Missing anterior teeth are commonly replaced immediately with partial dentures, but this may cause embarrassing problems and a poor aesthetic appearance. This paper describes a technique for replacing anterior teeth using the natural tooth as a pontic with a resin-bonded cast metal framework. The main advantage of this technique is that a fixed restoration is provided on the same day as the tooth is removed, providing a major psychological benefit to the patient and maintaining the aesthetics. The short-term results are promising- and recent advances in adhesive dentistry mean that the long-term result should be equally as good.

Modular Transitional Implants (MTI) are made from pure titanium and are used to support fixed provisional restorations during the osseointegration of definitive implants. This study histologically examined the jaw response to loaded MTIs in the dog mandible. Three implants were inserted transmucosally into each side of the mandible in 3 dogs. Stability was examined using a Periotest. Anterior and posterior implants were splinted using a cemented acrylic resin fixed partial denture to allow immediate loading. The middle implant remained unloaded and was used as a control. Dogs were sacrificed 11 to 12 weeks after implantation, and tissue blocks containing the implants were removed. Histologic examination showed that 10 of the 18 implants had good bone-to-implant contact, with the percentage of bone contacting the threaded portion of the implant varying from 30% to 65%. There was no statistical difference (p > 0.1) in percentage of bone-to-metal contact between loaded and unloaded implants. Six implants were entirely surrounded by connective tissue with or without inflammation; two implants were lost during the study. The success rate did not differ between loaded and unloaded implants. In the successful implants trabecular bone made good contact with the implant, forming supporting struts. There was bone remodeling in some bone-to-metal contact areas. It is believed that success was mainly influenced by the initial bone density at the implant site and by the uncontrolled load that the animals applied to the implants during the early healing stage.
Immediate loading of dental implants can reduce treatment time and management concerns, thus increasing patient acceptance. This paper reports the authors' preliminary experience with partially edentulous patients having received non-occlusally loaded provisional restorations within 24 hours of surgery, compared to patients treated according to an early loading protocol. The present data suggest that implant supported fixed partial prostheses can be immediately non-occlusally loaded and restored with a predictability similar to early loaded implants.

The conventional method of adding teeth to an existing prosthesis has the disadvantage that patients must be without their dentures while the teeth are added. Whilst a new immediate denture avoids this, it means that the patient will have to grow accustomed to the shape of a new polished surface. The method described in this paper eliminates these problems by making an immediate addition to the existing partial denture. Additionally, the technique has been found to be beneficial to those patients who are unable to tolerate conventional impression taking procedures.

The use of provisional implants as the sole or adjunct mean of stabilization for interim fixed partial dentures has gained popularity in implant prosthodontic treatment. These implants can be used between submerged implants during the healing phase to provide support for interim fixed or removable partial dentures. In this clinical report, a provisional implant used to provide support for a full-arch fixed interim restoration is described. Positioned between adjacent conventional implants, the provisional implant was placed during the same surgical procedure. As a result, an immediate acrylic fixed partial denture supported by natural teeth and the provisional implant provided a comfortable and esthetic interim restoration during the osseointegration period.

The article deals with the planning of different acrylic dentures, i.e. immediate and temporary denture, transitional denture and overdenture as definitive oral rehabilitation. A case where on a single molar a telescope crown and an Ipsoclip anchor were used is described. By an Ipsoclip anchor an additional retention was provided. To achieve a successful treatment with an acrylic partial denture the use of a surveryer is obligatory. Besides that, instead of wire made, elements for support and stabilization cast of semiprecious and non-precious alloys also give much better results.
PURPOSE: The aim of the present study was to compare the treatment outcome of TiUnite- and turned-surfaced Brånemark System (Nobel Biocare AB, Göteborg, Sweden) implants when applying immediate loading of cross-arch designed fixed partial dentures in the anterior mandible. MATERIALS AND METHODS: Fifteen patients with edentulous mandibles participated in the study. In one half of the jaw, between the exit of the nerve-vessel bundle and the midline, one type of implant was placed and in the remaining half the other type. The implants were loaded the day of surgery via a fixed, temporary supra-construction. Ten days later, the permanent one was screw retained to the implant pillars. RESULTS: The present 18-month clinical trial failed to demonstrate any differences regarding healing and cumulative success rate of an an-oxidized implant surface (TiUnite) and a turned (turned) one when implants in the anterior mandible were exposed to functional load within 24 hours after installation. CONCLUSION: A high predictability regarding the treatment outcome for immediately loaded Brånemark implants in the anterior mandible was observed. Furthermore, no difference between the traditional turned and the an-oxidized implant surface (TiUnite) could be observed. However, it has to be stressed that all implants (irrespective of surface) were placed in the anterior mandible and also that all the patients demonstrated a high level of oral hygiene.

In a controlled clinical trial, 74 patients who required complete immediate dentures were randomly treated with immediate mandibular overdentures on two mandibular canines with or without magnetic attachments or with complete immediate dentures. All were borderline situations for overdenture therapy. The patients were evaluated for 4.5 years after denture insertion. The results demonstrated that 50% of the dentures had been renewed or had required replacement; in this respect, no differences were found among the three groups. In the overdenture groups, 15% of the abutments were extracted; most abutment restorations were replaced or had to be replaced. Two thirds of the magnets were lost or in bad condition. No significant differences were found in relation to patient satisfaction; most patients were satisfied with their dentures. It is the authors' opinion that even though the abutment conditions were disappointing after 4.5 years, mandibular overdenture therapy is indicated in borderline situations because of the 50% reduction in mandibular bone loss found.
