Warm heart surgery: concept, concerns, and future course.

Abstract

Hypothermia has been considered a prerequisite for decreasing oxygen consumption and providing myocardial protection. The decrease in myocardial oxygen consumption however, is more closely related to the state of electromechanical arrest than to hypothermia. The rationale for warm blood cardioplegia is based not only on electromechanical quiescence but equally on facilitation of O2 delivery. This report outlines the basis for the concepts, the various concerns and the possible future directions of warm blood cardioplegia.

Intermittent warm blood cardioplegia in the surgical treatment of congenital heart disease: clinical experience with 1400 cases.

OBJECTIVE: To analyze our experience with warm blood cardioplegia in pediatric cardiac surgery. METHODS: We used intermittent (every 15 minutes after initial injection) warm blood cardioplegia in the treatment of 1400 patients. Results were retrospectively compared with those of 950 patients treated with cold blood cardioplegia. The following parameters were analyzed: (1) hydric balance of cardioplegic solution; (2) resumption of rhythm after aortic crossclamp removal; (3) duration of mechanical ventilation, intensive care unit stay, and incidence of mortality in 4 selected diagnostic groups: ventricular septal defect, tetralogy of Fallot, atrioventricular septal defect, and transposition of the great arteries. These 4 groups, treated with warm or cold cardioplegia, were comparable with regard to age, weight, crossclamp times, and percent with Down syndrome; (4) troponin level at 12 hours after aortic crossclamping; and (5) duration of intensive care unit stay for the 1400 patients. RESULTS: Warm versus cold cardioplegia: negligible fluid addition with warm cardioplegia compared with blood loss/prime dilution induced by cold cardioplegia; spontaneous resumption of sinus rhythm in 99% versus 77% of patients (P < .001); shorter duration of ventilatory support in each diagnostic group, significant in all cases; smaller increase in troponin in each group (P < .05). Incidence of early death was not different in the 2 groups. For the whole group, duration of the intensive care unit stay was less than 48 hours in 86% versus 75% (P < .001). CONCLUSION: In our experience, normothermic cardioplegia has not generated any particular inconvenience and its use was contemporary, with improved outcomes.

Sudden cardiac death: directing the scope of resuscitation towards the heart and brain.

BACKGROUND: The fundamental goal of cardiopulmonary resuscitation (CPR) is recovery of the heart and the brain. This is best achieved by (1) immediate CPR for coronary and cerebral perfusion, (2) correction of the cause of cardiac arrest, and (3) controlled cardioplegic cardiac reperfusion. Failure of such an integrated therapy may cause permanent brain damage despite cardiac resuscitation. METHODS: This strategy was applied at four centers to 34 sudden cardiac death patients (a) after acute myocardial infarction (n = 20), (b) "intraoperatively" following successful discontinuation of cardiopulmonary bypass (n = 4), and (c) "postoperatively" in the surgical ICU (n = 10). In each witnessed arrest the patient failed to respond to conventional CPR with ACLS interventions, including defibrillation. The cardiac arrest interval was 72 +/- 43 min (20-150 min). Compression and drugs maintained a BP > 60 mmHg to avoid cerebral hypoperfusion. Operating room (OR) transfer was delayed until the blood pressure was monitored. In four patients femoral bypass maintained perfusion while an angiographic diagnosis was made. RESULTS: Management principles included no repeat defibrillation attempts after 10 min of unsuccessful CPR, catheter-monitored peak BP > 60 mmHg during diagnosis and transit to the operating room, left ventricular venting during cardiopulmonary bypass and 20 min global and graft substrate enriched blood cardioplegic reperfusion. Survival was 79.4% with two neurological complications (5.8%). CONCLUSIONS: Recovery without adverse neurological outcomes is possible in a large number of cardiac arrest victims following prolonged manual CPR. Therapy is directed toward maintaining a monitored peak BP above 60 mmHg, determining the nature of the cardiac cause, and correcting it with controlled reperfusion to preserve function.

Glucose-insulin-potassium echocardiography detects improved segmental myocardial function and viable tissue shortly after acute myocardial infarction.

Previous studies showed that glucose-insulin-potassium (GIK) increases cardiac output in patients after cardiac surgery and improves segmental myocardial wall motion. We hypothesized that GIK improves regional wall motion, detects contractile reserve, and predicts functional recovery at follow-up to a similar extent as low-dose dobutamine (LDD) in patients with recent myocardial infarction. Forty-one patients underwent LDD and GIK echocardiography. Data were analyzed according to a 13-segment model. Segments were scored from 0 (normokinesia) to 2 (a-/dyskinesia). Wall motion score index was calculated for baseline and intervention. During GIK, wall motion score index improved from 0.60 +/- 0.25 to 0.39 +/- 0.20 (P < .0001) and from 0.58 +/- 0.25 to 0.39 +/- 0.21 (P < .0001) during LDD. Overall agreement between GIK and LDD echocardiography to detect contractile reserve (improvement of segmental function by >or= 1 point) was 93% with a kappa value of 0.88. Sensitivity, specificity, and positive and negative predictive values of GIK echocardiography to predict functional recovery at follow-up (mean time to follow-up, 13 months) were 74%, 84%, 85%, and 72% respectively, and values were similar to LDD echocardiography. Thus, GIK infusion improves regional left ventricular function and allows the detection of myocardial viability to a similar extent as LDD in patients shortly after infarction.
