Introducing a bereavement support programme in ICU.

Abstract

This paper outlines the development and introduction of a support programme for bereaved relatives in the intensive care unit. The programme is distinguished by nursing follow up of relatives following the patient's death.

A synthesis of psychological interventions for the bereaved.

Several interventions have been implemented to address the adverse psychological and physical consequences associated with bereavement. In this review, we summarize four major theories of bereavement, present a qualitative review of bereavement intervention studies, and assess the overall effectiveness of bereavement intervention studies in a quantitative meta-analysis. Summaries of the theories are drawn from published theoretical works. The qualitative and quantitative reviews were based on searches of Medline, PsychINFO, and Dissertation Abstracts International databases using the keywords "bereaved" and "bereavement." Overall, the interventions were largely methodologically flawed, rarely specified what theory of bereavement they were testing, and slowed surprisingly weak effect sizes. Possible interpretations for the small effect sizes are discussed, and future directions are outlined.

The development of evidence-based nursing.

This paper argues that the current conception of evidence-based medicine has its limitations in the promotion of research which effects the quality of service in any health care system. It also poses something of a difficulty for the development of evidence-based nursing in particular. This paper advocates the more broad based concept of evidence-based practice and discusses its potential for addressing theory/practice problems and the uptake of nursing research. The broader conceptualization of evidence-based practice focuses on the integration of available evidence and the tacit knowledge of the investigator. An evidence-based practice project undertaken in Hong Kong is outlined as this provided the basis of many of the conclusions made in this paper. Three vignettes are given in order to demonstrate the nature of the evidence-based practice projects which have been conducted. The critical elements of evidence-based practice projects are outlined. Finally issues concerning the process of generating evidence, the relationship to continuous quality improvement and the cost effectiveness of evidence-based practice are discussed in more detail.

Grief work of HIV positive persons and their survivors.

Grief and bereavement following the death of a loved one present clients with significant challenges to the physical, emotional, and spiritual health and well-being. The grieving process for the person with HIV infection may be compounded by shame, stigma, and lack of social support. For communities with the highest prevalence of HIV infection, AIDS-related losses have resulted in mounting, and often overlapping, bereavements. Addressing the impact of these bereavements on the physical and emotional health of clients and their survivors is imperative to designing appropriate interventions to alleviate the potential adverse health outcomes associated with epidemic loss. Facilitating the grief work of HIV positive persons and their loved ones serves to assuage unhealthy sequelae, promote effective coping skills, and provide opportunities for personal growth.
