Overweight and aerobic fitness in children in the United States/Mexico border region.

OBJECTIVE: To study overweight and aerobic fitness among children in the third and fourth grades of elementary schools in a city in the United States of America (El Paso, Texas) and a city in Mexico (Chihuahua, Chihuahua) that are on or near the border between those two countries, and to compare the results from those two cities with earlier findings for other children in the United States. METHODS: We followed the El Paso children (427 boys and 385 girls, 93% of them of Mexican descent) from third to fourth grade and assessed the change in their body mass index (BMI). In the city of Chihuahua we cross-sectionally measured the BMI of a sample of third grade children (221 boys and 237 girls) and a sample of fourth grade children (268 boys and 215 girls). BMI and triceps skinfolds were measured for all the children studied in the two cities. BMI was used to assess risk for overweight (at least the 85th percentile BMI for age and gender) and overweight (at least the 95th percentile BMI for age and gender) in all the children. The distance that El Paso children ran in nine minutes was used to assess their aerobic fitness (aerobic fitness was not measured in the Chihuahua children). The data from El Paso were collected in 1999, 2000, and 2001, and the Chihuahua data were collected in 2000 and 2001. RESULTS: In the El Paso boys, overweight significantly increased in the one year from third grade to fourth grade, from 22% to 28%, while risk for overweight significantly increased, from 37% to 44%. In the El Paso girls, risk for overweight significantly increased over the same one-year period, from 29% to 37%. The El Paso boys and girls were significantly less fit when compared to samples of children from throughout the United States. Third and fourth grade children from Chihuahua had similar rates of risk for overweight and of overweight when compared to the children from the same grades in El Paso. CONCLUSIONS: Children in both El Paso and Chihuahua were more overweight than were non-Hispanic white children throughout the United States. In addition, the children in El Paso were less aerobically fit than were non-Hispanic white children and than were other Mexican-American children in the United States. These results clearly show that efforts should be made in the border regions of both Mexico and the United States to develop physical activity and nutrition programs to help stem rising rates of overweight.

Sonographic fatty liver, overweight and ischemic heart disease.

AIM: To demonstrate the prevalence of sonographic fatty liver, overweight and ischemic heart disease (IHD) among the male workers in Taiwan, and to investigate the possible association of these three factors. METHODS: From July to September 2003, a total of 2 088 male aircraft-maintenance workers aged from 22 to 65 years (mean 40.5) underwent an annual health examination, including anthropometrical evaluation, blood pressure measurement, personal medical history assessment, biochemical blood analysis, abdominal ultrasonographic examination and digital electrocardiography (ECG). The Student's t-test, chi(2) test and multivariate logistic regression analysis were utilized to evaluate the relationship between IHD and salient risk factors. RESULTS: The all-over prevalence of overweight was 41.4%, and that of fatty liver was 29.5% (mild, moderate and severe fatty liver being 14.5%, 11.3%, and 3.7%, respectively); while the prevalence of ischemic changes on ECG was 17.1% in this study. The abnormal rates for conventional IHD risk factors including hypertension, dyslipidemia, hyperglycemia and overweight increased in accordance with the severity of fatty liver. Overweight and severity of fatty liver were independently associated with increased risks for developing IHD. Overweight subjects had a 1.32-fold (95%CI: 1.01-1.73) increased IHD risk. Participants with mild, moderate, and severe fatty liver had a 1.88-fold (95%CI: 1.37-2.6), 2.37-fold (95%CI: 1.66-3.37) and 2.76-fold (95%CI: 1.62-4.72) increased risk for developing IHD. The prevalence of ischemic ECG for the fatty liver-affected subjects with or without overweight was 30.1% and 19.1%, while that of overweight subjects free from fatty liver was 14.4%. Compared to the subjects without fatty liver nor overweight, IHD risk for the three subgroups above was as follows: OR: 2.95 (95%CI: 2.31-4.09), OR: 1.60 (95%CI: 1.07-2.39) and OR: 1.11 (95%CI: 0.78-1.56), respectively. CONCLUSION: The presence of fatty liver and its severity should be carefully considered as independent risk factors for IHD. Results of the study suggest the synergistic effect between fatty liver and overweight for developing IHD. Abdominal sonographic examination may provide valuable information for IHD risk assessment in addition to limited report about liver status, especially for overweight males.

