Streptococcus mutans levels and caries prevalence in low-income schoolchildren.

Previous studies have shown that persons having high Streptococcus mutans levels in the saliva are "at risk" for dental caries. Most investigators agree, that if high levels of S. mutans were identified early in the life of at-risk children, dental decay could be reduced or eliminated through intervention. The purpose of this study is to show an association between S. mutans levels and caries prevalence in a sample of elementary school children. The study group consisted of 242 school children, ages 5-13 years. The subjects were divided into two age groups, 5-8 years and 9-13 years. Approximately 59 percent were African Americans. The sample of 242 children were equally females and males, 50 percent in each group. The Dentocult SM Test was used to make S. mutans determinations. The df-t index was used to determine the number of decayed and filled teeth of children ages 5-8 years; the DMF-T Index estimated the number of decayed, missing or filled teeth of children ages 9-13 years. Dental caries were found in 58 percent of the children (mean = 2.67, and range of 1-11). Approximately 47 percent of the children with caries had high S. mutans levels (100K-1M). Females had higher S. mutans levels than males in the 9-13 age group, p < .05. Analysis of Variance Test indicated that S. mutans levels for older females (ages 9-13) were significantly higher than those observed in males the same age (p < .01). This trend was not observed in younger children, ages 5-8 years. In addition, no significant difference or interaction was noted by sex for S. mutans levels and decayed or filled teeth (df-t) for younger children. We conclude that high levels of Streptococcus mutans are related to increased number of decayed teeth and conversely, low Streptococcus mutans levels are related to fewer dental caries. This study was supported in part by Colgate-Palmolive Company and the National Dental Association Foundation, Inc.

[The condition of the permanent teeth of students 15 to 17 years of age in secondary schools in Csongrád County].

The teeth of altogether 1253 students studying in 38 secondary educational establishments of the County of Csongrád have been examined. The number of the examined amounts in case of the 15 years old to 13.51% and in case of the 17 years old to 8.8% of the total number of youth of similar age living in the County. The caries frequency is 98.95% at the age of 15 years, and 98.78% at the age of 17 years. The DMF-T index is 10.35 for the 15 years old, and 10.93 for the 17 years old. With respect to the DMF-T indices found with people of the same year in Szeged in 1961 the present data are higher by 39.11% (15 years old) and by 29.66% (17 years old), respectively.

[Enamel resistance to acid dissolution and its correlation with dental caries].

Enamel resistance to acid dissolution is a factor which has an influence upon dental caries susceptibility. The objectives of this study were to determine enamel resistance to acid dissolution by applying the RM technique, and to correlate data obtained to the prevalence of dental caries. Two hundred and seventy one children between seven and nine years of age were chosen by non probabilistic sampling in two city districts, (six public schools in Mexico City). These children's central permanent incisives had already erupted. The DMF-T and dmf-t indexes were recorded, and the RM enamel resistance test was performed on them. A total of 56.4% of the subjects in the sample had very resistant enamel and 27.3%, less resistant enamel. A proportion of 57.9% was free of dental caries on the permanent dentition and 10% in the temporary dentition. The average obtained for the DMF-T index was 0.93 +/- 1.34 and that for dmf-t was 4.7 +/- 3.1. Data suggest that enamel resistance distribution is not homogeneous and this increases proportionally in relation to the eruption third (p < 0.05.) Spearman's correlation coefficient was found to be negative and statistically significant at p < 0.05. The RM technique showed the presence of individuals with different enamel resistance to acid dissolution.

Bucodental health condition in patients with Down syndrome of Cordoba City, Argentina.

