Hock injuries in cattle kept in straw yards or cubicles with rubber mats or mattresses.

Hock damage is one of the most common traumatic injuries suffered by dairy cows, but most hock injuries should be avoidable. This study investigated the effect of housing system on the development of hock damage in first lactation Holstein heifers. After calving, 60 heifers were randomly allocated to either straw yards, cubicles with butyl rubber mats or cubicles with mattresses filled with chopped tyres. The hocks of these heifers were examined in the first week after calving (week 1) and in weeks 6, 12 and 26 of lactation. Hock damage was scored as either 1 (hair loss only) or 2 (all other damage). Heifers housed in cubicles with mats had significantly worse lesions at week 26 than at week 1, while there was no change in heifers housed in cubicles with matresses, and heifers housed on straw had significantly lower lesion scores. Additionally, heifers with no lesions at calving were significantly more likely to develop lesions when kept on mats than heifers kept on straw or mattresses. These data suggest that replacing mats with mattresses in cubicles can result in a significant reduction in traumatic hock injury.

Placebo-controlled trial of house dust mite-impermeable mattress covers: effect on symptoms in early childhood.

We investigated the effect of house dust mite (HDM)-allergen avoidance on the development of respiratory symptoms, atopic dermatitis, and atopic sensitization by performing a double blind, placebo-controlled trial. In total, 1,282 allergic pregnant women were selected (416 received HDM allergen-impermeable mattress covers for the parents' and child's mattress in the third trimester of pregnancy [active], 394 received placebo covers, 472 received no intervention). Data on allergen exposure, clinical symptoms, and immunoglobulin E were collected prospectively. The prevalence of night cough without a cold in the second year of life was lower in the group with active covers compared with the group with placebo covers (adjusted odds ratio 0.65; 95% confidence interval 0.4-1.0). No effect of the intervention was seen on other respiratory symptoms, atopic dermatitis, and total and specific immunoglobulin E. It can be concluded that application of HDM-impermeable mattress covers on the child's and parents' beds reduced night cough, but not other respiratory symptoms, atopic dermatitis, and atopic sensitization in the first 2 years of life. Follow-up will determine the long-term effect of the intervention on the development of atopic disease.

Allergen reduction measures in houses of allergic asthmatic patients: effects of air-cleaners and allergen-impermeable mattress covers.

Recommendations for allergen avoidance or allergen reduction measures play an important part in the treatment of allergic asthmatic patients. The purpose of this study was to test recently developed air-cleaners with respect to their capacity to capture airborne allergen particles and to improve clinical parameters of asthmatic patients sensitized to aeroallergens. Forty five allergic asthmatic patients were studied in a double-blind procedure for 6 months. The patients were divided into three groups of 15 patients. In Group 1, the intervention consisted of the application of active air-cleaners in living-rooms and bedrooms. In Group 2, placebo air-cleaners were used in combination with allergen-impermeable mattress covers. In Group 3, the same intervention was performed as in Group 2 but with active air-cleaners. Allergen levels in mattress and floor dust were measured before, and 3 and 6 months after the interventions. After 6 months, the air-cleaners were dismantled and the filters were analysed for the amount of dust collected and allergen content. Immunological and lung function parameters were measured before, and 3 and 6 months after the interventions. Considerable amounts of airborne dust and allergenic particles were captured in the filters of the air-cleaners. Up to the 18.9 g of dust, 4,513 ng of house dust mite allergen, Der p 1, and 50,000 mU of cat allergen, Fel d 1, (in houses with cats) were collected by air-cleaners in living-rooms. Only in Group 3 (in which both active air-cleaners and mattress covers were used) was a small (less than 1 doubling dose) but statistically significant improvement of provocative concentration of histamine causing a 20% fall in forced expiratory volume in one second (PC20) observed (from 5.96 to 9.02 mg x mL(-1)). The amount of dust and house dust mite allergen collected in the filters was significantly correlated with an improvement of peak flow variation. In combination with other allergen avoidance measures, the examined air-cleaners can contribute to diminished allergen exposure and improvement of airway hyperresponsiveness in asthmatic patients.

Forced-air warming maintains intraoperative normothermia better than circulating-water mattresses.