Leptin concentration in relation to body mass index (BMI) and hematological measurements in Thai obese and overweight subjects.

The weight, height and body mass index (BMI), including waist/hip ratio, serum leptin and hematological parameters of 48 male and 166 female overweight (BMI > or = 25.00) Thai volunteers who came for a physical check-up at the Out-patient Department, General Practice Section, Rajvithi Hospital, Bangkok during the period March-October 1998, were investigated. There were statistically significantly higher levels of serum leptin, mean corpuscular mean corpuscular hemoglobin concentration (MCHC) and mean corpuscular volume (MCV) in the overweight than in the control subjects. The median serum leptin concentration in overweight subjects was 19.6 (2.0-60.0 ng/ml) compared with 9.0 (range 1.0-30.0 ng/ml) in the control subjects (p < 0.001). The medians of leptin in overweight and obese males were significantly higher than those of overweight and obese females. 66.7% (32 out of 48) of overweight and obese males were found to have elevated leptin levels, while 87.3% (145 out of 166) were found in overweight and obese females. Anemia was found in 18.7% of female overweight and obese subjects, using hemoglobin as an indicator. Significant associations were found between weight, height, BMI, waist, hip, waist/hip ratio, hemoglobin, hematocrit, and serum leptin in both male and female overweight subjects. A negative correlation was found between serum leptin and hemoglobin, and hematocrit in both overweight and obese subjects.

B vitamins, vitamin C and hematological measurements in overweight and obese Thais in Bangkok.

The dynamic changes of socio-economics leading to the industrialisation of countries are known to affect lifestyle and nutritional behaviours of the population. Review of the literature on the prevalence of obesity showed increasing numbers of the overweight and obese during the past decade. However, information on health and nutritional status of the obese in Thailand has not been widely publicized. This study reveals the vitamin status and hematological picture in 270 overweight and obese Thais in Bangkok, Thailand, compared with 175 normal subjects. No statistically significant differences in haemoglobin and hematocrit were observed in the overweight compared with the control subjects. The prevalence of anaemia was 9.8 per cent among male and 17.2 per cent among female overweight and obese subjects compared with 2.6 per cent and 21.2 per cent in male and female normal controls using the cut-off point of haemoglobin concentration as an indicator of anaemia. Prevalence of hypertension was exhibited in both male and female overweight and obese subjects. Even if there were no statistically significant differences in vitamin B1, B2 and B6 in overweight and obese subjects compared with the controls, high percentages of vitamin C and vitamin B2 deficiencies were observed. Vitamin B2 deficiency was detected in 19.7 per cent of overweight and obese males as well as in 28.7 per cent of overweight and obese females using glutathione reductase activity coefficient (alpha EGR) < 1.5 as the cut-off point. However, clinical signs of vitamin B2 deficiencies were rare. There was also a high percentage of vitamin C (antioxidant vitamin) deficiency in 51.5 per cent of the overweight and obese subjects and 41.7 per cent of the controls, respectively. The results suggest more attention should be paid to health study and nutritional problems for the overweight and obese population, especially concerning vitamins and oxidative stress. Further research is still needed in these aspects.

Overweight and obesity mortality trends in Canada, 1985-2000.

