[Exulcerating cystadenolymphoma of the parotid gland. A case report].

A recurrent adenolymphoma of the left parotid gland occurring in a 79-year-old woman is reported having caused an extensive ulceration of the epidermis, clinically suggestive of a malignant tumour. The ulcerative tumour process cannot be explained by a particular pattern of growth or a malignant transformation of the adenolymphoma. It is assumed that residual tumour cells of the initially incompletely removed adenolymphoma spread via a long-standing postoperative fistula to the epidermis leading to the uncommon ulceration.

[The value of sialography in the diagnosis of parotid tumors].

Sialographic manifestations of 442 patients with primary parotid tumors, 336 benign and 106 malignant, were studied. The sialographic appearance were divided into five types: benign, aggressive benign, low-grade malignant, malignant, and adenolymphoma. The overall corresponding rate between roentgenographic and histologic diagnosis was 81.9%, benign tumors was 89.3, malignant tumors 58.5%, and adenolymphoma 88.9%. Irregular-arrangement, dilation and narrow of branch ducts were the characteristics of adenolymphoma. Extension of main duct, smooth defect of posterior border of mandibular ramus, posterior displace of styloid process, and filling defect of branch ducts and acini in the pterygomandibular space were seen in deep lobe parotid tumors. The causes of mistaken, and the value of sialography were discussed.

[Color Doppler ultrasound studies of parotid tumors].

BACKGROUND: Color-coded duplex sonography is being increasingly used in the head and neck as another method of diagnosis of, for example, stenosis of arteries or veins or of hemangioma. Moreover, it is of greatest interest in having the ability to differentiate benign from malignant tumors. Since the underlying type of tumor is directly responsible for the (operative) treatment recommended we have investigated histologically proven benign and malignant parotid tumors. PATIENTS: Thirty-six patients (20 women, 16 men, ages 32-66 years) with parotid tumors of unknown histology were examined by means of ultrasound and color-coded duplex sonography. Afterwards, tumors were treated by complete parotidectomy. Histological results were compared postoperatively to the data from the color-coded duplex sonography. MATERIALS AND METHODS: Ultrasound B sonography. All patients were examined with a "Sonoline SI-450" (Siemens), using a 7.5 MHz transducer. Color-coded sonography. The color-coded duplex sonograph used was the Quantum 2000 (Siemens). All investigations were carried out with a 7.5 MHz transducer. Criteria of color-coded duplex sonography. Criteria of the investigation were the qualitative range of color distribution as an indication of vascularization and measurement of perfusion velocity--as far as possible--with mean pulsatility indices. The mean pulsatility index represented peripheral resistance and was proportional to it. All data were collected from three areas in the center of the tumor and tumor margins, respectively. Tumors selected were 17 histologically proven pleomorphic adenomas, 15 adenolymphomas and 4 adenoid cystic carcinomas. RESULTS. In comparing perfusion velocity and mean pulsatility indices, no significant difference was detected between pleomorphic adenomas and adenolymphomas. A loss of perfusion and color was seen in both tumor types, although it occurred more often in the adenolymphomas. These cystic areas in most cases had already been clearly detected with normal B-scan sonography. In two of the four malignancies there were no obvious differences to the benign tumors. In the other two cases of malignancy a pronounced vasularization was detected and, therefore, an extended distribution of color. This finding together with decreased perfusion velocity and low mean pulsatility indices indicated low peripheral resistance. CONCLUSIONS: At this point it is not possible routinely to utilize color-coded duplex sonography as an accurate indicator of underlying pathology of parotid neoplasms. A differentiation among different types of benign tumors is not possible using the technique, nor can malignancies be recognized definitively by this method.

Changing frequency of parotid gland neoplasms--analysis of 560 tumours treated in a district general hospital.

