[Inflammatory thyroid diseases].

Abstract

Thyroiditis are common causes of the goitrous enlargement of the thyroid, and comprise a number of inhomogeneous disorders. The only criterion they have in common is an inflammatory infiltration of thyroid tissue. The following diseases belong to this group: Hashimoto's thyroiditis, thyroiditis de Quervain, acute thyroiditis, Riedel's thyroiditis and rare forms of thyroiditis. These diseases are reviewed with regard to incidence, etiology, pathogenesis, clinical features, diagnosis and treatment.

Quantitative microvessel density. A staging and prognostic marker for human prostatic carcinoma.

BACKGROUND: Accurate methods of predicting pathologic stage and malignant potential of carcinoma of the prostate are lacking. Such advances would represent major additions to our ability to recommend therapy and offer prognostic information. Quantitation of microvessel density (MVD) has been shown in a number of organ systems to predict tumor aggressiveness. METHODS: We and others have investigated this modality in preoperative assessment of pathologic stage and freedom from progression in patients with prostatic carcinoma. RESULTS: Unique and useful staging information has been provided and indeed quantitation of MVD has been shown to be a better predictor of the findings of radical prostatectomy than clinical stage, serum prostate specific antigen (PSA), and/or the Gleason score. Applying a variety of therapeutic modalities, quantitative MVD can be shown to provide additional information to predict who will or will not progress. Recent observations have demonstrated that quantitative MVD may be performed on a limited tissue sample provided by prostate core needle biopsy. CONCLUSIONS: Quantitative MVD may provide important staging and prognostic information for males with carcinoma of the prostate. The results may influence the recommendation for additional staging studies, provide stratification of patients for neoadjuvant or adjuvant therapy, assist in technical aspects of definitive therapy, and aid in defining optimum monitoring regimens.

[Pseudocysts of the pancreas. Diagnosis, course and principles of treatment].

Pseudocysts of the pancreas are collections of liquid more or less completely composed of pancreatic secretions. Patients with acute pancreatitis without underlying chronic pancreatitis may develop necrotic pseudocysts while those with chronic pancreatitis may develop either necrotic or retention pseudocysts. The prevalence and localization are largely dictated by the cause of the pancreatitis. Ultrasonography and computed tomography give the diagnosis and reveal the size, localization and relations of the pseudocyst. Some pseudocysts may regress spontaneously, usually within a few weeks, especially necrotic pseudocysts measuring less than 6 cm. In others, complications include acute infection, intracystic bleeding, rupture, fistulization and compression of neighboring organs. Surgery was classically proposed for all pseudocysts, but puncture-evacuation and percutaneous or endoscopic drainage have also been successful. The risk of recurrence is higher with puncture-evacuation. Today, complicated forms and unsuccessful medical treatment are good indications for surgery. Cystodigestive drainage is preferred although exeresis may be required in specific cases.

Analysis of numerical aberrations of specific chromosomes by fluorescent in situ hybridization as a diagnostic tool in breast cancer.

BACKGROUND: Biopsy by fine-needle, aspiration has become a routine technique for the diagnosis of a dominant breast mass. In this study, fluorescent in situ, hybridization (FISH) analysis of interphase nuclei allowed the authors to detect genetic aberrations that are difficult to identify by conventional cytology. METHODS: To investigate ways of minimizing misdiagnosis of the cytology of breast tumors, and detecting genetic aberrations preoperatively, the authors performed FISH with specimens obtained by fine-needle aspiration biopsies of 106 primary breast tumors (78 primary breast cancers, 2 phyllodes tumors, and 26 benign breast tumors). Numerical chromosome aberrations were investigated using 3-color FISH performed with (peri)centromere-specific probes for chromosomes 1, 11, and 17. RESULTS: Sufficient materials for FISH analysis were obtained with aspiration biopsy from 98 of the 106 breast tumors (93.4%). None of the benign tumors nor phyllodes tumors showed evidence of aneusomy for any of the 3 chromosomes. However, 71 of the 74 breast cancers (95.9%) for which sufficient material was available demonstrated aneusomy of at least 1 of the 3 chromosomes tested. The FISH analysis also suggested a possible correlation between aneusomy of chromosome 17 and metastasis to regional lymph nodes (chi-square test = 7.78; P < 0.05). CONCLUSIONS: FISH analysis of fine-needle aspiration biopsies can be a practical and useful method for the preoperative diagnosis of breast carcinoma.
