Zopiclone poisoning: tissue distribution and potential for postmortem diffusion.

Abstract

Zopiclone is the first cyclopyrrolone hypnotic and is chemically unrelated to any existing drug. The authors studied the tissue distribution and postmortem redistribution of zopiclone in a fatal suicidal overdose. A 29-year-old female weighing 64 kg had cardiac blood ethanol 153 mg% and zopiclone blood concentrations in the range 0.9-2.0 microgram/ml in 10 distinct sampling sites. After 40 h at room temperature the range was 0.9-1.8 micrograms/ml in 15 samples. Portal venous blood and urine concentrations were 3.0 and 10.5 micrograms/ml, respectively. Tissue concentrations (microgram/g) were spleen 5.8, peri-renal fat 5.0, psoas muscle 3.3, brainstem 2.8, gastrocnemius muscle 1.9, myocardium 1.6, and kidney 1.7. Eight liver samples had concentrations in the range 0.5-4.9 micrograms/g, with highest concentrations in the left lobe and adjacent to the gallbladder, probably reflecting postmortem diffusion from gastric residue (700 ml, 55.1 micrograms/ml) and bile (14.1 micrograms/ml). Of six lung samples, paired upper and middle samples had concentrations in the range 2.1-2.3 micrograms/g, the right postero-basal 1.3 micrograms/g and the left postero-basal 3.4 micrograms/g. Drug concentration in putrefactive pleural fluid was also higher on the left (2.1 micrograms/ml) than the right (1.4 micrograms/ml), probably reflecting postmortem diffusion from gastric residue. The authors conclude that zopiclone showed little preferential concentration in solid organs and consequently has relatively stable postmortem blood concentrations, with little drug redistribution artefacts. Postmortem diffusion from gastric drug residue elevates drug levels in the left lobe of the liver and left lung lower lobe.

Rapidly decellularized adipose tissue induces soft tissue vascularization in potential anatomical spaces.

Decellularized tissues provide biological cues owing to the wealth of structural and regulatory factors that promote angiogenesis, adipogenesis, and myogenesis and facilitate neurite outgrowth. Here, we demonstrated the advantages of decellularized adipose tissue (adipoECM) over defined collagen-based biomaterials for host tissue integration. Three batches of human adipose tissue were decellularized using a rapid decellularization protocol and analyzed using mass spectrometry. To assess the biological activity of the decellularized materials, adipoECM and a reference standard of care biomaterial (Integra®DRT, also containing collagen I and glycosaminoglycans) were implanted subcutaneously, but far from the wound bed (in anatomical potential spaces) of immunocompetent BALB/c mice. The mice were euthanized in the acute (1 day) and chronic (day 60) inflammatory reaction phases, followed by biomaterial excision and Masson’s trichrome immunohistofluorescence imaging of the paraffin-embedded specimens. Each batch of processed tissue passed a quality control check, showing a low level of donor genomic DNA, lack of nuclei, lipids, endotoxins, and bacterial contamination. Mass spectrometry revealed that all batches of decellularized tissue mainly contained collagen I and, to a lesser degree, collagen III, collagen IV, collagen V, laminin, fibrillin, fibronectin, tenascin, and elastin. No acute inflammatory reaction was observed in either material one day post-transplantation. At 60 days post-implantation, different cell types were detected in adipoECM specimens, whereas Integra®DRT remained acellular. Additional immunohistochemical staining of adipoECM revealed CD31-positive cells in the blood vessels. Mesenchymal (CD90 positive) and myeloid (CD14 positive) cells were also detected. Primary cell types involved in soft tissue healing and remodeling were found in the adipoECM-treated group. The ingrowth of blood vessels and mesenchymal cells confirmed the effective integration of adipoECM with host tissues. Our results demonstrate that decellularized adipose tissue implanted away from the wound bed possesses contextual biological activities that promote efficient integration with host tissues.

The influence of parity order and body condition and serum hormones on weaning-to-estrus interval of sows.

This study was performed to investigate the weaning-to-estrus interval (WEI) in primiparous and multiparous sows in relation to their weight and back-fat thickness changes and serum hormone imbalance (insulin, growth hormone, and cortisol) at the end of gestation and during lactation. Ten primiparous and ten multiparous Camborough sows, fourth to seventh parity, were used in this experiment. During gestation, daily food intake was 2.4 kg (sow commercial diet: 2.96 Mcal/kg, 16% crude protein) and during lactation all sows were fed on a wet commercial diet (3.34 Mcal/kg, 17% crude protein) ad libitum, three times per day. Blood samples were collected and back-fat thickness at the P(2) site were recorded at 6 days before and 2, 7, 14, 21 and 25 days after farrowing. Body weight was recorded on the same dates, except the date before farrowing. The WEI was also recorded. The average daily feed intake was different (P<0.05) between primiparous and multiparous sows during lactation (5.23 versus 5.72kg per day, respectively). There was a difference (P<0.05) between primiparous and multiparous sows in total percentage of back-fat thickness loss from the end of gestation until weaning (-20.18 and -9.03%, respectively). The total percentage of weight loss during lactation was slightly greater (P>0.05) in multiparous than primiparous sows. Weaning-to-estrus interval was greater (P<0.05) in the primiparous group when compared with the multiparous group (5.55 and 4.22 days, respectively). No differences were found in insulin, growth hormone (GH), and cortisol concentrations between parity groups, except on the 21st day of lactation, when GH was greater in primiparous sows. There was no correlation between percentage of total weight loss and WEI, or percentage of back-fat thickness loss (total or by periods) and WEI. There were positive correlations between GH serum concentration on the 14th and 21st days and the percentage of weight loss in the third week of lactation (r=0.46, P<0.04 and r=0.52, P<0.02, respectively), and between GH concentration on the 21st and on weaning days and WEI (r=0.54, P<0.02 and r=0.42, P<0.06, respectively). Our results indicate that the hormone change (imbalance) during lactation, mainly GH, seems to be a better parameter to explain the difference in WEI between primiparous and multiparous sows than change in body condition.
