Valgus slip of the capital femoral epiphysis.

Valgus slip of the capital femoral epiphysis, a rare entity which may be acute or chronic, occurred in a 12-year-old boy. The displacement may be associated with a pre-existing coxa valga. The patient also had a varus slip of the capital femoral epiphysis on the contralateral side. Both hips were successful treated by in situ pinning. A review of the English literature reveals only 8 previously reported cases of valgus slipping of the capital femoral epiphysis. Either valgus or lateral slips are unusual.

Slipped capital femoral epiphysis. A study of 50 cases in black children.

In 50 black children with 70 slipped capital femoral epiphyses, a satisfactory result was obtained in 87% of the hips. Chondrolysis developed in 6% of the hips, occurring only when there was persistent intraarticular protrusion of the transepiphyseal fixation device. The chondrolytic process resulted in a relatively painless fibrous ankylosis of each hip. Avascular necrosis developed in 7% of the hips, and occurred only in the presence of a valgus reduction of an acute slipped epiphysis. Each case of avascular necrosis was of the whole-head type and was severely progressive. Biplane subtrochanteric femoral osteotomy was employed as a reconstructive procedure in the severely-displaced slipped epiphysis; it was delayed until after epiphyseal plate closure, and restored satisfactory hip function in all cases. Black children are not predisposed to poor results after slipped capital femoral epiphysis contrary to a commonly held impression.

The John Charnley Award Paper. Experimental epiphysiolysis: etiologic models slipped capital femoral epiphysis.

The several anatomic features that permit the growth plate to resist shear forces were identified. Specific variations in these anatomic features were correlated with pathologic factors known to be associated with epiphysiolysis, including clinical correlations with slipped capital femoral epiphysis. Two models of epiphysiolysis representing different pathogenetic pathways were established in the pig and compared to clinical slipped capital femoral epiphysis in the human. Both experimental models and other models reported in the literature bear similarities mechanically and histologically to the human disease. Available evidence suggests that slipped capital femoral epiphysis may not be a single disease but a final mechanical event that occurs as a result of a variety of disease processes. These disease processes reduce by different mechanisms the ability of the growth plate to resist shear forces.

A histochemical study of slipped capital femoral epiphysis.

UNLABELLED: Core biopsies of the proximal femoral growth plate from three patients with slipped capital femoral epiphysis were compared with three normal growth plates from patients of similar ages. The growth plates of patients with slipped capital femoral epiphysis had a normal resting zone, a loose and fibrillated proliferative zone, and a very disarranged and thickened hypertrophic zone. The proliferative zone stained strongly with alcian blue, suggesting the presence of abundant proteoglycans in the cartilage matrix. The thickened hypertrophic zone was strongly positive to periodic acid-Schiff stain, indicating the presence of abundant structural glycoproteins. Enchondral ossification was scanty and irregular, and there were large clusters of cartilage in the metaphysis. Slippage occurred through the thickened proliferative and hypertrophic zones. CLINICAL RELEVANCE: The proximal femoral growth plate in slipped capital femoral epiphysis is histochemically abnormal and histologically disarranged. These features contribute to abnormal widening of the growth plate and to further slippage.

[Daily variations in S-adenosylmethionine content and oxyindole-O-methyltransferase activity in rat epiphysis].

Rat epiphysis was found to contain a relatively high amount of S-adenosylmethionine (SAM), which undergoes daily variations with a maximum in the middle of the daytime period and a minimum at the night-time phase. The variations are of endogenous type and are persistent in blinded animals. On the contrast, upon continuous illumination the SAM level in rat epiphysis is decreased and impaired. The activity of oxyindole-O-methyltransferase, the terminal enzyme of epiphysial melatonin biosynthesis which consumes methyl groups of SAM, is not subjected to daily variations. It is demonstrated that the artefact of the previously observed daily variations in the oxyindole-O-methyltransferase activity is due to day-to-night changes in SAM content.

[About of 82 obstetrical astro-articular injuries of the new-born (excepting brachial plexus palsies). Limits of initial therapeutic aggression and follow-up of evolution, particulary concerning traumatic separation of upper femoral epiphysis (author's transl)].

Obstetrical fracture has not a bad character, because of quick healing and, above all, because of extraordinary modelling by growth. The authors record 33 fractures of long bones which confirm this good prognosis. 20 birth fractures of the clavicule associated with brachial plexus palsies are recorded on account of their pathogenic, diagnostic and therapeutic intricacies. Prognosis of obstetrical traumatic separation of the epiphysis (epiphysiolysis) would be more preoccupying. This feeling is not given off the series of 28 observations recorded which conern 13 upper humeral, 6 lower humeral epiphysis, 1 of 6 upper femoral and 1 lower femoral epiphysis, of upper and lower of tibia. Diagnosis is not so easy, helped, if necessary, by arthrography. The clinical aspect of "pseudo-dislocation" associated with periostal callus, early appearing (about on the 10th day) rectifies diagnosis. Early treatment, as little aggressive as possible, (essentially orthopedic management by tractions along the axis of limb, giving up an anatomy as close as possible to normality) avoids sequellae, not existing at shoulder nor elbow and, if they exist, extremely decreased at hip, and the surgical management of which is always delayed. Initially good result of one birth dislocation of cervical spine, precedently recorded is considerably impaired.