OBJECTIVES: To investigate the temporal trends in the mortality burden attributed to overweight and obesity in Canada between 1985 and 2000. DESIGN: Overweight and obesity prevalence data from six cross-sectional national population surveys, including the 1985 and 1990 Health Promotion Surveys, 1994, 1996 and 1998 National Population Health Surveys, and 2000 Canadian Community Health Survey, in conjunction with one published prospective cohort study on overweight, obesity and mortality. PARTICIPANTS: Adults 20-64 years of age. MAIN OUTCOME MEASURE: The number of deaths attributable to overweight and obesity at the national and provincial levels. Attributable deaths were estimated using the Population Attributable Risk (PAR), which combined the prevalence data with the relative risks of mortality associated with overweight and obesity. A two-way sensitivity analysis was conducted by simultaneously varying the population prevalences and relative risk estimates by +/-10%. RESULTS: Between 1985 and 2000, the national PAR for overweight and obesity increased from 5.1% to 9.3%, and the annual number of deaths attributable to overweight and obesity increased from 2,514 (966-4,061) to 4,321 (2,114-6,542). Cumulatively, 57,181 (25,075-89,227) deaths were attributed to overweight and obesity between 1985 and 2000. Although overweight- and obesity-related mortality is increasing in every province, the problem is particularly pronounced in Eastern Canada. CONCLUSIONS: Overweight and obesity are important public health problems in Canada, accounting for approximately 57,000 deaths over the last 15 years. Immediate and sweeping public health campaigns and interventions are required to slow or reverse the recent trends.

Prevalence and determinants of overweight and obesity in Thai adults: results of the Second National Health Examination Survey.

To describe the prevalence of overweight and obesity and examine their relationship with socio-demographic factors in Thai adults. Using data from a cross-sectional survey, the National Health Examination Survey II (NHESII), the authors examined the prevalence of overweight (BMI > or = 25 kg/m2) and obesity (BMI > or = 30 kg/m2) in 3,220 Thai adults aged 20-59 yr. Univariate analyses and Logistic regression models were used to examine the association of overweight and obesity with socio-demographic and behavioral risk factors. The overall age-adjusted prevalence of overweight and obesity were 28.3% and 6.8% respectively, with a higher prevalence for women than for men (overweight: 33.9% vs 19.2% and obesity: 8.8% vs 3.5%). The prevalence of overweight and obesity was greater among older compared to younger people and among residents of urban (34.8% and 9.9%) compared to rural areas (26.4% and 5.9%). The prevalence of overweight and obesity varied by region in line with the level of economic development--Bangkok, Central, North, South and North-East. By using logistic regression analysis, overweight was associated with a number of characteristics as follows: age (per ten years increase) with adjusted Odds Ratio (OR) of 1.3; women 1.4; married 2.2; being a current smoker 0.4, and living in Bangkok and the central region 1.6 (compare to North-East). There was no clear difference in prevalence of overweight and obesity among education levels and type of occupation after controlling for other covariates. In conclusion, women of middle age, married, and living in Bangkok and the Central region, are at greater risk of overweight and obesity. Without effective lifestyle modification programs to curb these physiologic risk factors at population level, it is likely that related disease burden will ensue. Public health surveillance and intervention to modify the risk factors of excessive weight should be implemented.

Body mass index rebound and overweight at 8 years of age in hyperphenylalaninaemic children.

AIM: To evaluate whether the age at body mass index (BMI) rebound may be associated with overweight at age 8 y in hyperphenylalaninaemic (HPA) children. METHODS: A longitudinal observational study including 97 HPA children born 1984-1993 and detected by the National Neonatal Screening programme. Children were followed up at the same institution and evaluated for dietary intakes and anthropometrical parameters from diagnosis up to the age of 8 y. Outcome measure was overweight at age 8 y, defined according to the International Obesity Task Force. The age at BMI rebound, BMI before and at rebound were considered as potential determinants. Familial overweight, breastfeeding and macronutrients intake at age 1 y were considered as confounders. RESULTS: Mean (95% confidence interval [CI]) age at BMI rebound was 5.0 (4.7-5.3) y. At the age of 8 y, 24.7% (95% CI 16.3-33.1%) of children was overweight. Children overweight at the age of 8 y exhibited earlier BMI rebound than non-overweight children (mean difference [95% CI] -2.1 [-2.8 to - 1.4] y) and higher BMI from the age of 1 y (mean difference [95% CI] 1.2 [0.9-2.5] kg/ m2) onward. Overweight was more likely in children with, rather than without, parental overweight (41.0% vs 19.8%). After adjustment for confounders, logistic analysis showed that earlier BMI rebound (odds ratio [OR] 2.4, 95% CI 1.2-4.8) and BMI at age I y (OR 2.3, 95%CI 1.1-4.98) were independently associated with overweight at the age of 8 y. CONCLUSION: Within the population of this study, overweight at age 8 y was positively associated with early BMI rebound and BMI at age 1 y.

