Professional development support for community nurses.

Abstract

This article presents the outline of a programme developed to support quality nursing care and provide professional development for staff nurses in a grade mixed, community nursing team. The initiative has been a joint venture between First Community Health NHS Trust and the University of Wolverhampton, School of Health Sciences, and shows the value of working together to establish a sound underpinning for practice.

The development and application of a public health skills assessment tool for use in primary care organisations.

The public health skills assessment instrument described in this paper was developed to provide a key group of UK primary care practitioners (health visitors) with a personal development planning tool and researchers with a robust assessment tool for use in evaluations of innovative local public health programmes. Pilot testing with over 120 practitioners has provided evidence of its reliability and validity as a research instrument as well as yielding useful insights for public health educators, practitioners and policy-makers. Factor analysis confirms 10 core competency domains for health visitors which are remarkably consistent with those recently identified for UK public health specialists, interpreted here as grounds for optimism in the greater co-ordination between strategic and front-line approaches to public health work in primary care. However, results also confirm earlier findings indicating low levels of skill amongst health visitors in tackling health inequalities through interventions such as community development, raising questions about their capability to work effectively in new roles proposed by the Department of Health.

Higher level practice: degree of specialist practice?

Higher level practice (HLP) is the determinant characteristic of any maturing profession. Nursing should make a quantum leap with the development of HLP if the patient, the nurse, the context of caring and the acquisition of expertise are at the forefront of such growth. This study shows the matching of nursing concepts with nursing context is at the heart of HLP. It also shows that nurses are developing their own understanding of HLP which is different from the UKCC functional criteria. The intention here is to contribute to the development and understanding of HLP. Seven key concepts emerge in the definition of HLP. These concepts are unpacked to reflect the possible criteria, which could be used in the practice, teaching, assessment, and further refinement of HLP. A series of thought provoking recommendations are made for the Nursing and Midwifery Council to consider.A learning module entitled "Higher Level Practice" anchored in the epistemology of practice has been developed and successfully implemented by the author within the MSc Nursing Practice course attended by senior and consultant nurses.