Epiphyseometaphyseal cupping of the distal femoral epiphysis following scurvy in infancy.

Bilateral epiphyseometaphyseal cupping of the distal femoral epiphysis occurred in a 9-year-old girl with a history of infantile scurvy. This rare condition is characterized by a cup-shaped depression of the metaphyseal end, growth disturbance at the distal femoral epiphysis and knee flexion contracture which develops slowly over many years of bone growth. Conservative treatment by corrective plasters and back slabs brought about recovery of the right knee flexion contracture and halted the growth disturbance. However, in the left knee, extension supracondylar femoral osteotomies had to be performed and severe growth disturbance of the distal femoral epiphysis led to premature epiphyseal fusion.

Irreducible Salter-Harris type II fracture of the distal tibial epiphysis.

An 11-year-old boy was admitted to the hospital after he fell to the ground from the roof, complaining of pain in his left ankle. Radiographs showed a Salter-Harris type II fracture-separation of the distal tibial epiphysis together with a transverse bending fracture of the distal fibula. An attempt at closed reduction was unsuccessful. At open reduction, torn periosteum and the posterior tibial tendon were interposed between the tibial metaphysis and epiphysis. After the tendon was deflected to its normal position, fracture-separation of the distal tibial epiphysis was easily reduced. At the latest follow-up 4 years after surgery, radiographs showed no evidence of growth arrest or residual deformity.

Comparative studies of Ca(2+)-ATPase activity in epiphysis of tibiatarsus of quail lines selected for body weight.

1. Ca2+ concentration in blood plasma and Ca(2+)-ATPase activity in epiphysis of tibia at 3 and 15 weeks of age were compared among three quail lines (LL, RR and SS) selected for body weight. 2. The Ca2+ concentrations in blood plasma were higher in order of LL, RR and SS at 3 and 15 weeks of age in both sexes. Ca2+ concentrations in blood plasma at 3 weeks of age were lower than those of 15 weeks of age in both sexes. 3. At 15 weeks of age, the concentration of females was significantly higher than that of males in all lines. 4. The activities of Ca(2+)-ATPase in epiphysis of tibia was higher in order of SS, RR and LL at both ages and in both sexes. 5. The activities of Ca(2+)-ATPase at 3 weeks of age were significantly higher than those of 15 weeks of age in all lines. 6. These results suggest that the selection for body weight changed the concentration of Ca2+ in blood plasma and the activities of Ca(2+)-ATPase in epiphysis of tibia, parallel to the change of tibia length.

Healing of large surgical defects of the epiphysial plate. An experimental study.

In this experimental study using 37 rabbits, the growth and the regeneration potential of the distal femoral epiphysial plate were studied by creating a large surgical defect in the central part of the plate and filling it with a free fat graft. The results evaluated by radiography and histology show that fat grafting makes it possible to restore the main part of the longitudinal growth of the bone compared with a normal bone (p < 0.01) or the bone injured but left without fat grafting (p < 0.001). Regeneration of the growth plate was demonstrated histologically and in radiographs. Parts of the epiphysial cartilage continue the growth symmetrically without deforming the joint contours. The results of this study suggest that in clinical situations it is possible to restore at least part of the longitudinal growth of the bone and prevent joint deformity by bridge resection even in cases of extensive epiphysial injuries.

Biomechanical comparison of 1-screw and 2-divergent pin internal fixations for treatment of slipped capital femoral epiphysis, using specimens obtained from immature dogs.

Biomechanical strength and stiffness of 2 fixation treatments used to repair acute slipped capital femoral epiphysis were evaluated in bone specimens form immature dogs. A servohydraulic testing machine was used to create slipped capital femoral epiphysis in 7 pairs of femurs by shearing the capital femoral epiphysis along the physis in a craniocaudal direction. The slip was reduced and repaired with one 3.5-mm-diameter screw placed in lag fashion or 2 double-pointed, 1.6-mm (0.062 inch)-diameter smooth pins and retested. Strength and stiffness of each intact femur (which served as the control) and repaired femur were compared. Results of the study indicated that the failure strength of 2-pin fixation was significantly (P < 0.05) weaker than its control and the 1-screw fixation. There was no significant difference between failure strength of the 1-screw fixation and its control. The stiffness of 1-screw and 2-pin fixations was not significantly different, compared with each other, but was significantly (P < 0.05) less, compared with their respective controls.

[Tissue reaction after implantation of sintered porous hydroxyapatite in the distal metaphysis or epiphysis of the femur of rabbits].

