[Origin of psychiatric disorders ex sexualibus: explanatory approaches at the beginning of the 19th century].

In the first half of the 19th century there were the first publications, which dealt systematically with the relation between deviations of the sexual system or behaviour and mental disturbances. The knowledge and the theoretical ideas of medicine and of psychiatric diseases at that time and a discussion concerning the consequences of masturbation can be demonstrated in works like "De mentis aberrationibus ex partium sexualium conditione abnormi oriundis" by Hermann Joseph Löwenstein (1823) and "Ueber die Beziehungen des Sexualsystemes zur Psyche überhaupt und zum Cretinismus ins Besondere" by Joseph Häussler (1826). Both publications try to sytemize the issue and prove their hypotheses by numerous casuistics. The autors belong to the so called "Somatis School" in psychiatry at that time.

The dysfunction of DSM-II (Diagnostic and Statistical Manual of Mental Disorders).

The task of seeking alternatives to a classification system such as DSM-II that will maximize its benefits and minimize its limitation is a complex and arduous one. There are no clear-cut answers. Certain issues had been elaborated on here. The task at hand is one that must be approached on a multiprofessional basis seeking to integrate a variety of theoretical bases into a functional conceptual framework that will accomplish the purpose of a classification scheme. There is also need to conduct systematic data collection in an effort to determine operational definitions related to the phenomenon that is being studied. There is a movement by some professionals to have more emphasis placed on the social functioning level of the individual and his significant othersmif a concerted effort is not begun to look at this problem it will remain unchanged for the most part or other interested professionals will have changed it with little impact from nursing.

A prevalence study of bestiality (zoophilia) in psychiatric in-patients, medical in-patients, and psychiatric staff.

The prevalence of bestiality (both actual sexual contacts and sexual fantasy) was investigated in an experimental group (psychiatric in-patients) and two control populations (medical in-patients and psychiatric staff). Psychiatric patients were found to have a statistically significant higher prevalence rate (55%) of bestiality than the control groups (10% and 15% respectively). Implications of these findings are discussed. It is recommended that due to the obvious prevalence of this condition, questions exploring this previously ignored topic should be routinely included in the psychiatric interview.

Ethical and legal aspects of imposed sexological treatment.

And if everything is so nebulous about a matter so elementary as the morals of sex, what is there to guide us in the more subtle morality of all other personal contacts, associations and activities? Or are we meant to act on impulse alone? It is all the 'darkness': Ford-Madox-Ford.

Three cases of paraphilias responsive to fluoxetine treatment.

Three cases are presented in which fluoxetine was employed in the apparently successful treatment of paraphilia, a disorder which has been difficult to treat pharmacologically. Therapeutic benefit may have been related to the efficacy of fluoxetine in the treatment of obsessive compulsive disorders or to direct effects on sexual activity, or both.

A perversion named desire.

Inspired by the title of the work, A Streetcar Named Desire by Tennessee Williams, the author argues that desire and perversion are equivalent, based on the following. (1) The notion of infanatile sexuality as polymorphously perverse, and whose components constitute the 'core of our unconscious' and the ultimate 'matter' of unconscious desire. (2) The perspective of narcissism, which establishes desire, supported by the theory that maternity implants an illusion of completeness which culture promotes, despite the fact that such an ideal conflicts with the laws governing the foundation of culture. (3) The resulting forms of the oedipal complex and the castration complex, whether man or woman, imply a visible or unapparent violence with regard to the original 'call' of desire. (4) The traditional structures refer to the possibility of the mother imagining herself as completed by the child: the blocking of that illusion is associated with psychosis; the weakness of the desire, once established, demands, in the context of perversion, the presence of the figure of plenitude. Neurosis prefers its absence on the level of the apparent while insuring its permanence on the unconscious level.

[The level of sex hormones in men with a sexual behavior disorder in the form of exhibitionism].

The paper is concerned with the results of radioimmunological studies reflecting the importance of the function of the hypothalamohypophyseal-gonadal system in 29 men with the signs of organic brain lesions, against whom criminal proceedings were instituted because of exhibitionism. Among the examinees the subgroups with the hypothalamic and temporal syndromes were distinguished. The data obtained were compared to those in 25 mentally normal men and 17 examinees with residual organic brain lesions without sexual behavior abnormalities. The data derived have demonstrated the impairment of the function of the indicated system in the test subjects, associated with sexual behavior impairment in the form of exhibitionism which may form the biological basis for distortion of sexual self-consciousness.

Tourette's disorder and associated complex behaviors: a case report.

A case of a man with Tourette's disorder associated with obsessive-compulsive disorder, multiple sexual paraphilias, and aggressive behavior is described. Treatment with haloperidol led to improvement in the characteristic tics of Tourette's disorder as well as to improvement in these three complex-associated behaviors. After haloperidol was discontinued, an exacerbation of tics and the associated behaviors occurred.

Legal and ethical issues in the use of antiandrogens in treating sex offenders.

Antiandrogen treatment of sexual offenders raises serious legal and ethical considerations in both the medical profession and in the courts. Discussion is offered on the use of antiandrogens in both an involuntary and voluntary context. The potential negative impact of this treatment modality on such constitutional issues as privacy interests, right to procreative freedom, freedom of speech and communication, and freedom from cruel and unusual punishment is explored and notable, germane court cases are presented. The need for clear ethical guidelines for the administration of this treatment is stressed.
