Gangrenous cholecystitis as a complication of hepatic artery embolization: case report.

Ischemic injury to the gallbladder has been described after hepatic artery embolization but has not been considered a clinically significant complication of this procedure. We present three cases in which therapeutic embolization resulted in symptomatic gangrenous cholecystitis requiring urgent surgical intervention. Clinical parameters that distinguish this infrequent ischemic septic process from the more common postembolization syndrome are discussed and recommendations concerning the diagnosis and management of these complicated patients are outlined.

[Protective silicone prosthesis prior to radiotherapy of recurrent colonic cancer].

Radiation therapy for abdominal recurrence of colon cancer is rarely an option due to subsequent bowel injury. Our case is a woman who underwent resection for a large retroperitoneal recurrence of caecal cancer. Tumour deposits encasing the iliac vessels had to be left behind. A silicone breast prosthesis for displacement of the abdominal content was implanted, allowing postoperative irradiation with 50 Gy. The prosthesis was removed once radiotherapy was accomplished; tumour regression was then complete. Complications are described, so are indications for surgical management of local recurrences of colonic origin as well as technical aspects of abdominal implantation of displacing prostheses. At follow-up after eighteen months the patient has no signs of enteropathy, she enjoys a good quality of life, and she is free of disease. Still, her prognosis is considered uncertain.

Carcinoids of the colon and rectum in patients aged 30 and younger.

Carcinoid tumours of the colon and rectum were diagnosed in 26 patients, 30 years or younger, in Sweden during 1969--1970. Patho-anatomical description was adequate in 24 cases and of these 23 were localized to the appendix and one to the caecum. The treatment was appendicectomy and in the last case ceacal resection. None of the patients had carcinoid syndrom. One patient was lost at follow-up, the 5-year survival rate of the remaining 25 patients was 100 per cent.

[Primary malignant lymphoma in the appendix vermiformis].

Primary gastrointestinal lymphoma (PGL) is a rare condition. A case story concerning a 17 year old boy is presented and discussed. The symptoms were fever and pain in the lower right quadrant of the abdomen, where a tender tumor was palpable. The tentative diagnosis was periappendicular abscess and the patient was treated with antibiotics. As the tumor persisted, a laparotomy with resection of the coecum was performed. Histologic diagnosis was malignant lymphoma originating in the appendix. There was no sign of dissemination in the abdomen. Postoperative treatment with chemotherapy and radiation was carried out. PGL in the appendix in young patients, almost entirely males, will as here often be of the Burkitt type. The histologic type of the tumor, the staging and the radicality at operation are important with regard to prognosis.