The hypothesis that forced-air warming preserves core temperature better than circulating-water mattresses was tested in: (a) 16 adults undergoing major maxillofacial surgery, including radical node resection and flap reconstruction; (b) 53 adults undergoing hip arthroplasty, having approximately 25% of their body surface area available for warming; (c) 20 infants undergoing minor maxillofacial surgery; and (d) 10 young children undergoing pelvic or femoral osteotomies. Patients having each type of surgery were randomly assigned to forced-air warming (approximately 40 degrees C) or conductive warming using a full-length circulating-water mattress at 40 degrees C. Forced-air warming was applied to the legs of the adults undergoing maxillofacial surgery and to one arm, the shoulders, and the neck in the adults undergoing hip arthroplasty; a U-shaped, tubular forced-air cover was positioned around the pediatric patients. Core temperatures increased in all patients given forced-air warming and decreased or remained constant in those without active warming. Furthermore, we needed to decrease the temperature of the warmer from high to medium (approximately 37 degrees C) in most patients assigned to forced-air warming to prevent hyperthermia. After 15 h of anesthesia, rectal temperatures in the adults undergoing maxillofacial surgery were 3.4 degrees C higher in the forced-air group (P < 0.01). After 4 h of anesthesia, esophageal temperatures had increased 0.8 +/- 0.5 degrees C in the patients warmed with forced-air and decreased 0.8 +/- 0.3 degrees C in those warmed by circulating-water mattresses (P < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

The Softform pressure-reducing mattress for high-risk patients.

This article highlights the fundamental difference between pressure-relief and pressure-reduction. The limitations of the conventional hospital mattress will be discussed in terms of lack of support properties and life expectancy. The need for a coordinated mattress replacement programme will be outlined and the advantages of the Softform pressure-reducing mattress will be described in detail.

Clinical evaluation of an electronic pressure-relieving mattress.

A 12-month clinical evaluation of the Talley Quattro Deep Cell 2000 mattress manufactured by Talley Medical was undertaken on both a trauma and surgical ward at the Royal Devon and Exeter Hospital. An identified registered nurse coordinated the evaluation with support from tissue viability clinical nurse specialists. Data on patients, including diagnosis, Waterlow score, pressure area condition and general comments by nurses and patients, were collected. The evaluation involved 35 patients who were at high- to very-high risk of pressure sore formation. The results showed that 16 out of 40 sores healed during the evaluation period and 16 sores remained unchanged. Only one patient developed broken skin while nursed on the mattress. Seventeen patients maintained tissue integrity. Although the study was limited the results suggest that the Talley Quattro Deep Cell 2000 mattress is useful in preventing and managing pressure sores in high- to very high-risk patients.

A clinical evaluation of the Transfoam mattress after 4 years.

It is recognized that pressure-reducing foam mattresses can be of benefit in the prevention of pressure sores but it is also recognized that more information is required to demonstrate their long-term efficacy (Cullum et al, 1995). It is therefore necessary to evaluate the pressure-reducing capabilities of this type of mattress some years after purchase to evaluate if in fact it can maintain its initial level of performance. This article presents the results of one such study carried out 4 years after the purchase of Transfoam mattresses.

The new QUATTRO PLUS mattress replacement system.

Since the early development of alternating pressure air mattresses (APAMs) there have been regular alterations in design. These designs have led to improved reliability and performance. This article outlines the changes made by Talley Medical to the QUATTRO PLUS replacement mattress. Improved safety features and design modifications continue the process commenced many years ago which has seen the use of alternating pressure air mattresses become widespread throughout the UK.

The Theracute alternating pressure replacement mattress.

The use of alternating pressure mattresses is still acknowledged as an effective adjunct in the prevention and treatment of pressure damage; however, with the plethora of equipment now available, there is an increasing need to provide information that supports the practitioners' decisions ensuring individual patients' clinical needs are met. This article reviews a new addition to the Talley Medical range of alternating pressure mattresses, the Theracute mattress replacement system.

Transfoam Visco: evaluation of a viscoelastic foam mattress.

It is both a massive financial commitment and a huge financial decision to standardize mattresses in a large trust. It should only be undertaken after evaluation of the evidence of efficacy supplied by relevant companies, together with independent research and reports by healthcare professionals. In addition, evaluation of the equipment should be made by back care advisers, from a user-safety viewpoint and physiotherapists, occupational therapists concerning rehabilitation. Ward-based nurses and patients need to assess equipment for comfort, ease of repositioning individual, and ability to relieve pressure. This article outlines the process by which a static mattress was evaluated for ease of movement, pressure relief and comfort by patients and healthcare professionals. The possible longevity of the mattress was considered as secondary relevance; however, cost-effectiveness and quality of product are essential in today's acute healthcare setting.

Improtec R Barrier: a mattress cover which does not break down.

Mattress cover material breakdown in the clinical setting has been a recognized problem for over 10 years. Shortly following disruption to the surface integrity, mattresses become contaminated with body fluids, which leads to bottoming-out of the foam, putting patients at increased risk of infection and pressure ulcer formation. Collaboration between manufacturers and clinicians has produced a product that should stand up to the rigours of the healthcare market, providing safety for patients and value for money as a result of extended mattress life.

The QUATTRO ACUTE mattress and pressure ulcer prevention.

There are a plethora of mattresses on the market and nurses are not always sure which mattress to select for each individual patient. This article outlines the issues in preventing and understanding pressure ulcers and the performance of the QUATTRO ACUTE trade mark mattress, manufactured by Talley Medical.