Trends in the association of poverty with overweight among US adolescents, 1971-2004.

CONTEXT: Prevalence of adolescent overweight in the United States has increased substantially during the past 3 decades. Whether socioeconomic disparities in adolescent overweight increased, decreased, or remained constant during this period is not known. OBJECTIVE: To examine trends in adolescent overweight from 1971 to 2004 by family poverty status, as well as trends in potentially relevant eating and physical activity behaviors. DESIGN, SETTING, AND PARTICIPANTS: Four cross-sectional, nationally representative surveys (US National Health and Nutrition Examination Surveys [NHANES] of 1971-1974, 1976-1980, 1988-1994, and 1999-2004) were examined for trends in the prevalence of overweight among adolescents aged 12 to 17 years by family poverty status. MAIN OUTCOME MEASURES: Prevalence of adolescent overweight, defined as body mass index at or above the 95th percentile for age and sex in the 2000 Centers for Disease Control and Prevention growth charts. Intermediate outcomes were physical inactivity in the past 30 days, proportion of caloric intake from sweetened beverages (24-hour recall), and whether respondent skipped breakfast (24-hour recall). RESULTS: Trends in the association of adolescent overweight with family poverty differed by age stratum (P = .01). In 12- to 14-year-old adolescents, prevalence did not significantly differ by family poverty status in any of the surveys; however, among non-Hispanic black adolescents, overweight prevalence increased faster in nonpoor vs poor families. In contrast, a widening disparity that disfavored adolescents from poor families was present in the 15- to 17-year-old adolescents. This trend was similar among male, female, non-Hispanic white, and non-Hispanic black adolescents, resulting in an overall prevalence of overweight in 1999-2004 more than 50% higher among adolescents in poor vs nonpoor families (23.3% vs 14.4%, respectively; P<.001). Additional analyses suggest that physical inactivity, sweetened beverage consumption, and skipping breakfast may contribute to these disparities. CONCLUSIONS: Trends of increasing overweight showed a greater impact in families living below the poverty line vs not living below the poverty line among older (15-17 years) but not younger (12-14 years) adolescents. Furthermore, physical inactivity, high consumption of sweetened beverages, and breakfast skipping may be candidate targets for prevention programs aimed at reducing this recently emerged disparity.

Attitude toward physical activity in normal-weight, overweight and obese adolescents.

PURPOSE: To investigate differences in physical activity and attitude toward physical activity in adolescents with different degrees of overweight and explore whether the prediction of physical activity by attitude is moderated by level of overweight. METHODS: Subjects were divided into a normal-weight group (n = 37, 18.8 +/- 1.2 kg/m2), an overweight group (n = 28, 25.9 +/- 1.3 kg/m2), and an obese group (n = 24, 33.7 +/- 4.1 kg/m2). Mean age was 14.6 +/- 1.2 years, with 72% girls. Physical activity was estimated using the Baecke Questionnaire. Attitude was measured by assessing perceived benefits and barriers. RESULTS: Participation in sports was higher in normal-weight compared with overweight (p < .05) and obese (p < .01) subjects. There was no difference in leisure-time physical activity between groups. Perceived benefits did not differ between groups, but normal-weight subjects perceived less barriers ('physical complaints', 'not being good at it', 'insecure about appearance', 'not liking it') than their overweight (p < .05) and obese (p < .001) counterparts. Obese adolescents had a less positive attitude compared with their normal-weight (p < .001) and overweight (p < .05) peers. Sport participation was significantly predicted by the perceived benefit 'pleasure' (p < .05) and by the perceived barrier 'not liking it' (p < .001), after taking into account level of overweight. The association between sport participation and attitude was not moderated by level of overweight. CONCLUSIONS: This study demonstrates that overweight and obese adolescents show lower sport participation and have a less positive attitude toward physical activity. Interventions in youngsters with weight problems should try to increase participation in sports by making activities more fun and attractive for these youngsters.

