[Macro- and microscopic anatomy of the heart conduction system in myocardial infarction].

Morphological and morphometric examinations of the cardiac conduction system (CCS) of subjects who had died in different periods of acute myocardial infarction in the presence of atrioventricular block versus those without conduction disturbances have indicated that the severity of circulatory disorders leading to destructive processes in the ischemic myocardium, as well as involutional processes in its autonomic nerve terminals are essential in the development of functional abnormalities in CCS in myocardial infarction. The more profound the dysfunction of CCS is, the closer it is to the myocardial alteration foci. The altered CCS with the block recorded should be interpreted as its impact of ischemia in the presence of fibrosis and calcinosis of ventricular septal structures.

[Sudden cardiac death as a consequence of heart conduction disorders. II. Sudden cardiac death in lesions of the heart conduction system].

The conduction system in 31 patients (average age 43 years) who died suddenly and unexpectedly is analyzed. Seven cases show essential lesions of the conduction tissue, these lesions being the only pathological findings at all in 5 cases. The loss of conducting fibers is severe and fulfills our morphological criteria for av-block (see Part I). Considering these lesions are the only pathologic findings, there is every reason to believe that these patients died of Adams-Stokes attacks. Lack of evidence of a basic disease means that there is idiopathic loss of conducting fibers. The site of the lesion is mainly the branching bundle and the origin of both bundle branches. Fibrosis is never a main feature, and this is why the author is reluctant to characterize the disease simply as bilateral bundle branch fibrosis.

[Steinert's disease with cardiac arrhythmia. Morphological findings in the heart conduction system].

This is a case report of a 63-year-old man with myotonic dystrophy in whom an ECG showed atrial flutter, complete A-V block, idioventricular rhythm with image of left bundle branch block and giant, wide and negative "T-U" waves. The microscopic study of the conduction system showed slight fibrosis of the SAN, fatty infiltration and cellular atrophy of the AVN and bundle of HIS, as wells ad basophilic degeneration of a cell at the begining of the left branch and diffuse sclerosis of the left subendocardial ramifications. Advanced coronary atherosclerosis was also found. Since only two complete pathologic descriptions of the heart in myotonic dystrophy have been reported, no definitive specificity could be atributed to this findings. It is suggested that in the present case both coronary atherosclerosis and an intrinsic myocellular dystrophy could have contributed to the final lesions of the conducting system.

[DNA synthesis in myocytes of the heart conduction system in myocardial infarction induced in young rats].

The myocardial infarction was induced in 13- and 21-day-old rats. The extent of the DNA synthesis activation in myocytes of conduction system compartments compared with atrial and ventricular myocytes was evaluated by cumulative indices of labeled nuclei after 30-times-repeated 2H-thymidine injections at 12h intervals. Labeling indices in atrial myocytes are two to six times higher (0.01 < P < 0.001) in rats with infarction than in control. On the contrary, in conduction system compartments there are no statistically significant differences between control and experimental groups (P > 0.05). An activation of DNA synthesis can occur in myocytes of His bundle in some animals.

[Antiarrhythmic activity of trimecaine in experimental arrhythmia and its effect on the heart conduction system].

The anesthetic trimecaine is shown to be capable of eliminating the flutter of atria in dogs simulated by an electric stimulation of the myocardium and atrial fibrillation in cats induced with aconitin and precludes ventrical fibrillation in rats arising due to intoxication with calcium chloride. Trimecaine noticeably mitigates the toxic effect of strophanthin. While depressing the automatism of the sinoatrial node the drug does not affect the conduction function.

[The characteristics of the medical and physical examination of flight personnel with anomalies of the heart conduction system].

The article summarizes the results of examination of a considerable number of flight personnel of the Air Forces with cardiac conduction system abnormalities. The article contains peculiarities and algorithm of examination of such patients. New expert approaches are given concerning this kind of patients as well as methodic recommendations on their medical examination. The data which were obtained during this researches make it possible to improve the work of flight medical evaluation.

[The specific gravity of the excitation formation and conduction system of the heart compared to the working myocardium as an indication of differences in structure and composition (author's transl)].

Samples of heart conducting system tissue and samples of ordinary heart muscle (left heart, right heart, and septum interventriculare) were taken from the hearts of 50 prime-conditioned bulls. The specific gravity of the heart conducting system was significantly higher than that of ordinary heart muscle. This is surprising because it cannot be explained by a difference in water content. The question arises whether or not there are differences in the composition of protein and in the content of glycogen.

A comparative enzyme histochemical study of glucose metabolism in the conduction system of mammalian hearts.

A histochemical study of some enzymes of glucose metabolism was performed on the heart conduction system of rat, dog, rabbit, pig, calf and lamb. Histochemical activities revealed a higher rate of anaerobic metabolism and a lower rate of aerobic metabolism in the conducting cells in comparison with the working myocardial fibres. An increase of the histochemical activities from the atrioventricular node to the distal portions of bundle branches was noted. The importance of the high glycogen content and the high phosphorylase activity in the heart conduction system was discussed.
