Retrovesical chondrosarcoma.

Abstract

Retrovesical sarcoma is rare. We report the first documented case of retrovesical chondrosarcoma. The patient underwent pelvic evisceration with creation of anileocecal conduit and colostomy. There have been no signs of local recurrence or metastasis.

Clear cell chondrosarcoma of the larynx: a case report of a rare histologic variant in an uncommon localization.

The authors describe a clear cell chondrosarcoma of the larynx. The clear cell type is a rare variant of chondrosarcoma that only twice has been reported in this localization. The light-microscopic diagnosis of the actual case was confirmed by immunohistochemical results, in particular by positive staining for S-100 protein and collagen type II, and ultrastructural findings. Loss of heterozygosity analysis demonstrated allelic loss at 9p22 and 18q21, but neither in the region of the Rb gene on chromosome 13q nor at the p53 locus on chromosome 17p where allelic loss has already been reported in chondrosarcomas. Furthermore, our molecular genetic investigations revealed a methylation of the cell cycle control gene p16, which is localized on chromosome 9p. This characteristic has been recorded previously only in high-grade chondrosarcomas. Mutations in the exons of p16, alterations of the putative tumor suppressor gene MMAC1/PTEN on chromosome 10q, or an amplification of the cyclin D1 gene (CCND1) on 11q13, which were found to be changed in other studies of chondrosarcomas, could not be demonstrated here.

Immunohistochemical staining for KIT (CD117) in soft tissue sarcomas is very limited in distribution.

We performed immunohistochemical analysis for KIT in 365 soft tissue sarcomas. Most tumors evaluated were completely negative for KIT, including all cases of leiomyosarcoma, rhabdomyosarcoma, myxofibrosarcoma, liposarcoma, solitary fibrous tumor, synovial sarcoma, dermatofibrosarcoma protuberans, schwannoma, malignant peripheral nerve sheath tumor, clear cell sarcoma, low-grade endometrial stromal sarcoma, and follicular dendritic cell sarcoma. Tumors showing occasional immunoreactivity for KIT included extraskeletal myxoid chondrosarcoma (2/20), Ewing sarcoma/malignant primitive peripheral neuroectodermal tumor (4/20), melanotic schwannoma (3/5), metastatic melanoma (4/20), and angiosarcoma (5/20). In most cases, staining for KIT was focal. Rare tumor cells showing KIT positivity were identified in a small number of other tumors. This study demonstrates very limited expression of KIT in soft tissue tumors other than gastrointestinal stromal tumors and underscores the discriminatory value of KIT immunohistochemical analysis for differential diagnosis. As some of these findings differ markedly from previous reports, it is evident again that variations in immunohistochemical technique can lead to major discrepancies in positive staining. Since treatment eligibility for selective tyrosine kinase inhibitors such as STI571 hinges on positive immunostaining, standardization and reproducibility of meaningful results are critically important.

[Crico-tracheal chondrosarcoma].

Chondrosarcoma is the most frequent mesenchymal tumor (nonepithelial) of the larynx. Radiographic studies and collaboration of pathologists have allowed that the number of cases diagnosed has increased considerably in the last few years. His privileged site is the subglotics. The late apparition of the symptoms delay the diagnosis, which is guided by CT scan and will be confirmed by histological examination of the surgical piece. The histological distinction between the different degrees of malignancy constitutes one challenge for the pathologist, so far, various criteria have been described for that aim. This biological behaviour is characterized by locoregional recurrence, in direct correlation with the grade of dedifferentiation. The treatment of choice is conservative surgical resection, and a regular follow up is obliged for early detection of recurrences and metastases. A case treated in our Center is presented, and the conclusions obtained after a literature review of the subject.
