Sertraline overdose.

Abstract

OBJECTIVE: To determine the clinical presenting signs and symptoms in presumed overdoses of sertraline, a recently approved antidepressant. METHODS: A prospective study involving five western regional poison control centers was performed to evaluate the clinical manifestations of presumed sertraline ingestions (overdoses). Information about calls pertaining to sertraline ingestions was recorded on a standard data collection form. Data including subject age, sex, amount ingested, coingestants, time interval to evaluation, vital signs, presenting signs and symptoms, ECG abnormalities, treatment given, disposition, and length of stay in the ED were collected over a nine-month period. RESULTS: Of 42 ingestions reported, two were adverse reactions to normal doses and 40 were overdoses. Stated amounts of sertraline ingested ranged from 50 to 8,000 mg (mean 1,579 mg). Mean patient age was 35.3 years (range 1 to 69 years). Mean interval to presentation was 3.0 hours. Seventeen of the 40 patients ingested sertraline alone. Of this subgroup, ten had no sign or symptom. The most common abnormalties reported in isolated sertraline overdose were tremor, lethargy, and nausea. Less common findings included agitation, confusion, and vomiting. There was no significant morbidity in this subgroup of presumed isolated sertraline ingestion. Of the 23 patients who ingested other medications along with sertraline, four were asymptomatic. Benzodiazepines and alcohol were the most frequently coingested substances. Lethargy, nausea, dry mouth, and mydriasis were the most common features reported in this group. Treatment included lavage, activated charcoal, and observation. Twelve patients were admitted for 24-hour observation, none had an adverse outcome. Of the patients released from the ED, the mean length of stay was 3.9 hours. CONCLUSION: Sertraline is commonly taken in overdose with other medications or alcohol. The signs and symptoms that develop in association with an overdose of sertraline appear to be minor and of short duration.

Highly selective 4-amino-1,8-naphthalimide based fluorescent photoinduced electron transfer (PET) chemosensors for Zn(II) under physiological pH conditions.

The design and synthesis of two novel fluorescent sensors based on the photoinduced electron transfer (PET) concept, and , for the detection of zinc under competitive media is described. These sensors are based on the 4-amino-1,8-naphthalimide fluorophore, which has an absorption band centred at 450 nm and emits in the green with lambda(max) approximately 550 nm. By functionalizing the chromophore with a simple benzyl or ethyl-aryl based iminodiacetate receptor at the 4-position, both high selectivity and sensitivity were achieved for the sensing of Zn(II) over other competitive transition and Group I and II metal ions. These sensors were also shown to be pH independent, with a pKa of 2.3 being determined for , which allows these to be used in highly competitive pH media. Upon sensing of Zn(II) the fluorescence emission spectrum is 'switched on' demonstrating the suppression of PET from the receptor to the fluorophore. For , the sensing of Zn(II) was achieved with Kd = 4 nM when measured in pH 7.4 buffered solution, in the presence of 1.1 mM of EGTA.

A stronger DNA damage-induced G2 checkpoint due to over-activated CHK1 in the absence of PARP-1.

Poly(ADP-ribose) polymerase 1 (PARP-1) is involved in multi-pathways to respond to DNA damage. Lack of or inhibition of PARP-1 activity leads to slow progress of cell cycle and sensitization of cells to different stresses. Recently, it was reported that besides the Ku dependent main nonhomologous end joining (NHEJ) pathway, there is a PARP-1 dependent complementary NHEJ pathway to repair DNA double strand break (DSB). Here we show that compared with PARP-1+/+ cells, PARP-1-/- cells display a much stronger G2 checkpoint response following ionizing radiation (IR). Treatment with Chk1 siRNA abolishes the stronger G2 checkpoint response and sensitizes PARP-1-/- cells to IR. These data indicate that the stronger G2 checkpoint response in PARP-1-/- cells is CHK1 dependent, which protects cells from IR induced killing. We also show that 4-Amino-1,8-naphthalimide (4-AN, inhibitor of PARP) but not methoxyamine (inhibitor of base excision repair (BER)), affects IR induced G2 arrest and cell sensitivity in PARP-1+/+ cells, resulting in the phenotypes similar to those of PARP-1-/- cells. These results indicate that DSB repair from the complementary NHEJ pathway of PARP-1, but not single strand break (SSB) repair from the BER function of PARP-1, may play an essential role in the over-activated CHK1 regulated G2 checkpoint response and radiosensitivity in PARP-1-/- cells.

Determination of 1-naphthylamine by using oscillating chemical reaction.

A simple and rapid analytical method for determining 1-naphthylamine was proposed by perturbation with different amounts of 1-naphthylamine on the classical Belousov-Zhabotinskii (B-Z) oscillating chemical system. The results show that the changes both in oscillating period and amplitude were linearly proportional to the logarithm of the concentration of 1-naphthylamine (logC) very well ranging from 7.08x10(-5) to 7.08x10(-6) molL(-1) and 7.08x10(-5) to 1.0x10(-6) molL(-1), with the corresponding regression coefficient are 0.9957 and 0.9922, respectively. For the later, a lower detection limit of 5.64x10(-9) molL(-1) was obtained. Influence of injection point, temperature and reactant variables on this oscillating system was also investigated in detailed. The results obtained were compared with other determination methods. A possible reaction mechanism was interpreted by using bromide ion selective electrode to inspect the concentration change of Br(-) ion in the oscillating process.
