Ivermectin quantitation with an abamectin internal standard.

Abstract

The potent antiparasitic ivermectin is detected at low concentrations (ng/g) in liver and muscle tissue by liquid chromatography after conversion to a fluorescent derivative with 1-methylimidazole and trifluoroacetic anhydride. This acetylation reaction can be compromised by residual water that leads to decreased yields. Yields of derivatives of ivermectin and abamectin, a related avermectin, are identical under all circumstances tested. Use of abamectin as an internal standard eliminates derivative yield as a source of analytical variation.

Human allergy and geohelminth infections: a review of the literature and a proposed conceptual model to guide the investigation of possible causal associations.

Geohelminth infections and allergic disease are major public health problems and there is evidence in developing countries that they are associated. Although there is an extensive literature of the relationship between geohelminth infections and allergy, there is little consensus on whether the association is causal and if so, whether geohelminth infections may increase or decrease the risk of allergy. An explanation for the conflicting findings of epidemiological studies is that geohelminths decrease the risk of allergy in areas of high infection prevalence and increase the risk of allergy in areas of low prevalence. Chronic geohelminth infections are inversely associated with allergy and anthelmintic treatment may increase the prevalence of allergy. In this paper, we review studies that have investigated the relationship between geohelminths and allergy; discuss the relevance of prevalence and timing of geohelminth infections and propose a conceptual model to define relevant scientific questions in future human and animal studies.

Cost containment in a school deworming programme targeting over 2.7 million children in Vietnam.

Vietnam is one of the countries in the world most affected by soil-transmitted helminthiases. Large areas of the country, such as the Northern Uplands and the North and Central Coast, are reported as having infection rates of 75-85% for Ascaris lumbricoides, 38-40% for Trichuris trichiura and 27-28% for hookworm infections. Periodical deworming of schoolchildren is therefore strongly recommended. Managers of the Helminth Control Programme decided to apply a number of measures to improve cost efficiency in order to deworm as many schoolchildren as possible with the limited financial resources available. This low-cost intervention targeted over 2.7 million schoolchildren. Coverage was estimated at over 95% and the cost for each treated child was US$ 0.03, which represents a saving of approximately 50% of costs presently reported in the literature. This article describes the measures applied that resulted in cost containment but maintained high treatment coverage.