The oral health condition of children and youngsters with Down Syndrome (DS) was evaluated on a sample of 86 mongolic subjects ages 3 to 19, both sexes, residents in the city of Córdoba (Argentina), and compared with control groups. Those persons were attended special educational institutions for the care of that type of disabled individuals. In every age group, the dmf-t and dmf-s indexes were higher in the mongolic children than in the control population, while from the age of 10 onwards the DMF-T and DMF-S of the control population were higher than those of the DS individuals. In spite of this, the scarce participation of the DF component in the mentally disabled showed deficiencies in their dental care. This population exhibited a high frequency of retarded eruption, agenesis, conoidism, Angle's type III malocclusion, posterior cross bite and deficient gingival health. A positive correlation was found between tha activity of Lactobacillus and the amount of Streptococcus and the caries indicators. A high concentration of calcium and secretory IgA was found in the group of mongolic subjects. Our analysis evidences that DS patients are at a disadvantage in relation with healthy individuals in terms of oral health. An early program of preventive measures is proposed (dental hygiene, anti-plaque agents, Therapy of Orofacial Regulation) which would involve the education of parents and teachers.

[Oral health promotion for children in boarding schools].

INTRODUCTION: In Israel there are approximately 9,000 "sheltered" children who are lodged in several boarding schools. These children have been removed from their parents' guardianship, some with their parents permission and some by court order, usually due to neglect or abuse. This article describes these children and the efforts implemented regarding oral health: dental care insurance, a preventive program which has been conducted for approximately 10 years among 80 boarding schools and family care shelters, evaluation of these services and caries levels among the children. METHODS: A sample of 123 children was examined in four Israeli boarding schools. The DMF(T) index was employed according to WHO guidelines, by one examiner, using a torch, plane dental mirror and probe when required. RESULTS: An average DMF(T) level of 4.62 was detected among 12 year olds. Among all the children from ages 1-18 years, only 30% were caries free. The rate of caries-free children decreased with age. CONCLUSIONS: Caries prevalence among this population was higher than levels reported in Israel and abroad. There is a clear need for oral health promotion among this community.

Socioeconomic differences in the effectiveness of oral health programs for dental caries prevention in Europe: A systematic review and meta-analysis.

AIM: The meta-analysis systematically summarized evidence on how socioeconomic status (SES), access to dental care, and oral health programs (OHP) affect caries prevalence and severity among children and adults in Europe. METHODS: Four electronic databases (PubMed, Scopus, Cochrane Library and Embase) were systematically screened from 1947-2026, supplemented by cross-referencing and manual searches, without language restrictions. Study selection, data extraction and quality assessment were done in duplicate. Methodological quality was assessed using the NHLBI Study Quality Assessment Tools according to study design. Certainty of evidence was graded using GRADE Profiler 3.6. Mean differences (MD) were calculated for changes in DMFT, and odds ratios (OR) for the presence of dental caries in individuals using fixed-or random-effects models. RESULTS: Electronic searching identified 1800 articles; 23 studies were included in the review (>169,000 participants) and 14 in the meta-analyses (>88,000). Nine studies evaluated the effectiveness of OHP stratified by SES, and ten examined OHP in low-SES populations. Among low-SES individuals, participation in OHP was associated with a significantly lower increase in DMFT (MD[95% CI]=-0.63[-0.94;-0.31];very low certainty) and significantly lower odds of having dental caries (OR[95% CI]=0.61[0.52;0.73];very low) compared with non-participation. Among program participants, individuals with low SES showed a significantly greater increase in DMFT than those with high SES (MD[95% CI]=0.79[0.30;1.21];very low) and had significantly higher odds of having dental caries (OR[95% CI]= 2.88[1.97;4.22];very low). CONCLUSION: Participation in OHP may be associated with reducing dental caries in European populations, but benefits are unequally distributed and increase with higher SES. However, this conclusion is based on a limited number of well-conducted trials.

[Dental health status of students living in places supplied with drinking water of very high and very low levels of fluorides].

