Establishing a national universal vaccination programme.

Abstract

The collaborative efforts of healthcare providers, governmental policy and law makers and the public are often needed to provide the pressure necessary to establish a national universal vaccination programme. Key initiatives for those beginning to establish such a programme are the following: secure a scientific consensus or base of support; clarify relevance of the subject to all concerned; increase awareness of everyone (providers and consumers); recruit and involve influential people (angels) for support; seek out agendas within which to review the topic; generate cost-benefit data; encourage a consensus to be reached; identify legislative pathways to be used; convince politicians of the value of such a programme; request budgeting and funding; and, finally, follow up with surveillance studies to demonstrate the benefits of the programme.

Should antihepatitis B virus core positive or antihepatitis C virus core positive subjects be accepted as organ donors for liver transplantation?

Since the introduction of liver transplantation as a routine surgical procedure for the treatment of end-stage liver disease, there has been an increasing gap between the number of available grafts and the number of patients on the waiting list. This has led transplant centers to expand the donor pool by different means. One of them has been the introduction of living donor liver transplantation. Other strategies include using less than optimal allografts from deceased donors, the so-called marginal donors, which include the use of grafts from older subjects, livers with moderate amounts of steatosis, or from donors with markers of past or current infection with hepatitis viruses who have absent or minimal liver biochemical or histologic injury. In this review, we will focus on the current use of allografts from donors with antihepatitis B core antibody and/or antibodies against hepatitis C virus in cadaveric and living donor liver transplantation.

World Wide Web hepatitis B virus resources.

BACKGROUND: Hepatitis B virus infection is a global public health issue, often under-discussed due to social prejudices related to its mode of transmission. The World Wide Web, an increasingly popular means of dissemination of health-related information, can serve continuing medical practitioner awareness and enhance public health literacy. OBJECTIVE: The authors sought to investigate the existence of, and evaluate the content of websites offering information on hepatitis B. STUDY DESIGN: Sites were selected by certain criteria (sponsor, language options, free access, validation of content by independent medical and non-medical personnel), which unavoidably rendered the lists subjective. RESULTS: At least three medical personnel-oriented websites (American Association for the Study of Liver Diseases, Clinical Care Options and The Hepatitis B Foundation sites) offer significant, up to date information on hepatitis B for clinicians. Sites offering information for the public used simple patterns as fact sheets and question-answer sets. The majority of the sites were based in the US. CONCLUSIONS: Hepatitis B virus infection is adequately represented in the web, regarding the needs of medical practitioners. Dissemination of information for the public appears in various modes, and at least at present, can only safely be achieved through simplified reports on the disease.

Evaluation of the clinical performance of the Beckman Coulter Access AbHBsII immunoassay for the detection of hepatitis B surface antibodies.

BACKGROUND: Hepatitis B virus (HBV) surface antibodies (anti-HBs) testing is useful in various clinical circumstances, including identification of HBV susceptible individuals in pre- and post-vaccination programs. OBJECTIVES: To assess the clinical performance of Beckman Coulter's anti-HBs chemiluminescence immunoassay (Access AbHBsII). STUDY DESIGN: Laboratory performances were evaluated on 1207 routine samples pre-screened with Abbott Axsym anti-HBs assay and divided into three different panels: vaccinated subjects (n=232), subjects with resolved HBV infection (n=150) and negative subjects for anti-HBs (n=825). Sera with discrepant results were resolved by an alternative method and further chart review. RESULTS: The overall concordance between Access and Axsym assays was 95.8%. The relative sensitivity, relative specificity, positive predictive value and negative predictive value were 97.8%, 98.1%, 96%, and 99%, respectively. Of the 51 discrepant results, eight were false negative by Access, fifteen were false positive by Access, including sera from seven pregnant women, two patients with acute leukemia, and four with inflammatory syndromes. CONCLUSIONS: The Beckman Coulter's Access AbHBsII assay displays satisfactory relative sensitivity and specificity performances. The assay has good precision and reliability and is technically simple and fast.