The present investigator implanted sintered porous hydroxyapatite rod (phi 3 mm x 10 mm) in the distal metaphysis or epiphysis of the right femur of 40 Japanese white rabbits and studied tissue reaction around hydroxyapatite (abbreviated as HAp) with radiograms and microphotographs. As control, stainless-molybdenum alloyed (abbreviated as SMO) rod was used. In the groups with metaphyseal implantation of HAp or SMO, the distance between the center of implant and the distal femoral epiphyseal line increased during 12 weeks after operation. New bone was directly bonded to implanted material only after implantation of HAp. Ratio of length of HAp bone interface and length of HAp circumference was greater in the groups with implantation in the metaphysis 2 weeks after operation than those in the epiphysis. On the other hand, it was greater in groups with implantation in the epiphysis than those in the metaphysis 12 weeks after operation. In the groups with metaphyseal implantation of HAp, the ratio in groups 2 weeks after operation was greater than that in 12 weeks and there was a firm interrelationship between the ratio and the distance between the center of implant and the distal femoral epiphyseal line.

Treatment of the unstable (acute) slipped capital femoral epiphysis.

Slipped capital femoral epiphysis, the most common hip disorder in adolescence, traditionally has been classified according to symptom duration. An acute slip is 1 in which there are symptoms for < 3 weeks; for a chronic slip, there are symptoms for > 3 weeks. An acute-on-chronic slip is characterized by a combination of both with a recent exacerbation of symptoms. This classification system is misleading because it does not consider stability. A stable slipped capital femoral epiphysis has a good prognosis, but an unstable slip has a guarded prognosis. The priorities in treating an unstable (acute) slip are (1) to avoid avascular necrosis, (2) to avoid chondrolysis, (3) to prevent further slip, and (4) to correct deformity. The last priority, correcting the deformity, is associated with a high incidence of complications including avascular necrosis and chondrolysis, so manipulative reduction under anesthesia or an acute corrective osteotomy is not recommended. To address these priorities in treatment, the authors recommend preoperative bed rest to decrease the synovitis and intraarticular effusion. Operative stabilization is done in an elective fashion once the synovitis has subsided. The technique includes careful patient positioning on the fracture table, which may cause an incidental reduction, but no attempt is made to do a manipulative reduction. The technique is dependent on radiographic control. The femoral head and neck must be well visualized on the anteroposterior and lateral intensifier images before the operation is started. The slipped capital femoral epiphysis is stabilized with a single central screw, and nonweightbearing ambulation with crutches is recommended until a satisfactory painless range of motion has returned.

[Rôle of growth hormone therapy in the genesis of slipped capital femoral epiphysis].

The authors report two cases of children treated with growth hormone who had a secondary slipped capital femoral epiphysis. Endocrine disease, radiotherapy are some well-known etiological factors reported in literature, but growth hormone influence is still discussed. This hormone could have an indirect incidence on this pathology because of its effects on growth cartilage. The increased number of treatment with growth hormone is certainly a risk factor for slipped capital femoral epiphysis. Regular checking from "France Hypophyse" allows an early diagnosis of epiphysis and surgical fixation can be made immediately. Systematic controlateral side fixation it still discussed but allowed to continue the treatment with the same posology.

[Guanidinoacetate-N-methyltransferase in epiphysis of mammals: stimulating effect of norepinephrine in vitro].

The activity of the enzyme of creatine biosynthesis, S-adenosylmethionine: guanidineacetate-N-methyltransferase (E. C. 2.1.1.2) is discovered in rat and bovine epiphysis homogenates. The enzyme activity was highest in epiphysis of immature and young rats and then decreased linearly with the animal age. No reliable activity of the enzyme was found in adult rats (200-300 g). Incubation of rat and bovine epiphysis with norepinephrine (10(-5)-10(-6) M) in the presence of S-adenosylmethionine and guanidineacetic acid resulted in the increase of the creatine synthesis rate.

[The circadian dynamics of the biochemical activity of the epiphysis in silver foxes].

Specific features of light-dependent biosynthesis of melatonin in the epiphysis are analyzed in female silver-black foxes. Just as in most previously studied animals, the serotonin and 5-hydroxyindolylacetic acid content of epiphysis decreased during the dark time of the 24 h period, while the activity of the melatonin-synthesizing enzyme N-acetyltransferase and melatonin content of epiphysis and plasma, on the contrary, markedly and reliably increased. However, the pattern of changes in the neurotransmitters dopamine and noradrenaline shows specific features.

[Surgical treatment for Salter-Harris type II distal femoral epiphysis injury].

Results of surgical treatment for Salter-Harris type II distal femoral epiphysis injury in 4 boys aged 10-16 are presented. In 3 cases the epiphysis was fixed with two screws introduced above the growth plate. Kirschner wires inserted through epiphysis were used in one case. Healing occurred in all cases. Shortening of the femur due to the injury to the distal growth plate was observed in 2 patients. In these cases surgery was preceded by two attempts to reduce displacement conservatively. MRI and CT studies are useful in diagnostics for epiphyseal injuries and their consequences.

[20 years of subcapital osteotomy of slipped upper femoral epiphysis. Indications and results].

Between 1945 and 1985, 570 patients with a slipped upper femoral epiphysis were treated at our hospital. During the last two decades a subcapital osteotomy was performed if the degree of slip exceeded 50 degrees. Twenty two such operations were carried out in 21 patients. In five hips complete avascular necrosis of the epiphysis occurred, and in another hip there was partial necrosis. The success of the operation depends upon careful preoperative planning and a meticulous surgical technique.
