[Focus group interview as a qualitative research method].

Abstract

Focus group research is a qualitative method with interesting properties. It is performed by planned discussion and interview with a small group of people conducted by a moderator. The participants are sampled from the study population. The aim is to obtain knowledge of the participant's considerations and ideas on a specific topic. The method is feasible in illuminating the variation of viewpoints held in a population. It is used as a single source of data or in combination with other methods. As the method provides data in a social context it is used as an alternative to individual interviews, when appropriate, or in the initial development of a questionnaire. The focused interviews is feasible in methodological triangulation or when other methods are suboptimal. Focus group interviews is considered to be of value in health research.

Using storyboarding to determine components of wellness for university students.

Using a needs-assessment technique called storyboarding to determine characteristics of health promotion and wellness among university students is discussed. Storyboarding allows students to work together as they generate ideas and eliminates the territorial defensiveness that often occurs when new ideas are suggested. Sixteen graduate students in a health education teaching methods course demonstrated the use of the technique to identify characteristics of wellness for university students. In addition, they identified content areas and outlined key programs, services, personal habits, and skills useful for enhancing university students' wellness.

The Practice Environment Project. A process for outcome evaluation.

OBJECTIVE: Evaluating the practice environment is essential to determine if the practice environment is conducive to the delivery of quality patient care. The Practice Environment Project (PEP) was created to provide a framework for current and ongoing evaluation of the practice environment. BACKGROUND: Major changes in the delivery of patient care services have focused attention on the evaluation of the effectiveness of these changes. The extent and type of change may vary throughout an institution. The identification of core variables to assess the effectiveness of these changes sets the framework for ongoing evaluation of new unit-based models for delivery of services. METHODS: In part 1, quantitative data were collected from nursing staff members (job satisfaction, collaboration with physicians, autonomy), physicians (quality of nursing care, collaboration with nurses), and patients (satisfaction with nursing care). In part 2, focus groups were held with nursing personnel to discuss factors that affected the provision of services. Unit-based action plans were developed to manage barriers to the delivery of services. RESULTS: Patients and physicians reported a high degree of satisfaction with patient care. Physicians reported a higher level of collaboration with nurses than that reported by nurses. Nurses reported a high degree of autonomy in practice, however, in other areas of job satisfaction (development and recognition), they suggested areas for improvement. CONCLUSIONS: The PEP created a mechanism to evaluate the current state-of-the-practice environment by identifying core elements for evaluation of work redesign. It also provided a framework for managing barriers that disrupted the delivery of patient care services.

Re-engineering for dramatic improvement in the medication-use process.

Planning for re-engineering the medication-use process at a university hospital is described. In summer 1992 the University of Cincinnati Hospital hired a consultant on quality to help with work-process improvement and re-engineering. By early 1993, re-engineering for patient-focused care was under way. Within a year of the implementation of patient-focused care in a pilot unit, it was realized that all departments that provided direct and indirect care would also have to undergo re-engineering. In August 1994 a retreat was held to begin the re-engineering effort in the pharmacy department. The retreat participants set as goals of the re-engineering (1) support of the hospital's strategic initiatives, (2) getting the right drug to the right patient at the right time, (3) improved patient outcomes, and (4) reducing the pharmacy labor and drug budget. Pharmacy activities for re-engineering were identified, and two planning teams were created, a pharmacy production team and a pharmacy knowledge-transfer team. The production team was made responsible for re-engineering dispensing, inspecting, producing, storing, and transporting, and the knowledge-transfer team was assigned teaching, recording, treating, and monitoring. A detailed plan was prepared for each team to follow using a framework provided by the consultant and consisting of strategy, technology, process, and personnel. Careful planning and a consultant helped a university hospital prepare for organizationwide re-engineering.