An analysis of all parotidectomies performed for neoplastic lesions in the maxillofacial unit at a district general hospital during a 26-year period between 1974-1999 was undertaken. The details analysed were age, sex, histology and temporal variations in the frequency of specific tumour types during the study period. A total of 538 parotidectomies performed on 529 patients in whom 560 tumours were present, formed the basis of this study. Marked variations were present in the age and sex distribution and relative frequency of specific tumour types in this study, when compared to previous reports. There were also differences in the age and sex distribution of pleomorphic adenoma and adenolymphoma (P <0.0001) in this study. The relative frequency of benign tumours and adenolymphoma increased, whereas that of pleomorphic adenoma decreased during the study period. In addition, there was a statistically significant decrease in the relative frequency of pleomorphic adenoma (P <0.0001) and an increase in adenolymphoma (P <0.0001) when comparisons were made with previous studies. This study from a defined population may be more representative of the true proportion of specific tumours in this population. The potential implications of the results on the investigation and treatment of parotid neoplasms is highlighted.

Immunohistochemical distribution of human epidermal growth factor in salivary gland tumours.

Immunohistochemical identification of human epidermal growth factor (hEGF) was carried out in a total of 152 cases of salivary gland tumours, consisting 107 pleomorphic adenomas and their variants, 13 adenolymphomas and 32 adenoid cystic carcinomas. A high percentage of pleomorphic adenomas revealed markedly positive hEGF staining of the luminal surface cells of tubuloductal structures and of modified or neoplastic myoepithelial cells. Clear cells of the tumour showed various reactivities from very slight to strong. Eosinophilic epithelial cells of adenolymphoma gave a positive reaction for hEGF in all the cases, whereas most adenoid cystic adenoma lacked hEGF staining; however some cases showed positive staining of the tumour cells. The immunohistochemical detection of hEGF in most salivary gland tumours suggests this factor to be a possible new marker of salivary glands tumours, and to have a biological role in tumour proliferation.

Contrast-enhanced color Doppler sonography of parotid gland tumors.

The objective of the present study is to assess the potential of color Doppler sonography in imaging tumors of the parotid gland enhanced by application of ultrasound contrast agents. To this end 26 patients with tumors of the parotid gland were investigated. The color Doppler signal areas before and after administration of Doppler signal-enhancing agent (Levovist, Schering, Germany) were determined by means of computer-assisted analysis, and the relevant parameters were evaluated. The tumors can be classified as follows: 11 pleomorphic adenomas, 8 adenolymphomas and two squamous cell carcinomas of the parotid gland, two lymph nodes, one adenoma, one neurinoma of the facial nerve and one non-Hodgkin's lymphoma. Before the administration of Doppler signal-enhancing agent, the adenolymphomas showed a significantly stronger Doppler signal area as compared to the pleomorphic adenomas. The maximum color Doppler signal area after administration of Doppler signal-enhancing agent showed no difference within both groups. If one considers the relative change in the Doppler signal area before and after applying enhancing agent, however, a significantly stronger enhancement of perfusion (increase in Doppler signal area) is noted within pleomorphic adenomas. The changes in the Doppler signal area after administration of signal-enhancing agent show a different course depending on the histology of the investigated tumors.

Identification of oncocytic lesions of salivary glands by anti-mitochondrial immunohistochemistry.

AIMS: To evaluate the immunohistochemistry using an anti-mitochondria antibody in the investigation of various oncocytic lesions of the salivary glands. METHODS AND RESULTS: Ten cases of adenolymphoma (Warthin's tumour) and one case each of benign oncocytoma and oncocytic carcinoma of the salivary glands were examined. Normal salivary glands were also tested. They were investigated immunohistochemically using mouse monoclonal antibody against human mitochondria. In normal salivary glands, epithelial cells of the striated ducts showed a thick linear immunoreactivity, which corresponded well to the intracytoplasmic distribution pattern of mitochondria. In addition, a small number of swollen epithelial cells showing an intense, finely granular immunoreactivity in the cytoplasm were scattered in the ductal system and acini ('oncocytic metaplasia'). Almost all neoplastic cells involved in adenolymphoma, benign oncocytoma, and oncocytic carcinoma showed an intense, finely granular immunoreactivity in the cytoplasm. CONCLUSIONS: Immunohistochemistry using the anti-mitochondria antibody proved to be a highly sensitive and specific method for light microscopic identification of mitochondria and superior to routine H & E or PTAH stain especially in the detection of isolated oncocytic cells.

[Bilateral tumors of the parotid glands. (author's transl)].

