[The prepapillary venous loop in glaucoma].

Abstract

58-year-old patient presents open angle glaucoma at both eyes, with bigger intraocular pressure at right eye. This eye presents two epipapillary venous curls, the nasal one being sharper and aplatized. Asymmetrical evolution with bigger intraocular pressure and papillar excavation at this eye presenting venous curls is determined by local modifications in blood elimination. It is discussed the importance of epipapillary venous curls, with retino-ciliary venous decrease in worsening of existent glaucoma.

[Uveal effusion syndrome: clinical and ultrastructural aspects].

UNLABELLED: Spontaneous serous detachment of the choroid and ciliary body, together with bullous serous detachment of the retina (uveal effusion syndrome) is a rare but well-defined syndrome frequently associated with nanophthalmos. CASE REPORTS: Based on three cases whose one familial, clinical and ultrastructural characteristics of the syndrome are reviewed and the role of ultrasonic biomicroscopy (UBM) presented. CONCLUSION: The severe and potentially blinding complications encountered after any surgery on these eyes make the recognition of the classical clinical signs and symptoms of the syndrome particularly important. In this context, UBM represents an interesting new tool in terms of diagnosis and pathophysiology understanding.

Cutaneous metastasis of ocular malignant melanoma. An unusual presentation simulating blue nevi.

Cutaneous metastases from primary ocular melanoma are not well characterized in the literature, and the occurrence of cutaneous metastases in the absence of other systemic involvement is distinctly uncommon. A case of primary ocular malignant melanoma is reported in which the only evidence of metastatic disease initially was skin lesions that simulated cutaneous blue nevi. Knowledge of this patient's history of ocular malignant melanoma coupled with a review of the clinical and histopathologic specimens was necessary to arrive at the diagnosis. This case illustrates and characterizes an unusual presentation of cutaneous metastases of primary ocular melanoma.

Endoscopic fluorescein angiography of the ciliary body in glaucoma management.

BACKGROUND AND OBJECTIVE: The laser endoscopy system has been modified to permit high resolution intraoperative fluorescein angiograms. Because the ciliary processes can be directly viewed and photocoagulated from a limbal wound in the phakic, aphakic, or pseudophakic eye in virtually any patient, the novel opportunity to angiographically study the ciliary processes before and after various forms of ablation was created. PATIENTS AND METHODS: Eighty patients were divided into four groups and were studied under various conditions. RESULTS: The ciliary processes normally fill with fluorescein early in the angiogram and maintain hyperfluorescence throughout. Functional ciliary processes (or portions of them) are hyperfluorescent; ablated processes are densely hypofluorescent. This technique can delineate inadequately treated zones, permitting appropriate correction at the time of initial surgery. Eyes with previous transscleral cyclodestruction did not experience the degree of ciliary process ablation that the surgeon reported. CONCLUSION: Intraoperative delineation of treated and untreated ciliary processes permits accurate correlation with observed intraocular pressure response.
