Scabies: case presentation.

Abstract

Scabies is a skin infestation of a mite which is spread by skin-to-skin contact and causes an intense generalized intractable pruritus. This case presentation demonstrates how scabies can be mistaken for other skin conditions, and the importance of proper diagnosis, treatment, and followup.

Prenatal detection of a Wilms' tumor.

Kidney tumors occur very rarely in utero but when present, they are most often congenital mesoblastic nephroma. A newborn boy was transferred to our hospital with a history of fetal renal mass which proved to be a Wilms' tumor. The clinical history, imaging results, and differential diagnosis are presented with a discussion of neonatal Wilms' tumor. While imaging may not specify the exact diagnosis, it provides staging and anatomic information for the surgeon and the oncologist.

Differential diagnosis: a reasonable clinical approach.

The reasonable clinical approach to the patient who has low back pain includes a search for any of the red flags that would prompt consideration of additional diagnostic testing for serious underlying disease. The common patterns of back pain would be explored in an effort to categorize the patient's symptoms and to begin formulation of a diagnostic impression. Re-evaluation of the patient in light of his response (or lack of response) to these measures is an essential component of the clinical assessment. Finally, some consideration must be given to the various biopsychosocial factors that may affect prognosis, particularly in patients who have more chronic pain and disability-related issues.
