Bedside terminals support quality patient care.

In order to effectively evaluate hospital information systems today one must consider all aspects of the System, not simply the placement of terminals. A well designed system will be totally integrated via a single database, will support the concept of patient care management by exception, and will be bedside-based. Bedside-based data entry is a key aspect of a well designed system. The placement of terminals should support the work flow, not vice-versa. There are times when nurses and physicians will logically document at the nursing station, in the nurse's lounge, the physicians office, or a clinic; therefore, terminals should be located in these areas. However, the majority of care rendered is provided to the patient in a bed. Therefore, if computers are going to be support tools for clinical applications they must include data entry devices at the bedside. Results to date show this to have a uniformly positive effect on cost of care, quality of care and patient satisfaction.

X-ray reports faster and cheaper.

At Woodhull Hospital, a 600-bed municipal hospital in New York City, the department of radiology uses a computer-based dictation system for transcription of x-ray reports. The system is "trained" to recognize the radiologists' voices and the vocabulary used in the department and can print on command the dictated reports. Outside transcription bills have been reduced significantly, and the success of the unit has initiated the purchase of another system. Moreover, JCAHO surveyors seemed pleased with the method we chose to meet its requirements for the timely delivery of x-ray reports.

Summarized patient bill reduces complaints.

Reading traditional itemized bills can be a frustrating experience, raising many questions from patients speaking in raised voices. But nary a harsh word is heard from AMI-Columbia Hospital's billing office since the hospital switched from the often confusing itemized bill to a shorter summary bill.

Taking stock of a money-saving system. Now may be the time for just-in-time.

The urgent need to trim costs is apparent in hospitals across the country. One target for internal belt-tightening is business forms. Pick-and-pack programs are computerized just-in-time storage and delivery systems developed specifically to reduce a facility's administrative costs for forms. Under pick-and-pack, business forms are stored in a central location, which is connected to either the facility's or a vendor's computer system. Forms are shipped directly to the requesting user as needed. The result is substantial reduction in labor, inventory investment, and storage space. The system provides all the information necessary to keep inventory at optimum levels, while meeting stocking needs and minimizing inventory investment. Better yet, pick-and-pack provides a monthly summary billing, significantly reducing administrative processing costs. Of additional help to the hospital is a report that allocates charges by cost center and includes important data for budgeting and cost control.

The integration of respiratory therapy into nursing: reorganization for improved productivity.

This article outlines a program, implemented by a 133-bed acute care facility, designed to provide more productive patient care delivery while maintaining the level of service delivered. This program required downsizing the respiratory therapy department, integrating a portion of its function into the nursing department, and reducing the annual salary budget by approximately $270,000 or 2 percent of total hospital salaries. The article describes the process and strategies that were used to implement this method of providing respiratory care to hospital patients.
