Testing for viral hepatitis.

Abstract

Advances made within the past decade in the delineation of the structural components of the hepatitis B virus (HBV), including the hepatitis B surface antigen (HBsAg) and the hepatitis B core antigen (HBc), have resulted in the recent commercial availability of new methods of serologic testing. These tests, which include the core antigen antibody (anti-HBc) and the surface antibody (anti-HBs), facilitate the diagnosis and management of this illness by primary care physicians. In addition, a serologic test (anti-HAV) is available to diagnose or exclude hepatitis A, and research efforts are progressing in an attempt to identify a specific marker for non-A non-B hepatitis.

Hepatitis vaccination among drug users.

Hepatitis virus infections are traditionally a major health problem among drug users (DUs). Several factors may favor the rapid spread of hepatitis infection in this category of patients. HBV and HCV are easily transmitted through exposure to infected blood and body fluids. DUs often prepare and use drug solutions together. Many in the DU community are infected and this provides multiple opportunities for transmission to others. Many of these patients with chronic hepatitis virus infection are not aware of their infections and this facilitates the spread of the diseases. Viral hepatitis is not inevitable for DUs. Although multiple factors have prevented the development of vaccines for hepatitis C, both hepatitis A and hepatitis B can be prevented by immunization. The purpose of this overview is to summarize and discuss the hepatitis vaccination in this population.

A case study of the evaluation of a public health intervention.

Large public health interventions to control infectious disease outbreaks are common, but rigorous evaluation to improve the quality and effectiveness of these is rarely undertaken. Following a large community-based clinic to prevent a hepatitis A outbreak, a multifaceted and multidisciplinary evaluation was conducted involving consumers, health professionals and industry partners. The results of this evaluation were used to produce practical operational guidelines for the planning and conduct of future interventions. These guidelines have been distributed to all public health units in New South Wales and may be included in the next edition of the NSW Health notifiable diseases manual. The evaluation approach can be applied to all public health interventions across NSW and Australia to assist in the development of operational guidelines, in order to increase the quality of public health action in outbreak prevention.
