[Spectro-temporal mapping (STM)].

Abstract

In recent years, various kinds of time-varying spectral analysis which estimate the frequency content of a signal as a function of time have been proposed. Spectro-temporal mapping (STM) is one of such methods with elaborate modification of a simple fixed period analysis. Merits and demerits of this method in clinical application are described in this chapter. Certain technical aspects pertinent to the interpretation of STM were also discussed. In conclusion, STM in present form is inferior to time domain analysis for the prediction of serious ventricular arrhythmias. But it has some usefulness to supplement time domain analysis in noise detection and application to patients with broad QRS complex.

Noninvasive biomagnetic imaging in coronary artery disease based on individual current density maps of the heart.

OBJECTIVE: In this paper we present an attempt at noninvasive imaging of distributed myocardial electrical activity in patients suffering from myocardial infarction and in healthy subjects. Although advances have been made, noninvasive three-dimensional imaging of cardiac electrophysiological activity is still in its infancy and extending our knowledge of cardiac electrophysiological properties may be a valuable guide in the treatment of patients with coronary artery disease. METHODS: Magnetic field mapping data formed the input for an inverse solution that is based on a multiple dipole model. The lead field normalized minimum norm least square criterion was applied to predefined myocardial source geometry. Current density distributions were calculated for the left ventricle during ventricular depolarization. Images from two patients with previous myocardial infarction were compared to images from two healthy subjects. RESULTS: Low regional and global current density was found in the infarction patients. Regions of low current density corresponded to infarcted segments. The images of the healthy subjects displayed less marked areas of low current density. CONCLUSION: The proposed multiple dipole model may be able to distinguish viable from scarred myocardium. A prospective clinical study should be undertaken to investigate the spatial resolution and the diagnostic performance of this method.

Spatial resolution of body surface potential maps and magnetic field maps: a simulation study applied to the identification of ventricular pre-excitation sites.

The spatial resolution of body surface potential maps (BSPMs) and magnetic field maps (MFMs) is investigated by means of an anatomically accurate computer model of the human ventricular myocardium. BSPMs and MFMs are calculated for the simulated activation sequences initiated at 35 pre-excitation sites located along the atrioventricular (AV) ring of the epicardium. Changes in the BSPMs and MFMs corresponding to different pre-excitation sites are quantified in terms of the correlation coefficient r. The spatial resolution (selectivity) for a given pre-excitation site is defined as the half-distance between those neighbouring locations at which morphological features of maps, in terms of r, become distinct (r < 0.95). It is found that, at 28 ms after the onset of pre-excitation and with no noise added, this distance +/- SD, for all sites along the AV ring for the 117-lead BSPMs, is 0.83 +/- 0.32 cm, and for the 64-lead and 128-lead MFMs it is 1.54 +/- 0.84 cm and 1.15 +/- 0.43 cm, respectively. The findings suggest that, when features of non-invasively recorded electrocardiographic and magnetocardiographic map patterns are used for identifying accessory pathways in patients suffering from WPW syndrome, BSPMs are likely to provide more detailed information for guiding the ablative treatment than MFMs. For some sites MFMs provide more information. Both modalities may provide additional assistance to the cardiologist in locating the site of the accessory pathway.

Useful lessons from body surface mapping.

Useful Lessons from Body Surface Mapping. Body surface potential maps (BSMs) depict the time varying distribution of cardiac potentials on the entire surface of the torso. Hundreds of studies have shown that BSMs contain more diagnostic and prognostic information than can be elicited from the 12-lead ECG. Despite these advantages, body surface mapping has not become a routinely used clinical method. One reason is that visual examination and sophisticated analysis of BSMs do not permit inferring the sequence of excitation and repolarization in the heart with a sufficient degree of certainty and detail. These limitations can be partially overcome by implementing inverse procedures that reconstruct epicardial potentials, isochrones, and ECGs from body surface measurements. Furthermore, ongoing experimental work and simulation studies show that a great deal of information about intramural events can be elicited from measured or reconstructed epicardial potential distributions. Interpreting epicardial data in terms of deep activity requires extensive knowledge of the architecture of myocardial fibers, their anisotropic properties, and the role of rotational anisotropy in affecting propagation and the associated potential fields.