Heat production and body temperature during cooling and rewarming in overweight and lean men.

OBJECTIVE: To compare overweight and lean subjects with respect to thermogenesis and physiological insulation in response to mild cold and rewarming. RESEARCH METHODS AND PROCEDURES: Ten overweight men (mean BMI, 29.2 +/- 2.8 kg/m(2)) and 10 lean men (mean BMI, 21.1 +/- 2.0 kg/m(2)) were exposed to cold air for 1 hour, followed by 1 hour of rewarming. Body composition was determined by hydrodensitometry and deuterium dilution. Heat production and body temperatures were measured continuously by indirect calorimetry and thermistors, respectively. Muscle activity was recorded using electromyography. RESULTS: In both groups, heat production increased significantly during cooling (lean, p = 0.004; overweight, p = 0.006). The increase was larger in the lean group compared with the overweight group (p = 0.04). During rewarming, heat production returned to baseline in the overweight group and stayed higher compared with baseline in the lean group (p = 0.003). The difference in heat production between rewarming and baseline was larger in the lean (p = 0.01) than in the overweight subjects. Weighted body temperature of both groups decreased during cold exposure (lean, p = 0.002; overweight, p < 0.001) and did not return to baseline during rewarming. DISCUSSION: Overweight subjects showed a blunted mild cold-induced thermogenesis. The insulative cold response was not different among the groups. The energy-efficient response of the overweight subjects can have consequences for energy balance in the long term. The results support the concept of a dynamic heat regulation model instead of temperature regulation around a fixed set point.

Risk factors for childhood overweight.

PURPOSE OF REVIEW: We review the 2004 literature on risk factors for childhood overweight. Given the steady increase in the prevalence of childhood overweight, the identification of risk factors is of increasing importance. RECENT FINDINGS: A number of studies confirmed that parental overweight is the most potent risk factor for childhood overweight. Childhood temperament was found to mediate the effects of parental overweight with a difficult temperament increasing the risk of overweight. Newer findings implicate regular consumption of fast food and sweetened drinks as risk factors, as well as sedentary behavior; although a meta-analysis suggested that the effects of the latter factor are small. Other work was consistent with the finding that parental overcontrol of children's feeding behavior, particularly for those at high risk of developing overweight, may lead to overweight. SUMMARY: Identification of groups of children at high risk for becoming overweight offers the potential for early intervention to reduce the intergenerational transfer of obesity. This is an area for future research because the nature of effective intervention is at present unclear. There is a suggestion that focused behavior change efforts on one target such as reducing the intake of sweetened beverages may be more successful than attempts to make broader dietary changes. There is also evidence that targeting parental behaviors may be more effective than interventions directed solely toward children, suggesting that modification of the environment that a child is exposed to during early development may have a lasting effect.

The association between stunting and overweight in Latin American and Caribbean preschool children.

