Tilted view reconstruction in optical microscopy. Three-dimensional reconstruction of Drosophila melanogaster embryo nuclei.

The resolution along the optical axis (z) is much less than the in-plane resolution in any current optical microscope, conventional or otherwise. We have used mutually tilted, through-focal section views of the same object to provide a solution to this problem. A tilting specimen stage was constructed for an optical microscope, which with the use of a coverslip-free water immersion lens, allowed the collection of data sets from intact Drosophila melanogaster embryos at viewing directions up to 90 degrees apart. We have devised an image processing scheme to determine the relative tilt, translation, and sampling parameters of the different data sets. This involves the use of a modified phase cross-correlation function, which produces a very sharp maximum. Finally the data sets are merged using figure-of-merit and local area scaling techniques borrowed from x-ray protein crystallography. We demonstrate the application of this technique to data sets from a metaphase plate in an embryo of Drosophila melanogaster. As expected, the merged reconstruction combined the highest resolution available in the individual data sets. As estimated from the Fourier transform, the final resolution is 0.25 microns in x and y and 0.4 microns in z. In the final reconstruction all ten chromosome arms can be easily delineated; this was not possible in the individual data sets. Within many of the arms the two individual chromatids can be seen. In some cases the chromatids are wrapped around each other helically, in others they lie alongside each other in a parallel arrangement.

Effects of radiation therapy on reconstruction of mandibular defects with a titanium reconstruction plate.

Titanium fixation plates are commonly used in reconstruction of mandibular defects resulting from tumor resections in head and neck surgery. The effects of radiation therapy on the interface of bone, plate, and soft tissue were examined in this in vivo study. Four conditioned beagles had 1-cm segmental mandibular defects that were reconstructed with titanium plates. Two of the four also had placement of vascularized bone grafts. Healing was evaluated with or without postoperative radiotherapy. After collection of tissues using histologic methods and analysis with energy-dispersive spectroscopy, we found that the effects of radiation on bone-plate and bone-screw interfaces is minimal. Although radiation decreased the bone density and the rate of bone repair at the bone-screw interface, this did not appear to affect stability of the plate repair or the viability of bone tissue.

[Comparative radiological studies of different types of gastric reconstruction: Roux-Y; Longmire-Guetgemann; Tübinger reconstruction (author's transl)].

The indications for gastrectomy are increasing. More than 50 operations have been devised for reconstruction of the gastric function, but none is ideal. They should be judged particularly by their ability to form a reservoir which permits intermittent empyting of food into the small bowel. The risk of ischaemic changes or of chronic mucosal inflammation must be avoided. On the assumption that functional disturbances could be characterised by study of the morphology, we have carried out studies of three different types of gastric reconstruction (Longmire-Gütgemann, Roux-Y and Tübinger). The parameters we chose to measure were the maximum diameter of the interposed segment, the thickness of the mucosal folds and, as far as possible, th length of the segment. Most marked dilatation was found with the Tübinger procedure. Nevertheless, the increased occurrence of circular contractions following the Longmire-Gütgemann and Roux techniques may also result functionally in the formation of a reservoir. In none of the above techniques was there evidence of disturbances of blood-flow, as shown by mucosal swelling in the interposed segment.

Segmented multiple plane reconstruction: a novel approximate reconstruction scheme for multi-slice spiral CT.

A new reconstruction scheme for multi-row spiral CT is described and results are presented. The spiral path is decomposed into small, overlapping segments which are used for a separate convolution and backprojection yielding a stack of segment images which contain only projection data of a partial scan (typically in the range of 20). These segment image stacks are, in a second step, reformatted to the requested image planes. In a third step, the reformatted segment images are added to obtain full images. The main benefit of the proposed algorithm is superior images quality. A 64-row dataset with a cone angle of 6.4 and a table feed of 80 mm per spiral turn has been reconstructed with excellent image quality. A filter direction for three-dimensional (3D) backprojection algorithms is suggested by investigating the limit where the partial scan size goes to zero.

An accurate iterative reconstruction algorithm for sparse objects: application to 3D blood vessel reconstruction from a limited number of projections.

Based on the duality of nonlinear programming, this paper proposes an accurate row-action type iterative algorithm which is appropriate to reconstruct sparse objects from a limited number of projections. The cost function we use is the Lp norm with p approximately 1.1. This norm allows us to pick up a sparse solution from a set of feasible solutions to the measurement equation. Furthermore, since it is both strictly convex and differentiable, we can use the duality of nonlinear programming to construct a row-action type iterative algorithm to find a solution. We also impose the bound constraint on pixel values to pick up a better solution. We demonstrate that this method works well in three-dimensional blood vessel reconstruction from a limited number of cone beam projections.

