Constrained acetabular cup disassembly.

Abstract

Dissociation of modular acetabular components in total hip arthroplasty has recently been reported. This case report describes two instances of polyethylene to metal dissociation with constrained polyethylene liners following total hip arthroplasty.

[Osteochondritis dissecans of the acetabulum].

A case of osteochondritis dissecans (OD) of the acetabulum in a 14-year-old girl is presented. The initial clinical manifestation was hip pain and restricted movements of the joint. Magnetic Resonance Imaging confirmed the diagnosis. The patient was treated conservatively, using crutches. One year after the first examination, no clinical improvements were found. X-rays of the hip showed increased sclerosis around the osteochondritis-segment, indicating incipient healing. The patient is still conservatively treated, with regular clinical examinations. The diagnosis should be remembered in the case of patients with unexplained hip-pain, and may require magnetic resonance imaging for confirmation.

Acetabulum

A study of the arterial supply of the human acetabulum using a corrosion casting method.

This anatomical study concerns the arterial supply of the human acetabulum. The purpose is to contribute to prevention of postoperative necrosis of the acetabular bone fragment after rotational acetabular osteotomy (RAO). The arterial supply of the acetabulum was determined using a corrosion casting method. The acetabular branch of the obturator artery supplies the acetabulum through the acetabular notch. The pubic branches supply the pelvic surface of the acetabulum. Deep branches of the superior gluteal artery supply the superior region and the inferior gluteal artery supplies the postero-inferior region. From these results, we recognized that the important artery for achieving successful RAO is the acetabular branch of the obturator artery that supplies the acetabulum through the acetabular notch.

Acetabular supporting ring in total hip replacement.

A series of 14 total hip arthroplasties with a Müller acetabular supporting ring including 3 primary and 11 revision arthroplasties is reviewed. This device was used in cases of acetabular bone deficiency, and bone graft was added in 11 cases. The mean length of follow-up was 39.9 months. Twelve patients had good or satisfactory results, while two operations failed. These results indicate that the acetabular supporting ring is a useful implant in cases with acetabular bone stock deficiency.