BACKGROUND: Although some segments of the population continue to suffer from undernutrition, other groups exhibit excess weight gain, resulting in the coexistence of undernutrition and obesity and leading to a dual nutritional burden. OBJECTIVE: To explore the association between stunting and overweight in preschool children from Latin American and Caribbean countries. METHODS: We analyzed cross-sectional data from children 0 to 5 years of age from 79 nationally representative surveys, compiled by the World Health Organization (WHO) Global Database on Child Growth and Malnutrition. This database defines stunting as low height-for-age and overweight as high weight-for-height. These variables were explored with the use of simple and multiple regression models. RESULTS: There were significant differences between subregions in the prevalence of stunting: the prevalence was 7.4% in the Caribbean, 11.3% in South America, and 20.4% in Central America (p < .001). In contrast, the estimated prevalence of overweight was similar between subregions. The overall prevalence rates of stunting and overweight in Latin America and the Caribbean in the year 2000 were 13.7% and 4.3%, respectively. We found an inverse relationship (r = -0.3) between the prevalence rates of overweight and stunting, overall and within subregions. South America exhibited the highest slope and intercept on the regression of overweight on stunting. CONCLUSIONS: Different subregions of Latin America and the Caribbean have different prevalence rates of childhood stunting but similar prevalence rates of overweight. There is an inverse relationship between stunting and overweight. The South American subregion had the highest increase and prevalence of overweight of the Latin American region.

Parenting styles and overweight status in first grade.

OBJECTIVE: The goal was to determine the relationship between the 4 parenting styles (authoritative, authoritarian, permissive, and neglectful) and overweight status in first grade. METHODS: Data from the National Institute of Child Health and Human Development Study of Early Child Care and Youth Development were analyzed. Children with complete data for parenting parameters at 54 months and measured weight and height in first grade were included in the analysis. Overweight was defined as BMI of > or =95th percentile. The 4 parenting styles were constructed with 2 scales, namely, maternal sensitivity and maternal expectations for child self-control. Multivariate logistic regression analysis was used to evaluate the relationship between parenting style and overweight in first grade, controlling for gender, race, maternal education, income/needs ratio, marital status, and child behavior problems. RESULTS: A total of 872 children, 11.1% overweight and 82.8% white, were included in the analysis. Children of authoritarian mothers (n = 298) had an increased risk of being overweight, compared with children of authoritative mothers (n = 179). Children of permissive (n = 132) and neglectful (n = 263) mothers were twice as likely to be overweight, compared with children of authoritative mothers. Of the covariates, only income/needs ratio was significant and did not alter the relationship between parenting style and overweight risk. CONCLUSIONS: Among the 4 parenting styles, authoritarian parenting was associated with the highest risk of overweight among young children. Understanding the mechanisms through which parenting styles are associated with overweight risk may lead to the development of more-comprehensive and better-targeted interventions.

[Maternal overweight and obesity in Mexican school-age children. National Nutrition Survey, 1999].

OBJECTIVE: To assess the effects of maternal overweight and obesity as risk factors for overweight and obesity in Mexican school-age children (5 to 11 years old) who participated in the National Nutrition Survey 1999 (NNS-99). MATERIAL AND METHODS: Information obtained by the NNS-99 was used to evaluate the relationship between maternal overweight and obesity and overweight or obesity in school-age children. Multivariate logistic regression analysis was used. RESULTS: A total of 9 259 children were studied. The prevalence of overweight or obesity was 19%, according to the criteria suggested by Cole.Thirty-nine percent of mothers were overweight and 26.4% were obese. After controlling for age, gender, and other sociodemographic characteristics, children of overweight mothers had a 1.9 times higher risk of being obese (95%CI 1.62-2.18), and children of obese mothers had a 3.4 times higher risk of being obese (95%CI 2.96-4.00), compared to children of mothers with normal BMI. CONCLUSION: Maternal overweight and obesity are risk factors for overweight or obesity in Mexican school-age children. Interventions to prevent obesity in children should be aimed towards promoting family lifestyle changes and modifying obesigenic environments.

[The prevalence of body overweight and obesity and its changes among Chinese people during 1992 to 2002].