Bilateral parotid tumors occur very rarely; they account for 0.5% of 544 cases of parotid tumors recorded at the National Cancer Institute of Milan from 1945 to 1974. Three cases of bilateral tumors are described; two tumors were synchronous and one metachronous. The patients were women of 51, 46 and 52 years. The histologic type was the same on both sides in all cases, namely adenolymphoma, mucoepidermoid carcinoma and mixed tumor respectively. Surgical exicston was the treatment of choice; according to the histology of the tumors partial (adenolymphoma) or total parotidectomy (mixed and malignant tumors) was performed. As to the case of mucoepidermoid cancer of both parotid glands no recurrence was observed after five years.

Presence of endothelin-1 in human salivary glands and tumors.

To elucidate the presence and distribution of Endothelin-1 (ET-1) in tissues of human salivary glands, we performed an immunohistochemical analysis of ET-1 in 15 normal salivary glands, 8 adenolymphomas, 13 pleomorphic adenomas and 5 carcinomas, using the mouse monoclonal antibody against human ET-1. In normal glands, immunoreactivity for ET-1 was observed in the striated duct cells. In adenolymphomas, the columnar cells of the granular epithelium showed strong intracytoplasmic immunoreactivity. In carcinomas, moderate or strong immunoreactivity was observed in the tumor cells, whereas in pleomorphic adenomas, weak immunoreactivity was observed. A good relation was detected between the size of pleomorphic adenoma and ET-1 immunoreactivity, as well as between the duration of tumor in carcinoma and ET-1 immunoreactivity. The presence and distribution of ET-1 in salivary glands and salivary gland tumors suggests a possible role for ET-1 in the regulation of electrolytes and water transport in salivary glands, and as a growth-promoting factor for tumors.

[Cytogenetic patterns in Warthin tumor].

Our systematic studies of different types of benign and malignant salivary neoplasms in humans were initiated about two years ago. Analysis of karyotypes revealed that virtually all tumors are associated with chromosomal abnormalities. That those abnormalities may also take part in tumor initiation and progression is suggested by the association of specific chromosome rearrangement with particular cancers. We here with report cytogenetical observation in five Warthin's tumors giving particular attention to the possible occurrence of cells with (a) a normal karyotype or with (b) numerical changes, either loss of the Y chromosome or monosomy 4. The present series od adenolymphomas was karyotyped by a direct chromosomal analysis method. The method reported was highly successful, in several types of tumors showing the possibility of having good banded metaphases. All Warthin's tumors showed stemline (S) with a normal karyotype. These normal cells constituted the only S in two cases and the primary S in the other three cases. The normal cells observed in all adenolymphomas could be interpreted as outgrowing stroma cells. Most types of previously well-studied benign human tumor types, however (meningiomas, pleomorphic adenomas, lipomas), has a S group with a normal karyotype, regardless of the the tissue culture technique used. Thus, we believe that these normal cells usually represent neoplastic elements. Furthermore, we think that all benign tumors originate with a rather normal S, which, however, as the first indication of neoplastic transformation, shows chromosomal instability, such as the loss of chromosomes.(ABSTRACT TRUNCATED AT 250 WORDS)

Immunohistochemical expression of epidermal growth factor receptor in salivary gland tumours.

Immunohistochemical localization of epidermal growth factor receptor (EGFR) in normal salivary glands and tumours (108 cases) was studied using a monoclonal antibody. In the normal salivary glands, EGFR was occasionally detected in ductal segments of intercalated, striated, and excretory ducts, but not in acinar cells. The frequency of positive EGFR staining in salivary gland tumours was not high: pleomorphic adenoma, 33.8%; mucoepidermoid tumour, 25.0%; adenolymphoma, 44.4%; and sialoadenocarcinoma, 66.6%. Pleomorphic adenomas showed positive staining for EGFR on the luminal side of luminal cells and in squamous metaplastic cells of tumour tissue. Some modified myoepithelial cells were also reactive whereas outer spindle tumour cells were unstained. Adenolymphomas regularly exhibited positive EGFR staining in the cell membrane; mucoepidermoid carcinoma displayed positive staining in cell membranes in epidermoid tumour cells and cytoplasmic staining in mucous-secreting tumour cells. Sialocarcinomas revealed cell membrane staining and whole cytoplasmic staining for EGFR. The immunohistochemical localization of EGFR could be classified into two types, one the cell membrane-positive type found in epithelial tumour cells, and the second the cytoplasmic positive type seen in normal ductal cells, the luminal tumour cells of pleomorphic adenomas and mucous-secreting tumour cells.

