This paper explores the problems and dilemmas encountered while conducting a longitudinal qualitative research study in the clinical nursing environment and highlights strategies that helped to overcome the difficulties of using observations and interview techniques. The author concludes that research within practice is essential to ensure successful implementation.
This study examined the practice of nasogastric feeding on one intensive care unit (ICU). Data collection took place over 135 feeding days on 26 patients who met specified criteria. A generally inconsistent approach to nasogastric feeding was discovered. Some patients received considerably more calories than required while others received significantly fewer. Calories were also over-prescribed, contrary to the findings of other studies. Despite over-prescribing, patients either received their prescribed calories or high percentages of their prescribed calories at levels higher than those previously reported.
This is the first of our reports from the King's Fund Development Centre, highlighting units which have developed experience in a particular aspect of nursing care and are keen to disseminate their work. This report reviews work being done in nursing units to improve the links between primary and secondary care, and is based on a Nursing Development Network day held last year.