OBJECTIVE: To describe the prevalence of body overweight and obesity and its changes among Chinese people during 1992-2002, and to provide scientific basis for developing intervention strategies for obesity in China. METHODS: The data from "1992 China third National Nutrition Survey" (78,704 subjects, 38,323 male and 40,381 female) and from "2002 China National Nutrition and Health Survey" (209,849 subjects, 101,377 male and 108,472 female) were used in this study. The overweight and obesity were defined by using WHO Z-score criteria for children younger than 7 years old, Chinese age-, sex-specific BMI criteria for children aged 7-17 years, and Chinese BMI criteria for adults. RESULTS: The overall prevalence of overweight and obesity of Chinese people was increased by 38.6% and 80.6%, respectively during the period of 1992-2002. The prevalence of overweight and obesity of Chinese people aged 0-6 years, 7-17 years, 18-44 years, 45-59 years and above 60 years was increased 31.7%, 17.9%, 66.7%, 45.2% and 43.7%, respectively. The increase of the prevalence of overweight and obesity was higher in girls younger than 6 years than their male counterparts, while among other age groups, that were higher in boys than girls. The prevalence of overweight and obesity among people living in rural areas was lower than that of their urban counterparts, while the increment of overweight and obesity prevalence among rural people was greater than that of their urban counterparts. It was estimated that another 70 million overweight and 30 million obese Chinese people emerged in China from 1992 to 2002. CONCLUSION: The prevalence of overweight and obesity of Chinese people was increased rapidly in the past decade, which had affected 260 million Chinese people. It would continue to increase in the near future if effective intervention measures have not been taken.

[Epidemiology of obesity and overweight and relation thereof to the prevalence of hypertension in 14 provinces/municipality in China].

OBJECTIVE: To investigate the current epidemiological characteristics of obesity and overweight in the middle-aged and elderly populations of china and to disclose their relation with such chronic diseases as hypertension. METHODS: A questionnaire survey and physical examination were conducted among the 35-85-years-old persons from 14 densely populated residential areas representative in economic development and medical conditions in 14 provinces/municipality in China (i.e., Henna, Hebei, Shandong, Shanxi, Guangdong, Jiangxi, Tianjin, Inner Mongolia, Shaanxi, Hunan, Hubei, Yunnan, Sichuan, and Zhejiang) where a large-scale epidemiological survey of atrial fibrillation were being conducted, one residential area for a province, selected by cluster sampling to investigate the health-related personal conditions and examine the blood pressure (BP), body height, weight, body mass index (BMI), etc. so as to understand the prevalence of overweight and obesity based on the diagnostic criteria of Chinese adults recommended by the Working Group on Obesity in China. RESULTS: 29056 persons, 13549 males with the average body height of 168 cm +/- 7 cm, average weight of 68 kg +/- 11 kg, and average body mass index (BMI) of 24 kg/m(2) +/- 3 kg/m(2), and 15507 females with the average body height of 156 cm +/- 6 cm, average weight of 60 kg +/- 10 kg, and average BMI of 24 kg/m(2) +/- 4 kg/m(2), were surveyed. The BMI of women was significantly higher than that of men (P = 0.005). The total prevalence of overweight in the 14 provinces/municipality was 38.93%, and the standardized prevalence of overweight was 37.17%, the total prevalence of obesity was 13.94%, and the standardized prevalence was 12.63%. The prevalence of obesity of men was 12.29%, significantly lower then that of women (15.39%, P < 0.001). The prevalence of overweight of men was 412.18%, significantly higher then that of women (36.97%, P < 0.001). The prevalence rates of obesity and overweight increased with age in the age group 35-64; and decreased with age in the age group 65 and over. In the men, the prevalence of overweight was the highest in the age group 45-54 (44.57%), and the prevalence of obesity was the highest in the age 55-64 (13.60%). In the women, the prevalence of overweight was the highest in the age group 55-64 (40.10%), and the prevalence of obesity was the highest in the age group 65-74 (19.97%). The prevalence of hypertension increased obviously with the BMI in these populations. CONCLUSION: Overweight and obesity are prevalent in China. Control of body weight is of great importance to the prevention and treatment of such chronic diseases as hypertension.

