[Studies on background of gamma-GTP abnormality in human multiphasic screening. (Part 3): Multiple regression analysis by SAS statistic package for computer analysis].

Multiple regression analysis in 1493 cases (1256 male cases, 237 female cases) who were examined in the short-term human dry dock managed by the Fukuoka University Hospital, was done with gamma-GTP as the dependent variable and other parameters as the independent variables. As the result, 16 variables including LAP, alcohol, GPT, triglyceride, HDL and obesity rate were selected as the variables explaining gamma-GTP by stepwise methods in men. The standard regression coefficient of alcohol is shown to be greater than that of obesity on gamma-GTP. On the contrary, neither alcohol nor the obesity rate was included in this regression formula in women.

Range of frontal plane QRS axes. Electrocardiograms of subjects in a multiphasic screening program.

The range of mean QRS axes in the frontal plane was determined in electrocardiograms of subjects examined in a program of periodic health screening. Fifty-three of 5,163 individuals had a mean frontal QRS axis between -30 degrees and -60 degrees. Fourteen individuals had an axis between +110 degrees and +133 degrees. These findings suggest that individuals with no history of heart disease may manifest a considerable range of frontal plane QRS axes often to a degree previously considered abnormal. The axes were calculated by the use of leads 1 and 3. Other leads, or the use of the method of Grant, would give different results. Comparison of series of patients can be made only if these variables are understood.

Plasma fibrinogen, cholinesterase activity, and anemia: utility of fibrinogen in multiphasic screening and in assessing the activity of diseases.

I present the general pattern relationship between (a) plasma fibrinogen and cholinesterase activity and (b) plasma fibrinogen and hemoglobin, in 250 and 310 hospitalized adults, respectively. Although responses of cholinesterase were often unrelated to fibrinogen, when its activity in plasma was depressed, above-normal fibrinogen concentrations were present in about 90% of the subjects. Further, despite the observed independence of hemoglobin and fibrinogen concentrations, whenever frank anemia was present (hemoglobin less than 110 to 120 g/liter), hyperfibrinogenemia was also present in about 75-85% of such anemic patients. Thus, these studies affirm that fibrinogen determination would be an appropriate component of tests on hospital admission and also that hyperfibrinogenemia is a very sensitive "acute phase" reactant, and an important reaction for gauging the course of numerous disorders.

[Clinical evaluation of arteriosclerosis using high sensitive CRP in multiphasic screening].

In order to identify the chronic inflammation, hsCRP measurement at several time are recommended. The continuous higher value of CRP is observed as the signal of the chronic inflammation. In this case the average of CRP was more than 0.15mg/dl and the standard deviation was less than 100%. In this study, the hsCRP showed the biggest AUC (Aria Under Curve) at acute myocardial infarction (AMI). The blood pressure showed the biggest AUC at stroke. The event rate for AMI of patients who have the continuous higher CRP value is higher than the normal control. The relative risk for AMI is 20 fold. On the other hand, the relative risk for stroke is less than 1 fold. The key factor of the disease was suggested difference between AMI and stroke.

[Prevalence and risk factors for silent lacunar infarct in white matter lesion--brain multiphasic screening].

To clarify difference of significant risk factors and that of prevalence with increasing age between white matter lesion (WML) and silent lacunar infarction (SLI), 946 brain check-up subjects were investigated. Twelve risk factors including age, sex, and hypertension were analyzed using multiple logistic regression analysis. A case of WML, which changed into infarction, was presented. Significant (P < 0.01) risk factors for WML were aging (odds ratio; 1.12/year) and hypertension (3.66), while those for SLI were aging (1.10/year), hypertension (4.89), and retinal arteriosclerosis (3.46). Both WML and SLI emerged in middle age and thereafter the prevalence rates increased with age, although WML emerged earlier and was more frequent throughout than SLI. The results indicated that WML was essentially aging phenomenon, while SLI was strongly affected by vascular risk factors, although WML might possibly chang into infarction.

[Early detection of prostate cancer-results of a prostate specific antigen-based detection program at a "multiphasic screening"].

Determination was made of serum PSA in the early detection of prostate cancer in a population of 1,227 men aged 55 years or older who visited a "human dock". "Human dock" is a unique health examination facility open to public in Japan. Recommendation for biopsy was made based solely on this parameter when its value exceeded 2.0 ng/ml in IMx immunoenzymetric assay (Dinabot Co.). Of these, 162 (13.2%) had elevated values. The proportion of males with serum PSA greater than 4.0 ng/ml was only 3.6%. Of the 109 males who underwent ultrasound guided biopsies, 17 cancers were detected, the cancer detection rate thus being 1.4%. Most of these cases (82.4%) were clinically localized cancers. Eight patients with cancer had serum PSA levels below 4.0 ng/ml including 6 less than 3.0 ng/ml. Radical prostatectomy was conducted on 14 patients. All had histologic features of clinically significant cancer and 64.3% were pathologically confined. Oriental elderly males appear to have lower serum PSA than western counterparts. The optimal cutoff of serum PSA for early detection should be examined further in oriental male populations. Differences in the incidence of prostate cancer between ethnic groups may have been overestimated in previous studies owing in part to unawareness by the physician and general public. Though the application of PSA for early detection will likely disclose greater numbers of prostate cancers in Japan, whether early detection reduces mortality by minimizing risk of death from cancer remains a point to be clarified.

