[Hyperbaric oxygenation in the combined therapy of infectious diseases].

Hypoxia plays an important role in pathogenesis of infectious diseases. Therefore, its correction is an essential factor in pathogenetic antibacterial treatment. Hyperbaric oxygenation (HBO) as a leading method in current antihypoxic therapy was studied in combined medication for typhoid, diphtheria, meningococcal infection, viral hepatitis. HBO sessions were given to 331 patients, 363 controls did not receive HBO. Utilization of HBO provided much better saturation of blood with oxygen, eliminated metabolic disorders, warranted favorable course and outcomes of the diseases.

[Real activity of Kraków endocrine group in Institute of Pediatrics CMUJ on the eradication of IDD in Polish children].

The authors present the investigations concerned the Iodine deficiency disorders (IDD) elimination in children living in South-East of Poland in last 15 years. To achieve the full success, what, as we know is possible, the monitoring and the exact control of the salt iodine supplementation according to the National Program of Iodine Deficiency Elimination (1999-2003) should be maintained. To avoid any negligence, the prompt education of the whole population is necessary, but yet especially the pregnant women. It is important to instruct them about the direct influence of iodine deficiency on the mental development of the fetus and the newborn.

[Termination of atrial flutter using esophageal stimulation].

The risk of possible complications in atrial flutter leads to attempts to use all available therapeutic possibilities to eliminate this disorder of the cardiac rhythm. By oesophageal stimulation the sinus rhythm was restored in 40% and a change to atrial fibrillation was achieved in 50% of the patients. The advantages of this method include above all speed and the minimal risk of complications; the disadvantage of the method is the unpleasant sensation when the electrode is inserted and the painful perception of the stimuli. Oesophageal stimulation can be recommended as the method of choice in treatment of atrial flutter.

Post-Hinckley insanity reform in Oregon.

The 1983 Oregon legislature responded to public pressure to narrow the application of the insanity defense by eliminating personality disordered individuals from consideration for an insanity verdict. This article examined the effects of the statutory change, and found no significant change in the frequency of insanity acquittals of personality disordered subjects between the three pre-reform years (n = 21) and the three post-reform years (n = 14). We also reviewed how the Psychiatric Security Review Board handled these patients once committed to their jurisdiction. We constructed a matched comparison group of psychotic acquittees and found that in the pre-reform years the personality disordered subjects spent less time in the system and less time in the hospital than the psychotic patients. However, in the post-reform years their time in the system and time in the hospital was the same as the psychotic controls. There were fewer decisions to discharge personality disordered patients from the system after the reform, although this difference may be due to factors other than the statutory reform itself. The conclusion is that narrowing the insanity defense is a worthy goal which may be difficult to achieve.

[Which hypocholesteremic agents for which hypercholesterolemias?].

Three groups of drugs can be used in hypercholesterolemia: 1) Drugs chelating bile acids (resins) which stimulate LDL receptors. 2) HMG R inhibitors which competitively inhibit the enzyme and increase the synthesis of LDL receptors. 3) Fibrate derivatives which are second line drugs in pure hypercholesterolemia. They should be used in case of intolerance of resins and may be preferred to inhibitors in moderate forms for economic reasons of lower cost. However fibrates are the treatment of choice in hypertriglyceridemia and mixed hyperlipidemias. All fibrates increase HDL cholesterol. 4) Probucol, neomycin and nicotinic acid are adjuvant hypolipidemic agents. The following steps must precede the prescription of a hypolipidemic drug: 1) Classification of the disorder. 2) Elimination of secondary dyslipidemias. 3) Family assessment. 4) An absolutely essential dietary phase. 5) This is permanent treatment without gaps. 6) No other atherogenic risk factor should be allowed to persist. 7) Iatrogenic effects must be avoided.

[Distal renal tubular acidosis in primary biliary cirrhosis].

