Abstract

This paper outlines the realization of rich retrieval facilities and comprehensive data administration in an existing, large, integrated Hospital Information System (HIS) using SQL as a query-language and using a large collection of formal data descriptions stored in the Data Dictionary and Directory System (DD/DS). It is emphasized that the approach to enhance the access to the database is more important than the technology. Nevertheless, the facilities are presented that support non-routine retrieval. The Data Dictionary and Directory System functions as a catalog of the information system. The vehicle that facilitates the communication with the database is an implementation of SQL-in a number of appearances. It is stressed that only full benefit can be gained through a combined action of a large integrated HIS, a complete and correct DD/DS, and strong retrieval functions.

[Plea for the retention of the Dutch National Medical Registration (LMR) to provide reliable information regarding public health and healthcare].

Since 1963, information on each hospital admission has been collected from all general and university hospitals in the Netherlands (the 'Landelijke Medische Registratie'; LMR). This information includes, among others, one main discharge diagnosis and up to nine secondary diagnoses made during admission, the duration of the hospital stay, death during admission, times of admission and discharge, type of specialist who treated the patient, operations and related therapeutic procedures. This information system is not biased by financial incentives and is therefore a good resource for epidemiological studies and drug-safety research. As of 2005, a new type of administration has been introduced that records diagnosis-therapy combinations for reimbursement purposes. Because the reimbursement of closely related diagnosis-therapy combinations may differ, the system is not only sensitive to fraud but will probably also jeopardise the validity of epidemiological research with such data. Therefore, the LMR system cannot be missed.

Implementing a new ADT based on the HL7 version 3 RIM.

The University Hospitals of Geneva (HUG) are the result of the merge of six hospitals into one single organization. While a true fusion of the management has been effectively done, it was not the case 5 years after for several databases, and in particular the ADT (admission, discharge, transfer). In order to truly realize the fusion, a new ADT service has been built using state of the art technology and standards in order to replace the existing seven services. This paper presents the results of the redesign and development of the new ADT service. The data model, based on HL7 RIM, is described and the technologies selected are presented. Finally, a status after 1 year of production is presented.

The product and process of referral: optimizing general practitioner-medical specialist interaction through information technology.

With the growing complexities of health care delivery in western industrialized countries, the need for inter-organizational communication is increasingly emphasized. In this paper, we focus on a system - ZorgDomein - that was developed to optimize GP-medical specialist communication. Contrary to the notion of 'shared' or 'integrated care' that often assumes a 'seamless' health care, we will focus on the negotiated order of GP-specialist cooperation, showing the precarious localized arrangements that allow both a bridging and a separation of professional activities concerning patient care. Furthermore, we analyze how ZorgDomein changes the arrangements to maintain a working order. The main focus of the article is on the way GP-specialist referrals are on the one hand conceptualized as discrete events of information sharing, while on the other hand are part of a process of care. We will argue that in standardization attempts by national and local actors, embodied within the technology, information exchange between first and secondary care is made into a product. This conceptualization and materialization neglects the process in which this information comes about or is being created. We discuss the consequences of this for the design and use of the technology.