[Studies on background of gamma-GTP abnormality in human multiphasic screening. (Part 2): Principle component analysis by SAS statistic package for computer analysis].

In previous studies we reported that the abnormality of the gamma-GTP level was found not only in hepatobiliary diseases but also in obesity and ingestion of alcohol in healthy individuals. In relation to this results, the present study deals with the relationship between abnormal gamma-GTP level in serum on the one hand and obesity and ingestion of alcohol on the other hand on the basis of test data for 1493 cases (1256 male cases, 237 female cases) who were examined in the short-term human dry dock managed by the Fukuoka University Hospital. To observe how results of various measurements including the degree of obesity and the amount of alcohol ingested exert effects on one another, principle component analysis was done by means of SAS statistic package for computer analysis. In men, as the first principle main components, obesity rate, Hb, Ht, RBC, GPT, gamma-GTP, LAP, GOT, triglyceride and total protein were extracted. As the second main principle components, alcohol, gamma-GTP, MCH, MCV, and HDL were extracted. Theses result demonstrate that the obesity is accompanied by variations in Hb, Ht, RBC and triglyceride and that digestion of alcohol is accompanied by variations in MCH, MCV and HDL in male. In women, components related to erythrocyte and transaminase along with the degree of obesity were extracted as the first main principle components, and principle components related to erythrocyte and alcohol were extracted as the second main components. In either case, alcohol and gamma-GTP were not included.

[A study of smoking cessation education in a multiphasic screening setting].

A study of the effectiveness of individual anti-smoking advice was conducted on male smokers visiting a human dock. The intervention group (169 male smokers) were shown photographs depicting the health hazards of smoking with an explanation by tape recorder and headphone. Controls (187 male smokers) did not receive this instruction. In the intervention group, immediately after the instruction, and in the control group, approximately 60% expressed a desire to stop or decrease their smoking. Six months later there was no significant difference in the two groups in the proportion that stated that they were not smoking (8.9% in the intervention group and 5.9% in the control group). Some subgroups in the intervention group, such as those whose smoking index was less than 400 and those who had attempted to quit smoking in the past, had significantly higher quit rates than corresponding subgroups in the controls. Smokers who restricted their smoking in certain locations such as public areas had high quit rates in both groups. Standardized quit rates were utilized, adjusted for current age, age of smoking initiation, smoking index and past experience in smoking cessation. Immediately after receiving the education, the proportion that regarded smoking as a risk factor in gastric cancer, gastric ulcer, chronic bronchitis and as having a harmful effect on the fetus, was significantly larger in the intervention group (about 50-70%) than in the controls (about 30-60%). More than 90% of each group regarded it as a risk factor in lung cancer, and about 60% in myocardial infarction.(ABSTRACT TRUNCATED AT 250 WORDS)

[Studies on background of gamma-GTP abnormality in human multiphasic screening. (Part 1): Effect of obesity and ingestion of alcohol on gamma-GTP level in serum].

The present study deals mainly with the relationship between abnormal gamma-GTP level in serum on the one hand and obesity and ingestion of alcohol on the other hand on the basis of data for 1,493 cases (1,256 male cases, 237 female cases) who visited our clinic for their health check in the short-term human dry dock managed by the Fukuoka University Hospital. Abnormality in gamma-GTP serum level (> 60 mu/ml) was noted in 28.9% of men, which was second highest after 33.5% for the rate of triglyceride abnormality among the laboratory examinations performed in the human dry dock. In women the rate of abnormal LAP level in serum was the highest at 22.0%, while the rate of abnormal level of gamma-GTP was not more than 3%. The rate of abnormal gamma-GTP in serum increased with elevation in the obesity rate. Particularly in men there was a close relationship between the rate of abnormal gamma-GTP in serum and the amount of alcohol ingested, and the rate of abnormal gamma-GTP was 30.8% in men who take between 1 and 2 "gou" of Sake (1 "gou" of Sake contains 30g of alcohol) a day but reached 71.6% in case of the alcohol ingestion exceeding 2 "gou" of Sake. The relative risk ratio for abnormal gamma-GTP in serum was greater for alcohol than for obesity with 6.80 for those who are not obese but drink alcohol and 2.23 for those who are obese but do not drink alcohol. In both men and women, obesity plus ingestion of alcohol brought about a synergistic rather than additional effect. In men the relative risk ratio in the group having both obesity and ingestion of alcohol amounted to 11.5.

Initial radiographic findings of early gastric cancer detected in health check programs and human "dry dock" (multiphasic screening) collective health checks and treated by endoscopic mucosal resection.