The results of a cross sectional epidemiological survey for the purpose of evaluating the state of dental health of schoolchildren (aged 6-7 and 12-13) living in Sampacho and Porteña, two towns in the Province of Córdoba (Argentina), supplied with drinking water containing quite different levels of fluoride, are described and analized. In Sampacho, F- level is 9.05 mg/l. while in Porteña the concentration is of 0.19 mg/l. The proportion of schoolchildren (aged 6-7 and 12-13) without caries was significaticantly higher in Sampacho than in Porteña, while the dmf-t, dmf-s, DMF-T and DMF-S indexes were considerably higher in the latter place. The severity of caries in children (age 12-13) living in Sampacho ranged from low to moderate (DMF-T = 2.53), whilst in Porteña the range went from moderate to high (DMF-T = 4.41). No cases of dental fluorosis were recorded in Porteña; but in Sampacho, there was a high proportion of children with mild fluorosis (aged 6-7) and mild or severe fluorosis (aged 12-13). Calcium, phosphate, thiocyanate, protein and secretory Ig A salivary levels were similar not only among schoolchildren of both towns but also among those with different experience of caries and different degrees of severity of fluorosis. It is concluded that urgent (preventive and/or curative) sanitary measures are necessary for the purpose of reducing or controlling caries in Porteña as well as dental fluorosis in Sampacho.

[Longitudinal study of the dental status of pregnant women under prenatal care].

The correlation between pregnancy and caries resp. gingivitis has been investigated in 57 pregnant women under care, at least once in each trimester. The caries prevalence was 98.25 per cent, both DMF-T and DMF-S index mean values showed a small increase with progressing pregnancy. The mean number of decayed teeth decreased (from 2.58 to 1.54), the values of filled, resp. extracted teeth increased (7.82 to 8.88, resp. 2.33 to 2.51) for the 3.s trimester of pregnancy. The prevalence of gingivitis was 96.5 per cent, showing increasing index values from the first (2.43), through the second (3.10) to the third (3.40) trimester. The mean index values of oral hygiene showed a decrease, which is attributed to continuous dental care and treatment during pregnancy.

[Profession and dental health].

The German Association for Dentistry project "Epidemiology" studied the dental findings of 2,336 patients over the age of 15 in 15 dental practices in Middle Hesse. Occupation-related evaluation of the DMF-T index and the oral hygiene findings showed that the caries rate was relatively unrelated to the patient's occupation. Differences in the state of oral hygiene however were observed.

[Need for preventive dentistry in adult women: investigation in maternal convalescent homes. 2. Tooth and periodontal findings].

For this study an extensive oral examination was performed in 111 women between 21 and 48 years of age recreating themselves in special rest homes. A mean DMF-T of 19.1 and a mean DMF-S of 62.5 was found; women with a low education (Sonderschule) showed an especially high DMF-S of 85.0, participants with a secondary education (Abitur) had a DMF-S-Index less by half. The oral hygiene of the women was insufficient (mean QHI 2.6), papilla bleeding was easy to provoke in most of them. 98% of the women needed periodontal treatment, 10.8% showed a probing depth of 6-8 mm. 60.8% of 942 examined restorations needed to be renewed, the reason was mostly marginal leakage.

[Dental caries prevalence in permanent teeth of schoolchildren in the Municipality of Sao Paulo, Brazil, 1970-1996].

OBJECTIVE: To assess the evolution of dental caries in permanent teeth of schoolchildren in S. Paulo City, Brazil, during the period of 1970-1996. METHODS: The World Health Organization methodology for oral health surveys was applied in 2,491 schoolchildren, both male and female from 103 public and private schools. A probabilistic sample stratified by age, school category, and city zones was obtained by the Education State Service data system. Computational resources were used. RESULTS: Data from the 1996 S. Paulo survey were compared with others data provided by studies such as the one conducted in the same city and the reference-population in the second half of the 20th century. In the 60s and 70s the DMF-T Index among 12-year-old schoolchildren was around 7.0. In 1986 it was 6.5 and in 1996 it was registered 2.1, a decline of 68%. CONCLUSION: Dental caries in permanent teeth among S. Paulo City 12-year-old schoolchildren changed from a "very high" to "low" prevalence.
