Dynamic interplays during rhinoplasty.

Abstract

One of the complicating factors in rhinoplasty procedures is its intricate dynamics. Not only is there a complex interaction between the soft tissue and nasal bone frame on each nasal zone, there is an intriguing interaction between the different zones of the nose and the surrounding facial structures. Understanding these interplays is mandatory to proper design and execution of the aesthetic surgical plans. This article has covered the dynamic effects that occur along the different nasal zones, elucidated the control that the nasal frame has on the soft tissue, and reviewed the different ways in which the intended aesthetic objectives can be obtained.

Nasal anatomy and reconstruction.

Reconstruction of the nose provides many challenges owing to its differing subunits and varied surfaces. In this article, the nasal anatomy, anesthesia, and some of the reconstructive options available to the dermatologic surgeon are reviewed. These reconstructive options are based on the subunit and depth of the defect. This subunit approach may provide a framework of decision making and options that will help the dermatologic surgeon in reconstruction of defects on this unique anatomic structure.

The lips. Excision and repair.

Dermatologists commonly encounter lesions arising on the lips that require excision either for biopsy or treatment. Several surgical techniques used for excision and repair of the lip in the treatment of these conditions are reviewed. Topics covered include the importance of the lip, lip anatomy as it pertains to cutaneous surgery, techniques for obtaining lip anesthesia, partial-thickness and full-thickness lip excision and repair, surgical vermilionectomy, and carbon dioxide laser vermilionectomy. Emphasis will be placed on those surgical techniques that might commonly be used in a general dermatology practice.
