Abstract

PATHOGENESIS: Psoriasis is a proliferative disease of the epiderm which leads to major disorders in keratinocyte differentiation. The pathogenesis remains largely unknown. It has been hypothesized that intrinsic alterations of the keratinocyte or local or general immune disorders could be involved. GENES IMPLICATED: Psoriasis is a genodermatosis which is revealed or aggravated by environmental factors. The nature and chromosomal localization of the genes involved is currently under study. LOCAL TREATMENT: Over the last years, use of vitamin D has largely replaced former topical corticosteroid therapy. Phototherapy using narrow spectrum ultraviolet B is currently the standard treatment for extensive psoriasis. GENERAL TREATMENT: Methotrexate, retinoids and cyclosporin are effective in severe psoriasis but have side effects which limit their indications.

[Immunomodulation with fumaric acid. Systemic therapy in psoriasis].

The treatment of psoriasis vulgaris with fumaric acid esters has been controversial for more than 30 years. Recently the fumaric acid derivatives are marketed antipsoriatics in many European countries. In this paper the clinical efficacy, the side effects as well as the mode of action of these highly potent substances are summarized.

[Intralesional methotrexate injection: an effective time and cost saving therapy alternative in keratoacanthomas that are difficult to treat surgically].

A 53 year old female patient developed a keratoacanthoma, present on the left side of her nose for two months. After repeated unsuccessful trials with cryotherapy, she was treated with intralesional injection of methotrexate performed twice with a weekly internal. This led to complete resolution within 5 weeks. Although controlled studies on the effect of methotrexate in keratoacanthoma are not available, the rapid regression and the good cosmetic result suggest that this therapy may play a beneficial role in treating these lesions especially in problem locations.

Tanning preparations.

Increasing consumer awareness as to the hazards of UV light should fuel ongoing interest in self-tanning products. As a result of the benign toxicologic profile of DHA, products containing DHA represent a safe alternative to UV-induced tanning. The results obtained with these products depend on the final formulation, the individual application technique, and the consumer's complexion type. Greater experience in formulation combined with increasing sophistication on the part of the consumer should lead to continuing growth in interest and satisfaction with the use of DHA-containing sunless or self-tanning products. Individuals need to be informed that these products do not offer significant protection against UV-B. If formulated with standard sunscreens, consumers should be cautioned that the duration of UV protection is more short-lived than the color change.