[Detection of telomerase activity in salivary tumor and its significance].

OBJECTIVE: To investigate telomerase activity in human salivary cancer and corresponding adjacent tissues and to explore the possibility of telomerase as a tumor marker and its clinical significance. METHODS: Twenty-eight salivary cancers, 28 adjacent peritumoral tissues, 10 mixed tumors, 6 adenolymphomas, and 5 normal salivary tissues were examined for telomerase activity by the silver-staining TRAP assay based on PCR. RESULTS: Twenty-five of the 28 salivary cancers and 2 of the 28 adjacent peritumoral tissue specimens were positive for telomerase activity with a positive rate of 89.3% and 6.3%, respectively. Telomerase activity was negative in the 10 mixed tumors, 6 adenolymphomas, and 5 normal salivary tissues. There was no correlation between the clinical stage of salivary cancer and its expression of telomerase activity (P > 0.05). CONCLUSION: Positive telomerase activity occurs in the majority of salivary cancers examined. It can be used as a tumor marker in the diagnosis of salivary cancer. Detection of telomerase activity in the adjacent peritumoral tissues can be used as a monitoring marker after treatment.

[Preliminary study on diagnosis of tumor like Sjögren's syndrome].

OBJECTIVE: To explore the clinical features and diagnosis of tumor like Sjögren's Syndrome. METHODS: 22 cases with tumor-like Sjögren's Syndrome of parotid gland, 50 cases with pleomorphic adenoma of parotid gland and 25 cases with adenolymphoma of parotid gland were studied retrospectively. RESULTS: Statistical analysis showed that tumor like Sjögren's Syndrome is predominantly in middle-aged women (40-59y) with a woman to man ratio of 6.33:1. Patients commonly present with salivary gland swelling repeatedly, varying in size, response effectively with antibiotics, accompanying dry mouth, dry eyes and aching joints. The mass probably displays medium texture and smooth appearance. The discriminant functions with tumor like Sjögren's Syndrome, pleomorphic adenoma and adenolymphoma were also established. CONCLUSION: The discriminate functions provide a simple and practical method to diagnose tumor-like Sjögren's Syndrome.

Immunohistochemical detection of human lung and gastric cancer antigen in human salivary gland tumors.

Immunohistochemical identification of human lung and gastric carcinoma antigen detected with monoclonal antibodies (MoAbs) KM-93 and KM-231 respectively, was described in 83 salivary gland tumors, including 67 pleomorphic adenomas, 5 adenolymphomas, 3 mucoepidermoid tumors, 6 sialadenocarcinomas, and 2 adenoid cystic carcinomas as well as in normal salivary glands. The binding patterns of these two MoAbs was compared with that of MoAb recognizing epithelial membrane antigen (EMA). Serous cells of normal salivary glands showed positive KM-93 staining, whereas ductal cells were positive with KM-231, with ductal basal cells being characteristically so. EMA staining was confined to luminal and lateral borders of serous acini and ducts. Pleomorphic adenomas indicated positive depositions for both KM-93 and KM-231 in luminal tumor cells or luminal borders of tubuloductal structures. Adenolymphomas showed positive KM-231 staining in basal tumor cells and a positive KM-93 reaction in luminal tumor cells. Mucoepidermoid tumors revealed positive KM-231 staining in mucous-secreting cells, whereas weak KM-93 staining was found in all tumor cells. Sialoadenocarcinomas exhibited varying degrees of positive staining with KM-93 and KM-231 in their neoplastic cells. Adenoid cystic carcinomas showed luminal staining with KM-93 and KM-231 to their neoplastic cells. Adenoid cystic carcinomas showed luminal staining with KM-93 and MoAb EMA. The histogenesis of these salivary gland tumors is discussed in terms of the immunohistochemical features of staining patterns obtained with MoAbs KM-93 and KM-231.

[Well-differentiated acinic cell carcinoma of the parotid].

