Implementing a Physician's Workstation using client/server technology and the distributed computing environment.

Abstract

PWS is a physician's workstation research prototype developed to explore the use of information management tools by physicians in the context of patient care. The original prototype was implemented in a client/server architecture using a broadcast message server. As we expanded the scope of the prototyping activities, we identified the limitations of the broadcast message server in the areas of scalability, security, and interoperability. To address these issues, we reimplemented PWS using the Open Software Foundation's Distributed Computing Environment (DCE). We describe the rationale for using DCE, the migration process, and the benefits achieved. Future work and recommendations are discussed.

Computerization in breast imaging and diagnosis.

Computerization in breast imaging is very important for activity management and for the qualitative and quantitative control of the diagnoses. The connection through a personal computer network is necessary for the simultaneous input of more data. The activity in our center is completely managed by a specific software which controls and facilitates the input of all personal, administrative, history, instrumental and pathological data. The encoded case history permits quick and accurate data input and the relative information is easy to understand. Database information can be used to make periodical administrative and diagnostic reports and queries, to elaborate useful data for scientific reports and to manage didactic material. The growing use of computers will permit the exchange of data among breast imaging centers and also create important data banks for breast cancer studies.

Ambulatory care databases for managed care organizations.

The uses, advantages, and limitations of ambulatory care databases are discussed, and processes for extracting and using the data are described. Claims databases allow health systems, including managed care organizations, to generate descriptive statistics on patients, providers, and diseases; to conduct comprehensive cost and resource-use analyses; and to build economic models of diseases. The use of health care databases has several advantages over clinical trials, including lower data collection costs, shorter times for analysis, larger numbers of patients, and less inconvenience to patients and providers. These databases allow the effectiveness of a treatment, instead of its efficacy, to be assessed and drug-switching patterns within disease categories to be detected. This information can be used for determining the cost and outcome implications of new treatments and formulary changes, as well as for monitoring disease management programs. Limitations of health care databases include the omission of services not covered by health plans, the risk of coding errors, the absence of indicators of disease severity, and the lack of data that would assist with clinical outcome analysis. Medical and pharmacy claims data do not contain all the information contained in patients' medical records. Despite their limitations, ambulatory care databases are useful for describing patient, provider, and disease characteristics. The databases are also useful for predicting and estimating the implications of a change in the formulary, measuring the effects of treatment guidelines, and monitoring disease management programs.