Increasing prevalence of overweight among US adults. The National Health and Nutrition Examination Surveys, 1960 to 1991.

OBJECTIVE: To examine trends in overweight prevalence and body mass index of the US adult population. DESIGN: Nationally representative cross-sectional surveys with an in-person interview and a medical examination, including measurement of height and weight. SETTING/PARTICIPANTS: Between 6000 and 13,000 adults aged 20 through 74 years examined in each of four separate national surveys during 1960 to 1962 (the first National Health Examination Survey [NHES I]), 1971 to 1974 (the first National Health and Nutrition Examination Survey [NHANES I]), 1976 to 1980 (NHANES II), and 1988 to 1991 (NHANES III phase 1). RESULTS: In the period 1988 to 1991, 33.4% of US adults 20 years of age or older were estimated to be overweight. Comparisons of the 1988 to 1991 overweight prevalence estimates with data from earlier surveys indicate dramatic increases in all race/sex groups. Overweight prevalence increased 8% between the 1976 to 1980 and 1988 to 1991 surveys. During this period, for adult men and women aged 20 through 74 years, mean body mass index increased from 25.3 to 26.3; mean body weight increased 3.6 kg. CONCLUSIONS: These nationally representative data document a substantial increase in overweight among US adults and support the findings of other investigations that show notable increases in overweight during the past decade. These observations suggest that the Healthy People 2000 objective of reducing the prevalence of overweight US adults to no more than 20% may not be met by the year 2000. Understanding the reasons underlying the increase in the prevalence of overweight in the United States and elucidating the potential consequences in terms of morbidity and mortality present a challenge to our understanding of the etiology, treatment, and prevention of overweight.

Risk of overweight among adolescents who were breastfed as infants.

CONTEXT: Overweight during adolescence predicts short- and long-term morbidity as well as obesity in adulthood. The prevalence of overweight among adolescents is high and continues to increase. Physiological and behavioral mechanisms and preliminary epidemiologic data suggest that breastfeeding could lower the risk of subsequent obesity in adolescence. OBJECTIVE: To examine the extent to which overweight status among adolescents is associated with the type of infant feeding (breast milk vs infant formula) and duration of breastfeeding. DESIGN, SETTING, AND SUBJECTS: Survey of 8186 girls and 7155 boys, aged 9 to 14 years, who are participants in the Growing Up Today Study, a nationwide cohort study of diet, activity, and growth. In the fall of 1996 we mailed a questionnaire to each of the subjects, and in the spring of 1997, we mailed a supplemental questionnaire to their mothers, who are participants in the Nurses' Health Study II. MAIN OUTCOME MEASURE: Overweight status defined as body mass index exceeding the 95th percentile for age and sex from US national data. RESULTS: In the first 6 months of life, 9553 subjects (62%) were only or mostly fed breast milk, and 4744 (31%) were only or mostly fed infant formula. A total of 7186 subjects (48%) were breastfed for at least 7 months while 4613 (31%) were breastfed for 3 months or less. At ages 9 to 14 years, 404 girls (5%) and 635 boys (9%) were overweight. Among subjects who had been only or mostly fed breast milk, compared with those only or mostly fed formula, the odds ratio (OR) for being overweight was 0.78 (95% confidence interval [CI], 0.66-0.91), after adjustment for age, sex, sexual maturity, energy intake, time watching television, physical activity, mother's body mass index, and other variables reflecting social, economic, and lifestyle factors. Compared with subjects who had been breastfed for 3 months or less, those who had been breastfed for at least 7 months had an adjusted OR for being overweight of 0.80 (95% CI, 0.67-0.96). Timing of introduction of solid foods, infant formula, or cow's milk was not related to risk of being overweight. CONCLUSION: Infants who were fed breast milk more than infant formula, or who were breastfed for longer periods, had a lower risk of being overweight during older childhood and adolescence.