Well-differentiated acinic-cell carcinoma of parotid gland. The authors report a case of well-differentiated acinic-cell carcinoma of the parotid gland with an important lymphoid component, whereas in the aspiration a diagnosis of adenolymphoma was made. In our case cytologic examination showed an abundant cellularity, consisting mostly of lymphocytes. Epithelial elements were monomorphic and had a discrete quantity of amphophilic, granular cytoplasm. Such structures sometimes appeared to be in continuity with ductal-type formations, suggesting normal glandular acini of the serous type. Elements oncocytes-like with abundant, basophilic granular cytoplasm were seen also. These cells oncocytes-like, the rich lymphoid component and the presence of little amorphous debris, suggested an adenolymphoma. The differential diagnosis between ACC, adenocarcinoma, and lymphonodal metastasis did not show problems because of the lacking of the cytological atipie. Histologic examination of the surgical specimen did not show problems of differential diagnosis. Morphologic patterns included acinar, tubuloductal, microcystic and follicular structures. Periodic acid Schiff positivity, resistant to diastase, was noted in most serous cells. An acid alcian blue stain was negative within the cell cytoplasm but moderately positive in the microcystic cavities. The final histologic diagnosis was that of a well-differentiated acinic cell carcinoma with a rich lymphoid component.

Cytoskeletal proteins in oncocytic tumors of the parotid gland.

Oncocytomas and Warthin's tumors were studied ultrastructurally and with immunofluorescence microscopic techniques against cytokeratins, desmin, vimentin, and actin, the latter characterizing myoepithelial cells. The cell types were found within oncocytomas, one type packed with mitochondria and virtually without filaments, the other contained fewer mitochondria but large amounts of cytokeratins. In Warthin's tumors, all cells contained cytokeratins, the basal cell to a much higher degree. No tumor cells contained vimentin or desmin. Actin was found only close to the apical cell surface in the cylindrical cells in the Warthin's tumors and around the few acinar lumina found in the oncocytomas; thus, a myoepithelial origin of oncocytic tumors of salivary glands seems less plausible. The similarity in staining pattern, namely the pronounced dimorphism of both tumor types, indicates a common progenitor of oncocytomas and the epithelial component in adenolymphomas.

Elastosis in benign and malignant salivary gland tumors. A histochemical and ultrastructural study.

One hundred sixteen cases of various types of salivary gland tumors were examined for the presence of elastic tissue. Almost all the pleomorphic adenomas (97%) and all the malignant pleomorphic adenomas contained elastic tissue in varying amounts. A high percentage (82%) of adenoid cystic carcinomas also contained elastic tissue but the overall quantity was significantly less than in pleomorphic adenomas. All other salivary gland tumors studied, i.e. adenolymphomas, oxyphilic adenomas, mucoepidermoid tumor, and various variants of monomorphic adenomas, were devoid of significant elastic tissue. At the ultrastructural level, the elastic fibers were mainly seen close to neoplastic myoepithelial-like cells, and all stages of elastogenesis were present, ranging from young elastic fibers with a high microfibrilelastin ratio, usually associated with basal-membrane-like material, to mature fibers consisting mainly of an amorphous electron lucent central elastin component. It is postulated that elastic tissue in the salivary gland tumors is produced by the myoepithelial-like tumor cells rather than by tumor stromal induction as has been described in some types of breast carcinomas.

Immunohistochemical demonstration of steroid C-21 hydroxylase in normal and neoplastic salivary glands.

The presence of steroid C-21 hydroxylase in normal and neoplastic salivary glands was investigated immunohistochemically with the use of antibody against cytochrome P-450 specific for steroid 21-hydroxylation (P-450C21). In normal salivary glands, P-450C21 was exclusively present in the excretory duct system (intercalating, striated and large excretory ducts), and not observed in acini. In salivary gland tumors, P-450C21 was observed in most epithelial cells of duct origin within pleomorphic adenomas including cells of amorphous groups scattered in myxoid areas, and in adenolymphoma. Some mucous cells in mucoepidermoid tumor also were positive for this enzyme. These findings in the normal and neoplastic salivary gland suggest that the expression of P-450C21 is closely related to ductal differentiation.