[Evaluation of presence of Staphylococcus aureus on the beds of Hospital Escola's Intensive Care Unit, concerning the position on the mattress, before and after cleaning].

By means of culture medium, it was researched the position of the colony of Staphylococcus aureus on the mattress, to evaluate the efficaciousness of the methods of cleaning and disinfection of the river bed in the Faculdade de Medicina do Triângulo Mineiro's School Hospital (Uberaba). It were evaluated fifty mattresses on the period of October 22nd (2000) to January 16th (2001). The samples were collected and grown, the exhaustion technique draining, on two different nutrient bases (blood agar and mannitol salt agar) followed by catalase and coagulase tests. For the statistical analysis, were used non-parametrics tests Mann-Whitney, Kruskal-Wallis, Wilcoxon Matched Pairs Test with significance level p < 0.05 were used. Six hundred dishes of culture medium have been used. There was growing in 94 (15.6%), being 82 (87.2%) before and 12 (12.8%) after cleaning and disinfection. Concerning the position on the bed, the samples obtained from mannitol salt agar medium showed significant retention on the lower position of bed. The results alert to flaws in the procedure for cleaning and disinfection from the mattresses studied by us.

Permethrin-impregnated mattress liners: a novel and effective intervention against house dust mites (Acari: Pyroglyphididae).

This study tested the efficacy of permethrin-impregnated mattress liners in reducing house dust mites in the homes of volunteers with no previous recorded history of asthma, atopic eczema, or perennial rhinitis. The field trial using permethrin-impregnated (450 mg/m2 of pure permethrin in polyester netting weighing 35 g/m2) mattress liners (n = 9) was conducted for 27 mo. The permethrin-impregnated bedding significantly reduced house dust mites in mattresses for at least 27 mo. Allergen concentrations were significantly lowered at 15-mo postintervention. No adverse side-effects were reported. This is a promising development in house dust mite control.

Effectiveness of mattress overlays in reducing interface pressures during recumbency.

This study evaluates the pressure-reduction characteristics of seven mattress overlays. Thirty subjects were evaluated on each support surface to determine the interface pressures that are generated under the most common pressure sore sites. The results of this study indicate that there is great variability in the effectiveness of traditional mattress overlays. The most effective overlays are the Roho and Akros DFD mattresses; whereas 2-inch thick convoluted foam provides no significant protection for the trochanter when the subjects were lying on their sides (lateral position).

Sudden infant death syndrome (crib death) and possible relation to long-term mattress compression.

Evidence is presented to show that standard mattresses compress infants enough to increase the work required to maintain normal levels of ventilation and cellular respiration. The possible relation of long-term standard mattress compression to Sudden Infant Death Syndrome is proposed. Evidence is presented to show that waterbeds do not compress infants significantly. The suggestion is made that, if long-term standard mattress compression is related to Sudden Infant Death Syndrome, then the use of waterbeds designed for babies may help reduce crib deaths.

Mattress evaluation for long-term care facilities.

Patients spend more than a third of their hospital time in bed and considerably more than that if they are residents in a long-term care facility. When our facility, Fair Acres Geriatric Center, a 911-bed long-term care facility in Lima, PA, noticed increasing losses of our standard mattresses, we decided to evaluate new products. We listed very important criteria (bacteria resistance, flame retardation, self-deodorization and comfort and resiliency) and important criteria (stain resistance, cost and warranty) and then brought in three standard mattresses for our staff to evaluate. We were able to procure a mattress that met our criteria and halved our costs in the process.

An evaluation of mattress encasings and high efficiency particulate filters on asthma control in the tropics.

The effect of two allergen avoidance modalities, Allergy Control Covers (ACC) and High Efficiency Particulate Filters (HEPA) on asthma control in children were evaluated. This was an open study involving 24 dust mite sensitive asthmatic children. Following a 4 week run-in period, the subjects were randomly allocated to use mattresses fitted with ACC (n = 6), HEPA filters in their bedrooms (n = 12) or act as controls (n = 6) for a study duration of 4 months. Measurements of the major Dermatophagoides spp. mite allergens, Der p 1 and Der f 1, levels in dust samples obtained from mattresses were made at baseline, 1, 2 and 4 months post implementation. Daily symptom scores including morning and evening peak flow readings, and monthly spirometry and exercise bronchoprovocation tests were carried out Our results showed that dust mite allergen levels in mattresses fell at 1 and 2 months post implementation in the ACC group (p<0.05). In contrast, no decrease in allergen levels was seen in the HEPA and control group. At the end of the 16 weeks, only the ACC group showed improvement in FEV1 and reduction in diurnal peak expiratory flow rate (p<0.05). Improvement in mean symptom scores was also observed for both the ACC and HEPA groups, but not the control groups (p<0.05). Although the numbers in this study were small, the results Indicate that the effectiveness on mite exposure barrier covers was short-lived, and the improvement in asthma control though documented was not obvious.
