Abstract

Laparoscopic cholecystectomy is rapidly replacing traditional cholecystectomy as the treatment of choice for patients with cholelithiasis. Complications of cholelithiasis are likewise being treated laparoscopically. Choledocholithiasis represents a major challenge for the laparoscopic surgeon. This report is of the early experience with laparoscopy as the primary treatment of common bile duct stones. Two methods are described: (a) transcystic duct choledochoscopy and (b) laparoscopic choledochotomy. Twenty-two patients were encountered with choledocholithiasis (4.8% of total cholecystectomies). Successful laparoscopic choledocholithotomy was performed in 18 cases (82%). Techniques and options are discussed.

Common bile duct wall thickening due to intramural varices diagnosed by colour Doppler ultrasound.

Cases of common bile duct (CBD) wall thickening due to varicose dilation of intramural veins consequential to portal vein thrombosis (PVT) are rare and present a considerable differential diagnostic problem, as they can mimic cholangiocarcinoma. In such cases, colour Doppler ultrasound (CD US) is a very valuable diagnostic method. There are only a few reports in literature on CD US findings of collateral circulation in a thickened CBD wall following PVT. A patient with obstructive jaundice, a tumour of the pancreatic head, CBD wall thickening, and pre-existing PVT is presented. CD US demonstrated collateral circulation of the thrombosed portal vein in the thickened wall of the common bile duct, thus ruling out a mass in the CBD. Obstructive jaundice seldom occurs with choledocal varices. In this patient, the bile duct obstruction was due to the carcinoma of the pancreatic head.

[A case of benign schwannoma in the porta hepatis].

Schwannomas or neurilemmomas are uncommon tumors arising from schwann cells of neural sheath, which most frequently affect the extremities, trunk, head and neck area. Moreover, benign schwannoma in the porta hepatis is extremely rare. Approximately, 2 cases of benign schwannoma in the porta hepatis have been reported in the literatures. We report a case of benign schwannoma in the porta hepatis occurring in a 53-year-old woman who was asymptomatic and incidentally found to have a mass. Abdominal CT scan showed a 4.5 cm-sized low attenuating mass with septum-like enhancing solid portion at porta hepatis. Abdominal exploration revealed a yellowish, cystic and encapsulated mass attached to portal vein, common bile duct, liver and duodenal wall above the hepatoduodenal ligament. There has been no evidence of recurrence during 11 months of follow-up.
