electrodiagnosis. Examination of condition and strength of muscle and test of reflexes, sensitivity and coordination are involved in the receipt of clinical-physical investigation. Electrodiagnosis is divided into determination of rheobase, chronaxie and accommodation, electromyography and stimulation myography. The first method indicates a quick differential diagnosis about conditions of innervation at the extremities, the others, especially electromyography, take more time and lead to a highly specified determination of different neurogenic and muscular disfunction.

Temporal information transformed into a spatial code by a neural network with realistic properties.

Neurons exhibit a wide range of properties in addition to postsynaptic potential (PSP) summation and spike generation. Although other neuronal properties such as paired-pulse facilitation (PPF) and slow PSPs are well characterized, their role in information processing remains unclear. It is possible that these properties contribute to temporal processing in the range of hundreds of milliseconds, a range relevant to most complex sensory processing. A continuous-time neural network model based on integrate-and-fire elements that incorporate PPF and slow inhibitory postsynaptic potentials (IPSPs) was developed here. The time constants of the PPF and IPSPs were estimated from empirical data and were identical and constant for all elements in the circuit. When these elements were incorporated into a circuit inspired by neocortical connectivity, the network was able to discriminate different temporal patterns. Generalization emerged spontaneously. These results demonstrate that known time-dependent neuronal properties enable a network to transform temporal information into a spatial code in a self-organizing manner--that is, with no need to assume a spectrum of time delays or to custom-design the circuit.

Magnetostimulation in MRI.

In national and international bodies, there is active discussion of appropriate safety regulations of levels of magnetic field strength in MRI. Present limits are usually expressed in terms of the switching rate dB/dt, but the validity of this is open to debate. Application of the fundamental law of electrostimulation is well-established, both on theoretical and experimental grounds. Application of this law, in combination with Maxwell's law, yields a very simple equation that we call the fundamental law of magnetostimulation. This law has the hyperbolic form of a strength-duration curve and allows an estimation of the lowest possible value of the magnetic flux density capable of stimulating nerves and muscles. Calculations prove that the threshold for heart excitation is much higher than those for nerve and muscle stimulations. Experimental results from us and other authors confirm the correctness of the derived laws for magnetostimulation. In light of these findings, proposed safety limits should be reconsidered.
