[Panic attacks and panic disorders in the child].

Abstract

Panic disorders in adults have been the object of many studies. They are better known in adolescents but have been noticed insufficiently in children although panic disorder is potentially dangerous at that age. After demographic considerations, the clinical aspects are examined and illustrated using two detailed observations.

The influence of culture and context on the pre- and post-migration experience of school-aged refugees from Central America and Southeast Asia in Canada.

Pre- and post-migration contexts of refugee children from Central America and Southeast Asia were compared. The results suggest that the culture of origin radically modulates the relationship between the pre-migration experience and the developing post-migration universe. In the case of the Central American children, the state-sponsored violence suffered in the country of origin is strongly associated with subsequent family conflicts, whereas in the case of the Southeast Asians, conflict is associated with an active social network in the community of origin, suggesting that there is a delicate balance between the support provided and the burden imposed by the extended family.

Behavioral issues and implications with pediatric feeding disorders.

Behavioral and interactional aspects of feeding problems are considered within a "total" child approach. Basic principles and guidelines for optimal feeding relationships are presented within a developmental framework. Factors that can underlie behavioral components are discussed. Children with limited appetites and food refusal typically demonstrate poor weight gain, or even weight loss, and are diagnosed as failure to thrive (FTT). Management of their food avoidance is designed to make the eating process enjoyable and nutritionally adequate.

A systematic replication of the prescriptive treatment of school refusal behavior in a single subject.

We employed a multiple-baseline design to test the efficacy of a recently developed approach for reducing school refusal behavior. This approach uses a systematic method for conducting a functional analysis for the target behaviors and suggests specific intervention strategies based upon this analysis. To maximize external validity, the intervention was tested using a systematic replication strategy, whereby only the major conceptual elements of the intervention were retained from previous applications. The subject was a 10 year old girl who attended school with great difficulty and resistance and who was assigned DSM-III-R diagnoses of separation anxiety disorder and social phobia prior to treatment. Following 8 weeks of treatment, she showed marked reductions in problem behavior and no longer met criteria for any anxiety disorder diagnoses. The results highlight the importance of the use of functional analytic strategies to effect behavior change.
