Critical factors in selecting removable prostheses.

Preservation of as many natural teeth as possible should be the goal of conscientious dentists. However, occasionally, the sacrifice of some sound natural teeth may be indicated for partially edentulous patients to provide them with the most functional and esthetic masticatory apparatus. Selection of the proper prosthetic service is one of the most critical value judgments made by dentists. No matter how skillfully the wrong treatment is accomplished, it is still the wrong treatment. It is important, therefore, that all relevant data be gathered so that the best clinical judgment can be brought to bear in the decision-making process.

Anodontia↗

Alveolar bone loss in overdentures: a 5-year study.

The results of a 5-year clinical study show that patients treated with complete maxillary dentures and mandibular overdentures demonstrate less vertical alveolar bone reduction than patients with complete maxillary and mandibular dentures.

The maxillary complete denture opposing the mandibular bilateral distal-extension partial denture: treatment considerations.

The dentist should approach the treatment of the complete maxillary denture opposing the mandibular bilateral distal extension partial denture cautiously, and the institution of correct treatment initiatives is essential. Every patient must be aware from the outset that the longest possible life of any prosthesis with the least possible harm to the remaining tissues can only be ensured by regular recall and maintenance care.

Bone Resorption↗

Obturator-overdentures retained by nonrigid attachments.

A technique is described to attempt to predictably retain a few remaining teeth for added stability and retention of a complete maxillary denture combined with a nasopharyngeal obturator. A male nonrigid attachment is added to copings on the retained roots, and the female attachments are incorporated in the denture base.

Prosthodontic and speech rehabilitation after partial and complete glossectomy.

A multidisciplinary approach to the rehabilitation of four glossectomy patients has been presented. Points emphasized were the anatomic defects, their effect on the three oral functions of mastication, deglutition, and speech, and the various prosthetic modifications found most effective to restore the impaired oral functions.

Chewing patterns before and after treatment with completè maxillary and bilateral distal-extension mandibular removable partial dentures.

Prosthodontic rehabilitation with complete maxillary dentures and mandibular bilateral distal-extension removable partial dentures did not change the general masticatory function to any great extent. However, the velocity of the mandible was increased, especially in the opening phase, and so was the mandibular displacement. The latter seems to be the cause for a long closing duration. The chewing cycle became more even and teardrop shaped, and the prosthodontic rehabilitation also resulted in a decrease of the number of chewing cycles. The Selspot system offers a possibility to study adaptation to prosthodontic treatment objectively by measurements of time and movement.

Adaptation, Physiological↗

Use of a palatal reservoir in denture patients with xerostomia.

Dentures that included a reservoir for saliva substitute in the palate of the maxillary denture were constructed for three xerostomic patients. Results were obtained through examinations and interviews. All patients found that the reservoir system helped control the xerostomia. Details of design and function were reviewed and general applications were discussed.

Combination syndrome: a treatment approach.

A method of treating patients who require a complete maxillary denture opposing a mandibular bilateral distal-extension removable partial denture has been described. This technique attempts to minimize the destructive changes seen in these patients by carefully distributing occlusal stress over the hard and soft tissues and by developing an occlusal relationship that is stable and balanced.

Bone Resorption↗

Submucosal nodular chondrometaplasia in denture wearers.

In 39 denture-wearing patients in whom anterior maxillary flabby ridge tissue (prosthesis fibroma) was excised, 15.4% contained cartilaginous nodules within this tissue. The chondrometaplasia may arise due to mechanical irritation activating pluripotential mesenchymal cells; but the possibility that the cartilaginous nodules represent remnant embryologic tissue cannot be excluded.