5 patients with primary biliary cirrhosis (PBC), 9 patients with compensated hepatic cirrhosis of different etiology and 12 control persons were tested for renal acidification after peroral CaCl2 administration and urine Na2SO4 and pCO2 infusion as well as the gradient between partial urine pressure and blood pressure after NaHCO3 application. Distal renal tubular acidosis (DRTA) was diagnosed in one patient with PBC, latent DRTA in other 2 patients with PBC. Not even one patient's acidification disorder was eliminated through an increased sodium application to the acidification site after Na2SO4 application. After NaHCO3 application, the gradient between the CO2 partial pressure in the urine and blood in both patients with PBC was, however, latent, DRTA normal. After pH gradient elimination in patients with PBC and DRTA, the hydrogen iont secretion is thus comparable with the control persons. Based on this study, the authors believe that the gradient type of DRTA is characteristic of primary biliary cirrhosis.

Thirty-five years of progress in cardiovascular research.

In this overview of the achievements of the National Heart, Lung, and Blood Institute (NHLBI), the major developments in each major form of heart disease since the birth of the Institute 35 years ago are reviewed. In the case of congenital heart disease, it has become possible to establish an accurate diagnosis, often by noninvasive methods, and to correct by surgical treatment almost all congenital cardiac malformations. The major challenge now is to eliminate these disorders; this will require an understanding of the fundamental molecular basis of these lesions. Acquired valvular heart disease can also now be characterized by hemodynamic, angiographic, as well as noninvasive techniques, principally echocardiography. Surgical treatment is usually successful, but improvement in the durability of valves without thromboembolic potential remains an important challenge. While essential hypertension can now be managed pharmacologically in almost every patient and while such management reduces the excess mortality resulting from this condition, current research focuses on elucidating the underlying basis of this disorder. Atherosclerosis remains the most common cause of cardiac and vascular disease. Although its cause has not been defined, several abnormalities in lipid metabolism that play an etiologic role in many patients with atherosclerosis have been identified. The treatment of these disorders with cholesterol-binding resins, which increase the number of cellular receptors for low-density lipoprotein, and with inhibitors of the enzyme required for cholesterol biosynthesis holds considerable promise. Noninvasive techniques will be used increasingly for detection of atherosclerosis in asymptomatic persons and for determining the efficacy of therapy. The mortality resulting from acute myocardial infarction has been reduced in half by the development of coronary care units and the prompt treatment of potentially fatal ventricular tachyarrhythmias. Current research is focused on preventing pump failure by limiting infarct size; lysis of coronary thrombi now appears to be the most promising method of accomplishing this. Chronic angina pectoris can be relieved by the judicious use of three classes of drugs--organic nitrates, beta-adrenergic blockers, and calcium antagonists, and two mechanical approaches--percutaneous transluminal coronary angioplasty and coronary artery bypass grafting--are usually successful in relieving angina in patients who do not respond adequately to medical management. Whether or not any of these approaches prolong life is not yet settled.(ABSTRACT TRUNCATED AT 400 WORDS)

[Rehabilitation treatment of cancer patients in ambulatory facilities].

325 patients radically operated on for cancer of the breast, stomach, colon and rectum were given a rehabilitative treatment at a specialized out-patient department of the Dispensary. As a result, postoperative disorders were eliminated in 55% of breast cancer patients, 83.8% of cases of colonic and rectal cancer and 76.6% of gastric cancer patients. These indexes appeared to be significantly higher than those in controls who had not received rehabilitative treatment.

[Possible discrepancy between behavioral adequacy and motor automatisms in cats with phenamine-induced stereotypy].

Experiments in cats were made to study the effect of the increasing doses of the neuroleptics haloperidol and clozapin as well as those of metoclopramide with weak antipsychotic properties on various characteristics of amphetamine-induced stereotypy. Haloperidol was shown to return the animals' behavior adequacy to normal and to concurrently eliminate the motor automatisms. Clozapin produced an earlier recovery of the psychoemotional state, exerting a less powerful effect of motor manifestation of stereotypy. On the contrary, metoclopramide eliminated motor disorders more readily but did not return the behavior adequacy to normal. The reason for dissociation of both the characteristics of stereotypy may lie in dissimilar participation in it of different parts of the caudate nucleus or in different implications of the caudate and extracaudate mechanisms.

