Autonomy in endemic goiter.

Abstract

The occurrence of hyperthyroidism in many individuals after introduction of iodine prophylaxis in endemic goiter areas can have dramatic consequences for the affected individuals. It indicates that in such individuals the increase of serum thyroid hormone level in response to iodine supplementation does not exert its normal negative feedback on thyroid activity, ie, that in such individuals some thyroid tissue has become autonomous. In this short review we summarize what is known about the possible mechanisms, cause, diagnosis, and consequences of thyroid autonomy.

Goiter and other iodine deficiency disorders: A systematic review of epidemiological studies to deconstruct the complex web.

A systematic review of the available literature on goiter and other iodine deficiency disorders (IDDs) was carried out with the aim of analyzing available evidence and providing inputs to the policy makers and program formulators regarding the entire issue. The findings point to major issues such as the following: methodological issues in epidemiology of goiter and other iodine deficiency disorders (IDDs); lacunae in causal linkages; inadequate attention to multicausality; flawed assessment of the impact of intervention, i.e., iodized salt; and harmful effects of iodine not given due cognizance. Most of the research to date has been unidirectional and does not provide comprehensive data on all aspects of IDDs. To further compound the issue, many independent researchers, on finding something different from the existing dominant paradigm (iodized salt as panacea for goiter) have tended to ignore these in their final conclusions and recommendations. Thus, evidence from this systematic review demonstrates enough basis to start a debate on the entire issue, recognizing opposing research findings while continuing with the present strategy. This imposes specific problems and necessitates area-specific solutions instead of a universal solution, which apart from being less effective may be harmful in the long run.

Iodine content of various salt samples sold in Mangalore--a coastal city with endemic goitre.

OBJECTIVE: Iodine deficiency disorders continue to be high despite ban on sale of non-iodised salt. In order to assess the effectiveness of salt iodisation programme, a study was undertaken to assess the iodine content of salt samples sold in Mangalore. MATERIAL AND METHODS: One hundred salt samples were randomly collected from shops in Mangalore and iodine content measured using MBI iodine testing kits. RESULTS: Only 39 samples had adequate iodine of 15 ppm or more. There was a statistically significant difference in the salt samples with adequate content in rural versus urban areas (46.6% and 27.5%, respectively, with p < 0.05). Branded samples had significantly higher iodine content than non-branded salt (p < 0.05). Salt sold at Rs.2/- per kg had significantly lower iodine content than the salt sold at Rs.3-4/- per kg (p < 0.01). CONCLUSION: Salt iodisation programme is poorly implemented in Mangalore probably due to improper iodisation, inadequate storage, hot and humid weather conditions and lack of public awareness. Public education and education of shop owners regarding their role in prevention of iodine deficiency disorders may be of value in this direction.
