Mastering the removable partial denture. Part one: Basic reflections about construction.

Abstract

Because the arrangement of the remaining dentition may vary tremendously, the design of a removable partial denture is complicated with regard to function, esthetics, hygiene and oral comfort. Part One of this article will discuss how disassembling the RPD into its structural components and defining their functions will help the dental technician with RPD planning. Part Two will break down the components of a combination case using the same systematic approach.

Implant position record and implant position cast: minimizing errors, procedures and patient visits in the fabrication of the milled-bar prosthesis.

This article describes a new rationale and method involved in the fabrication of a patient detachable prosthesis supported by a milled bar. This simple procedure improves prosthesis retention. The overdenture is processed directly over a milled bar, which provides an intimate relation between the bar and the acrylic resin denture base to create resistance against rotational and lateral forces acting on the prosthesis. Incorporating simple and predictable attachments, with low maintenance needs, controls resistance to dislodgment along the path of insertion of the prosthesis. The concepts used in the fabrication of the milled bar include an implant position record (IPR) and an implant position cast (IPC) to reduce the need for time-consuming procedures, such as sectioning the cast bar and soldering it to make it fit the abutments. This procedure also reduces the number of patient visits required of the completion of the prosthesis.

Improper guiding planes are commonly used with conventional removable partial denture cast clasps. Short guiding planes with improper proximal plates decrease retention and cause repetitive traumatic movement of abutments. This article discusses the importance of guiding planes and proximal plates for conventional tooth-supported removable partial dentures with circumferential clasp. The retention force of the clasps was analyzed geometrically. If the guiding plane is short or the prepared surface is oblique, the contact between the proximal plate and the guiding plane or oblique plane diminishes at an early stage, which results in reduction of retention. The guiding plane should be parallel to the path of insertion and must be of a certain length for adequate retention. An elongated proximal plate that is parallel to the path of insertion may compensate for retention deficiency when an adequate guiding plane cannot be prepared.