Spiral scan long object reconstruction through PI line reconstruction.

The response of a point object in a cone beam (CB) spiral scan is analysed. Based on the result, a reconstruction algorithm for long object imaging in spiral scan cone beam CT is developed. A region-of-interest (ROI) of the long object is scanned with a detector smaller than the ROI, and a portion of it can be reconstructed without contamination from overlaying materials. The top and bottom surfaces of the ROI are defined by two sets of PI lines near the two ends of the spiral path. With this novel definition of the top and bottom ROI surfaces and through the use of projective geometry, it is straightforward to partition the cone beam image into regions corresponding to projections of the ROI, the overlaying objects or both. This also simplifies computation at source positions near the spiral ends, and makes it possible to reduce radiation exposure near the spiral ends substantially through simple hardware collimation. Simulation results to validate the algorithm are presented.

Double composite lid reconstruction: a new method of upper and lower lid reconstruction.

The double composite grafting technique utilizes two composite lid grafts from an opposing upper and lower lid for reconstruction of extensive lid defects. These two composite grafts are covered by a rotational pedicle advancement skin flap. The skin defect created by such an advancement graft is covered by a free skin graft. The donor areas are closed directly by a layer-by-layer approximation. This new single-stage technique allows reconstruction of large lid defects without covering an affected eye for a prolonged period and without jeopardizing the irreplaceable entity of a "normal" lid margin.

Primary mandibular reconstruction with the titanium hollow screw reconstruction plate: evaluation of 51 cases.

Fifty-one patients undergoing surgical reconstruction of mandibular defects with the titanium hollow screw reconstruction plate over a 4-year period were analyzed. Plate failure was defined as flap necrosis, plate extrusion, or plate fracture necessitating a further surgical procedure and occurred in 12 patients (24 percent). The incidence was highest for patients who had more than three mandibular regions resected. It was not affected by the primary site of the tumor or whether the patient had received radiation therapy. Although the failure rate was the same whether the patient attained full oral function or was fed by gastrostomy, plate fracture was limited to the former group. The overall cause-specific survival rate for this group of patients was 68 and 56 percent at 1 and 2 years, respectively. In addition, 81 percent of patients attained a full oral diet.

Functional reconstruction of swallowing and articulation after total glossectomy without laryngectomy: money pouch-like reconstruction method using rectus abdominis myocutaneous flap.

When a total glossectomy is performed without a laryngectomy, the functional recovery of swallowing and articulation is extremely important in maintaining the patient's quality of life. The authors established a money pouch-like reconstruction method in which a round and raised tongue is rebuilt using a rectus abdominis myocutaneous flap. In this method, the skin island of the rectus abdominis myocutaneous flap is created about 20 percent larger than the defect in both width and length. The skin island is sutured to the defect such that the excess skin folds into the shape of a money pouch. This allows the tongue to be reconstructed with its tip and dorsum touching the hard palate and its base bulging in the dorsal and posterior directions. Misswallowing was not detected in the three patients who had this surgery, and each of them had improved articulation sufficient to carry out daily conversations.

1D lattice distortions as the origin of the (2 x 2)p4gm reconstruction in gamma'-Fe4N(100): a magnetism-induced surface reconstruction.

The (100) surface of magnetic gamma'-Fe4N films epitaxially grown on Cu(100) shows a (2 x 2)p4gm reconstruction. The atom positions determined experimentally are properly accounted for by first-principles total energy calculations, which further reveal that the (2 x 2)p4gm reconstruction requires the topmost Fe4N2 plane to be magnetic and effectively decoupled from the rest of the crystal and that it originates from the perpendicular crossing of one-dimensionally dimerized Fe chains.

Reconstruction for chronic lateral ankle instability using the palmaris longus tendon: is reconstruction of the calcaneofibular ligament necessary?

The palmaris longus tendon was used to reconstruct the anterior talofibular ligament (ATFL) in 27 ankles with chronic lateral instability. The mean age of the patients at surgery was 23 years, and the follow-up was more than 2 years. The functional evaluation showed excellent or good results in all ankles. Twenty-seven ankles were divided into two groups according to operative findings: group A consisted of 11 ankles with old isolated injury of the ATFL, and group B consisted of 16 ankles with old combined injuries of the ATFL and the calcaneofibular ligament. There were no significant differences in clinical results between group A and group B. The preoperative mean talar tilt angles on stress radiograph in group B were significantly larger than those in group A. At follow-up, there were no significant differences in the mean talar tilt angles between group A and group B. We demonstrate that reconstruction of the calcaneofibular ligament along with the ATFL is not necessary for patients with chronic combined lateral ligament instability.