The adipocyte volume in human adipose tissue: II. Observations in diabetes mellitus, primary hyperlipoproteinemia and weight reduction.

Body-weight index does not appear suitable as a measure of metabolic risk, since the prevalence of diabetes mellitus, hypertriglyceridemia and hypercholesterolaemia increases only up to a Broca index of 1.2. Attempts were therefore made to demonstrate an abnormal adipocyte enlargement in the subcutaneous adipose tissue of the abdomen in patients suffering from various metabolic disorders. To eliminate the influence of sex and body-weight index, normal curves of the adipocyte volumes in 142 controls were used as basis of comparison. Adipocyte hypertrophy exceeding the degree predicted by the normal curves was observed in patients with subclinical diabetes (n = 20), patients with maturity-onset diabetes (n = 56) and in patients suffering from hyperlipoproteinemia of type IIb (n = 10), type III (n = 8), type IV (n = 42) and type V (n = 24). Excessive hypertrophy could not be detected, however, in juvenile-onset diabetics (n = 11) and in hyperlipoproteinemia type IIa (n = 9). Weight reduction gave further insight into excessive adipocyte hypertrophy. This was effected by reducing caloric intake combined with physical exercise for four to five weeks until 10 per cent of initial weight was lost. Adipocyte shrinking per kg loss of body weight was significantly more pronounced in patients suffering from subclinical diabetes (31 pl/kg,n = 10), from maturity-onset diabetes (26 pl/kg,n = 23) and from hypertriglyceridemia (17 pl/kg, n = 11) in comparison with the controls (7 pl/kg,n = 25).

[The medical aspects of nutrition in minor casualties].

It is recommended that a standard ration will be accompanied by "Unpit", natural casein concentrates, and "Glucosolan" vitaminized preparation to eliminate++ the disorders of trophological status of minimally wounded. There is a proposal to include the products of enteral nutrition for the ration of food supplies, because it would shorten the recovery time of minimally wounded, and, thus, accelerate their return to the ranks.

[Ventilation distribution disorders. Evaluation based on nitrogen elimination curves of the lung and on an inspiratory gas-distribution index derived from it].

An uneven distribution of the inspiratory gas volume to the alveolar volume leads to disturbances of the ventilation/perfusion ratio VA/Q with resulting arterial hypoxemia. These ventilatory disturbances can be verified by nitrogen-washout-curves and an inspiratory gas distribution index derived from those curves. Preoperative diagnostic and the control of respiratory diseases during intensive therapy are possible without technical difficulties.

A ten year review of the iodine deficiency disorders program of the People's Republic of China.

This paper reviews the Iodine Deficiency Disorders (IDD) Elimination Program of the People's Republic of China (PRC). Over 700 million people live in iodine deficient regions of China and the social and economic consequences of IDD are profound. National and international organisations have supported China's advocacy, social mobilisation and health education programs. Many countries affected by IDD have benefited from discoveries emerging from China's aggressive and effective multisectoral approach to this massive public health problem. The challenge of the next decade is to sustain the momentum through the use of cost-effective monitoring and quality improvement strategies, particularly, in those regions in which a large proportion of babies continue to be at risk of brain damage due to iodine deficiency. This paper sets China's IDD Program in an international context by reference to the literature and through consultation with public health experts.

Hypergonadotropic forms of amenorrhea in young women.

Hypergonadotropic forms of amenorrhea in young women are many and varied, and the disorder is not nearly as uncommon as previously believed. It is likely that all physicians seeing women with amenorrhea will encounter this disorder. Careful evaluation is warranted to eliminate any associated autoimmune disorders. Because of the high likelihood of osteopenia in affected individuals, estrogen replacement therapy is warranted. Although pregnancy is possible in women with secondary hypergonadotropic amenorrhea, remarkably it most commonly occurs in women utilizing exogenous estrogen. Women desirous of achieving a pregnancy are best served by assisted reproductive technology utilizing donor oocytes. Success rates in such patients have been quite high. Thus, physicians today can counsel affected women that pregnancy is indeed possible, even in women with so-called "premature ovarian failure."