BACKGROUND: The purpose of this study was to evaluate the capacity of detection (detectability), by radiographic examination, of minute gastric cancer that was indicated for endoscopic mucosal resection (EMR). METHODS: Fifty-five lesions in 55 patients with minute gastric cancer were endoscopically resected between 1992 and 2000 at the Foundation for Detection of Early Gastric Carcinoma. Of these 55 patients, 33 patients underwent indirect (with image intensifier and 100-mm roll film) and/or direct radiographic examination of the upper gastrointestinal tract as the initial screening examination, and subsequent upper gastrointestinal endoscopy. The remaining 22 patients underwent upper gastrointestinal endoscopy as the initial screening examination. As the first step, radiographic (indirect and direct) images were reviewed, and the abnormalities which had eventually led to the detection of the minute early gastric cancer (EGC) were reconfirmed. Then, they were analyzed in terms of the reproducibility of abnormal findings which enabled the detection of such a minute EGC, and in terms of whether they could be treated by EMR. The size, morphology, and location of the radiographically detected lesions were analyzed. RESULTS: Seventeen lesions of EGC were detected in the group in which indirect radiography was initially performed. Of these 17 lesions, some abnormality corresponding to the site of the lesion was pointed out in 7 lesions (2 lesions of type IIa and 5 lesions of type IIc). On the other hand, the remaining 10 lesions (type IIa, 3; type IIc, 5; type IIa + IIc, 1; and type IIb, 1) were detected by endoscopy, which was carried out to confirm the other abnormalities in the indirect radiographic images. Sixteen lesions of EGC were detected in the group in which direct radiography was initially performed. Of these 16 lesions, some abnormality corresponding to the site of the lesion was pointed out in 8 lesions (4 lesions of type IIa and 4 lesions of type IIc). The remaining 8 lesions (3 lesions of type IIa and 5 lesions of type IIc) were not detected by direct radiography. CONCLUSION: Indirect and direct radiographic examination enabled the detection of approximately 50% of EGCs which could be treated by endoscopic resection. The significance of radiographic examination should not be underestimated, even in the detection of EGCs which can be endoscopically resected.

[Relationships between serum lipid peroxide level (serum TBA level) and smoking, alcohol drinking, food frequency, serum vitamin C and E in subjects with multiphasic screening].

Relationships between serum thiobarbituric acid (TBA) level and smoking, alcohol drinking, differences in food intake with alcohol drinking, serum vitamin C (VC) and E (VE) were studied in 283 non-treated men (aged 20-69 years) who visited a human dock conducted in an urban area of Hyogo prefecture in 1989-1991 (annually May-July). The results were as follows: 1. An effect of smoking on serum TBA level was not observed. 2. The subjects were divided into three groups according to weekly sake intake levels, i.e., non-drinkers including ex-drinkers (ND), mild drinkers (MD, weekly sake intake: < or = 1260 ml), and heavy drinkers (HD, > 1260 ml). The serum TBA level of HD, but not of MD, was significantly higher than that of ND. 3. Serum TBA level had significant positive correlations with age, GPT, gamma-GTP, TC (total cholesterol), TG (triglycerides), PL (phospholipids), total serum lipids (TLP, i.e., TC+TG+PL), VE, eicosapentaenoic acid (EPA, C20:5) and docosahexaenoic acid (DHA, C22:6), but had no significant correlations with VC and VE/TLP. 4. The subjects (N+M) D, which included ND and MD, and those of HD were divided respectively into three groups according to weekly food intake levels, i.e., non-intake group (NF), low frequent intake group (LF, 1-3 days/week), and high frequent intake group (HF, > or = 4 days/week). The fish intake level of HD was significantly higher than that of (N+M) D, while the intake levels of vegetables, fruits and soft drinks of HD were significantly lower than those of (N+M) D. 5. Serum TBA level, GOT, GPT, gamma-GTP, TG, PL, TLP, VE, EPA and DHA of HD were significantly higher than those of (N+M) D. On the other hand, VC of HD was significantly lower than that of (N+M) D. 6. When the subjects of (N+M) D were divided into three groups according to weekly fish intake levels as mentioned above, serum TBA level, EPA and DHA of HF in (N+M) D were significantly higher than those of NF in (N+M) D. 7. Serum TBA level of HD in non-fish intake group was significantly higher than that of (N+M) D in the same group. These results suggest that increased serum TBA level in HD is closely related to the increased intake frequency of fish in addition to the effect of alcohol drinking.(ABSTRACT TRUNCATED AT 250 WORDS)

A multiphasic screening survey in San Jose.

In the interest of economy and of better service to the persons examined, three screening tests of a kind usually made separately were combined and applied in a single examination of 945 employees in four industrial establishments. Miniature x-ray films of the chest were taken, blood specimens drawn and urine samples obtained so that studies and tests might be made to determine the presence of pulmonary or cardiac disease, syphilis, kidney disease, or diabetes. The history of each person examined was taken. Through this multiphasic survey, 13 cases of significant disease previously unknown to the patient were discovered. Sixteen cases of significant disease, previously known, were also disclosed; and in several such cases treatment was begun or resumed. The results in case finding were considerably greater than those of the customary screening for a single disease.