Use of plastipore for ossicular chain reconstruction: an evaluation.

With the exception of stapes prostheses, previous use of implantable alloplastic materials in reconstructing the sound conducting mechanism in middle ear surgery has been uniformly unsuccessful. A new material composed of ultra-high molecular weight polyethylene has been developed as a replacement for middle ear ossicles. This implant, called the Plastipore total ossicular replacement prosthesis (TORP), and the Plastipore drum to the stapes prosthesis were inserted in 87 patients, all of whom had a long history of extensive middle ear disease and/or surgery. One TORP patient was lost to follow-up. The overall average hearing gain was 14.8 db. The overall follow-up was 12.4 months with a range of from 3 to 25 months. Eleven of the 15 patients receiving the drum-to-stapes prosthesis had hearing gain; 55 of 71 patients receiving the TORP had hearing gain. Eight prostheses were extruded. Nine patients had no improvement in hearing. Eight patients had subsequent decrease in hearing after initial gains. All ears are now dry and aerated. Early evaluation appears to demonstrate that the Plastipore prosthesis shows promise for use in difficult reconstructive middle ear problems.

[Value of osteo-chondral paste autologous transplantation in experimental cartilage defects reconstruction. Part III--Microscopic analysis of reconstructed cartilage thickness and surface regularity].

INTRODUCTION: A limited ability of the cartilage to heal after trauma was the reason to start research on new methods concerning better cartilage reconstruction. The aim of the study was evaluation of repair tissue thickness and surface regularity after osteochondral paste transplantation. MATERIAL AND METHODS: Full thickness defect (IV(o)--ICRS scale) on distal rabbit femur joint surface was made. Three groups were specified: A--defect with paste graft (cartilage and contiguous bone collected from joint surface, crushed into homogenous paste; B--defect with the paste graft covered with periosteum; C--defect left unfilled. The follow-up periods were established at 4, 8, 12 weeks. Repair tissue was evaluated microscopically according to modified O'Driscoll scale. RESULTS: Newly formed tissue was well integrated with surrounding cartilage in group A (paste graft). That trait of repair tissue in group A was much better than in other groups, especially in late observations. Structural integrity of tissue filling the defect was similar to integrity of normal cartilage in groups A and C, but tissue formed in group C didn't represent a hyaline-like cartilage character. In all the examined groups reconstruction of subchondral bone exhibited similar rate. 12 weeks from the procedure, around 80% of subchondral bone was rebuilt. The obtained results indicate, that osteochondral paste autologous transplantation in cartilage defects treatment effects with forming well integrated (structurally and with surrounding cartilage) cartilage tissue, of almost complete subchondral bone rebuilding.

Extracorporeal bypass preserved composite anterior thigh free flap (periosteo-musculo-fascio-cutaneous) for hemipelvectomy reconstruction: utilizing the periosteal component for abdominal wall fascial reconstruction.

We present a case of a right external hemipelvectomy for chondroblastic osteosarcoma originating from the right ilium and reconstruction with salvage parts; cardiopulmonary bypass preserved anterior thigh free flap. The resection required sacrifice of the right common iliac artery and vein and the entire right gluteus maximus muscle. The first stage of the procedure was a high above-knee amputation on the ipsilateral leg. The amputated extremity was placed on extracorporeal bypass utilizing the common femoral artery and vein. This technique, previously unreported, supported the successful utilization of the anterior thigh free flap for both soft tissue and abdominal wall facial reconstruction.

[Mandibular reconstruction with the titanium-hollow screw-reconstruction plate system (THORP)].

The THORP system is designed for bridging mandibular defects and for internal osteosynthesis of mandibular fractures. The major aspects and advantages of the THORP system compared with the conventional reconstruction plates and screws are: 1. The rigid fixation of the head of the screw to the plate, thus avoiding unphysiological loads to the bone underneath the plate. 2. The titanium-plasma coated and perforated hollow screws enable the development of direct bone-titanium contact at the area of the thread as well as the ingrowth of bone into the lumen and perforations. Further advantages are the preservation of small fragments such as the condylar process with only two screws and the stable fixation of bone transplants. The specially designed freely adjustable condylar prosthesis guarantees a correct guidance and function of the lower jaw. The fixation of this system to the lingual aspect of the jaw is particularly significant avoiding decubitus and perforation of the skin in the chin area. The results and the complications of 141 reconstructions of the lower jaw are discussed.
