Abstract

Prompted by evidence that the recently-adopted arrangements for ongoing education among established general practitioners are unsatisfactory, the first of a pair of papers examined the theoretical basis of continuing education for general practice and proposed a model of self-directed learning in which the experience of established practitioners is connected, through the media of reading, reflection and audit, with competence for the role. In this paper a practical, systematic approach to self-directed learning by general practitioners is described based on the model. The contribution which appropriate participation in continuing medical education can make to enhancing learning from experience is outlined.

The inversion In(2L)t impacts complex, environmentally sensitive behaviors in Drosophila melanogaster.

Genetic variation in behavioral traits allows organisms to respond and adapt to environmental challenges. Genetic variation in behavior is often affected by many genes and thus has a complex genetic basis. Inversions, the reorientation of genes along the chromosome, tightly link genetic variants together because they suppress recombination. Therefore, inversions are believed to have a major impact on phenotypic variation because they combine the effects of multiple genes, which can pleiotropically alter multiple aspects of behavior. This study investigates how the inversion In(2L)t, found in Drosophila melanogaster populations around the world, impacts different aspects of behavior in an environment-sensitive manner. We test the activity, foraging, and startle-induced behavior of flies with different In(2L)t genotypes across sex and temperatures. We observe that Drosophila homozygous for In(2L)t sleep less frequently, spend more time away from a food source, and have a longer duration of startle response. Additionally, the impacts of In(2L)t on aspects of behavior can be sex-specific and are largely consistent across temperatures. Taken together, our research demonstrates that inversions can regulate aspects of behavior, and suggests hypotheses explaining the distribution of In(2L)t across space and time.

Temporal stability in the distribution of behavioral states for individual human fetuses.

Although behavioral state analyses have been useful in differentiating between groups of normal and at-risk fetuses, the large between-subject differences in the percent time spent in the various behavioral states poses a major obstacle in identifying abnormal neurological functioning in individual fetuses. Does this variability represent a true difference in state organization between fetuses, or does it simply reflect individual fluctuations in state control at the time of observation? To answer this question, we examined each of 33 human fetuses for 4 h on three separate days between 38 and 40 weeks gestation. The percent time spent in each behavioral state and in transition and insertion periods was determined for each of the three 4-h study sessions, and within-subject analysis of variance was performed to obtain an objective measure of state profile consistency for each fetus. We found that, on the average, fetuses exhibited remarkable within-subject consistency in their state profiles. However, even among this group of low-risk fetuses, there were significant differences in the degree of state organization achieved by individual fetuses. These findings, which indicate the existence of a well-developed central nervous system before birth, suggest that individual differences in the consistency of behavioral state profiles may be indexing important between-subject differences in neurological development.

Clinical effects of cholesterol supplementation in six patients with the Smith-Lemli-Opitz syndrome (SLOS)

We describe the clinical effects of cholesterol supplementation in 6 children with the RSH-"Smith-Lemli-Opitz" syndrome (SLOS). The children ranged in age from birth to 11 years at the onset of therapy, with pretreatment cholesterol levels ranging from 8 to 62 mg/dl. Clinical benefits of therapy were seen in all patients, irrespective of age at onset of treatment, or severity of cholesterol defect. Effects of treatment included improved growth, more rapid developmental progress, and a lessening of problem behaviors. Pubertal progression in older patients, a better tolerance of infection, improvement of gastrointestinal symptoms, and a diminution in photosensitivity and skin rashes were also noted. There were no adverse reactions to treatment with cholesterol. This preliminary study suggests that cholesterol supplementation may be of benefit to patients with the SLOS.
