Abstract

The majority of patients with ovarian cancer have widespread abdominal disease. Before 1970, the response rate for patients treated with chemotherapeutic agents was less than 10%. With the introduction of cisplatin, the response rate tripled. However, patients treated with repeated courses of platinum-containing regimens may develop resistance to this form of therapy. Taxol (Bristol Laboratories, Princeton, NJ), a new drug, has been shown to be an effective agent in patients who have failed first-line or subsequent chemotherapy for the treatment of metastatic carcinoma of the ovary even in women with platinum-resistant disease.

The analysis of contingency tables with ordinal data: an application to monitoring antibiotic resistance.

Rationalization of antibiotic therapy in the management of infectious diseases is helped by a knowledge of the patterns of sensitivity and resistance of bacteria to antibiotics and their possible changes both in time and from one hospital unit to another. In this paper we present the results regarding the sensitivities of several groups of bacteria and different Units of the S.Orsola-Malpighi Hospital of Bologna in the period 1995-1997. We apply recent methods of analysis of ordinal contingency tables that rely on stochastic ordering of the rows to test the assumption that a decrease (or increase) in sensitivity of bacteria to specific antibiotics has taken place against the alternative that no such thing has happened. In most cases the results seem to indicate an increase in sensitivity rather than what was expected, namely the opposite.

Geographical stability of enterococcal antibiotic resistance profiles in Europe and its implications for the identification of fecal sources.

This manuscript describes the use of a recently developed microbial source tracking (MST) technique to determine sources of fecal bacteria (enterococci) from four separate European countries. The investigation aimed to evaluate whether the origin of bacterial populations from France, Spain, and Sweden (n = 456) could be predicted using a library of antibiotic resistance patterns of enterococci of known origin assembled in the United Kingdom (n = 2739). Bacterial isolates were exposed to a battery of 31 antibiotic tests and classified into source groups using discriminant function analysis (DFA). Results showed that while on average 72% of the U.K. isolates could be correctly classified as originating from either municipal wastewater (MW), livestock, or wild birds, only 43% of non-U.K. isolates could be successfully classified into the same source categories. The results suggested that patterns of resistance amongst isolates contained in the U.K. library were not representative of those found in the other locations and that it may not be possible to share libraries over large distances, such as those in this study. Future MST studies using antibiotic resistance analysis (ARA) in Europe may therefore require the assembly of watershed specific libraries, increasing the cost of such studies.

Plant biotechnology: ecological case studies on herbicide resistance.

The emerging field of molecular ecology aims to improve the ecological predictability of transgenic crop plants. The most widely cultivated lines are Roundup-Ready plants, which are genetically modified to be resistant to the broad-spectrum herbicide glyphosate. Recent publications demonstrate two ecological effects that were not anticipated: the widespread emergence of glyphosate-resistant weed biotypes and the formation of a metabolic herbicidal residue. Both effects appear to be due to the increased use of glyphosate rather than the genetic modification in the transgenic crop plant. With one prominent exception, opinions collected from the literature point towards a certain degree of resistance mismanagement and an inadequate testing of the ecological effects of extensive glyphosate use.
