[Oral treatment of continuous ventricular extrasystole with detajmiumbitartrate (Tachmalcor)--clinical studies on effectiveness and dose-time-effect relations including intraindividual therapy comparisons with ajmaline (Tachmalin)].

On 31 inpatients with ventricular permanent extrasystoles (12 females, 19 males) at the age of 17--17 years altogether 45 oral experiments of rhythmisation were performed, on them 29 times with the ajmalium derivate detajmium bitartrate (Tachmalcor) and 16 times with ajmalin (Tachmalin). In 14 cases the comparison of the effect of the two substances was performed in the intraindividual test. In ventricular extrasystoles the ajmalin derivation compared with the initial substance was significantly more effective antiarrhythmically already when the half dose was used. However, it led at the same time to a greater retardation of the intracardiac stimulus conduction. Heart rate and blood pressure were influenced only at the beginning of the treatment and only unessentially. The subjective tolerability was very good. In 9 cases with oral ambulatory long-term treatment the daily maintenance doses of 75--100 mg detajmium bitartrate proved to be fully effective in therapeutic respect. Practical therapeutic conclusions are derived from the results obtained.

[Therapeutic and preventive effect of oral lidocaine in extrasystole].

Lidocaine, Egypt production, P. R. of Hungary, was perorally administered in an average dose of 1.8 +/- 0.38 gr daily for a period of 14.23 +/- 28.9 days to 52 patients with and average age of 61,52 +/- 12.8 with various organic cardiac diseases and ventricular extrasystolic arrhythmia. In 18 of them (47.38%) complete inhibition of arrhythmia was attained, in 10 (26.31%) a combination with another antirhythm preparation was necessary and in 10 (26.31%) the treatment had no effect. In 14 patients the peroral lidocaine form followed its intravenous infusion in acute myocardial infarction with a prophylactic aim and no rhythm disorders were observed. It was established that the unaffected cases were with proportional growth, depending on the severity of arrhythmia, classified according to the prognostic indices of Lown. In 13.46 per cent of the patients, adverse effects were observed, mainly from the gastrointestinal system. The authors think that lidocaine is a proper drug in the treatment of ventricular extrasystolic arrhythmia in the first two classes of Lown and in the prophylaxis of the patients with rhythm disorders after acute myocardial infarction.

[Changes in the hemodynamics and myocardial contraction in ventricular extrasystole].

On the basis of electrokymographic and echocardiographic findings, the authors elucidated a sharp fall in myocardial hemodynamics and contractility during extrasystole contractions arising from the ventricles which was directly dependent on the severity of blood circulation insufficiency. Post-extrasystole contractions occurring after a full compensatory pause tended to compensate the worsened hemodynamics only partially; the degree of compensation being the higher, the better the functional state of the myocardium.

[Comparison of the anti-arrhythmia activity of novocainamide and obzidan in patients with ventricular extrasystole].

Comparative efficiency of obsidan, novocainamide and placebo was studied in patients with ventricular extrasystole of varying origins. Obsidan administered in a dose providing a beta-blocking effect is no less effective than novocainamide in terms of antiarrhythmic activity. Owing to this level of efficiency, obsidan can be recommended for long-term treatment of ventricular extrasystole.

[Bathmotropic effect of panangin and strophanthin in patients with extrasystole].

The antiarrhythmic effect of Panangine and Strophantin was studied by way of rhythmography in patients with extrasystole of different etiology. The strophantin (0.25 mg) an Panangine (2 vials intravenously) tests, and therapeutic courses of Panagine (2 coated tablets 3 times a day for 5--7 days) or of Strophantin (0.25 mg) with Panagine (2 vials intravenously) for 7--10 days were conducted. A course of Strophantin with Panangine was given only to those patient who did not increase the number of their extrasystoles after the Strophantin test. Such a course resulted in the most striking antiarrhythmic effect (60.3% of the patients).

[Sinus extrasystole].

A rare clinical case is described pertaining to a 70-year-old female patient with atherosclerotic cardiosclerosis accompanied by a combination of sinus and ventricular extrasystole. The importance of rhythmography and the criterion of allorhythmia is emphasized as being he indices that help to improve the diagnosis of sinus extrasystole.

[Comparison of antiarrhythmic and chronotropic effect of obsidan and isoptine in extrasystole].

The rhythmographic technique was used to study the antiarrthythmic and chromotropic effect of intravenous injections of 10 mg of Obsidan (105 patients), of 10 mg of Isoptine (105 patients), and of oral administrations of 120 mg/day of Obsidan (160 patients) and Isoptine (128 patients). All patients demonstrated persistent extrasystole of different genesis against the background of the sinus rhythm. With intravenous injections the antiarrhythmic effect of Obsidan at rest did not differ from that of Isoptine. Under physical exercises Obsidan tended to display a more powerful negative bathmotropic effect. With the oral route of administration Obsidan was more effective both at rest, and under exercises. It produced a distinct negative chromotropic effect, while Isoptine displayed a biphasic effect. The results of automatic detection of the classes of the antiarrhythmic effect of Obsidan as judged by the dynamics of the sinus rhythm characteristics permit to suggest the possibility of using the later for an individual selection of the therapeutic means for extrasystole.

Post-extrasystolic potentiation; effect of Ca, histamine, caffeine and epinephrine.

The influence of caffeine, epinephrine, histamine and high Ca solutions on post-extrasystolic potentiation of guinea-pig's isolated heart muscle was investigated. It was found that caffeine suppressed the potentiation process while epinephrine and high Ca solutions enhanced it. Histamine caused a gradual decrease in post-extrasystolic potentiation. The results support the view that the potentiation process is related to an increased Ca release by the terminal vesicles caused by paired stimulation.

[The clinical efficacy of verapamil in ventricular extrasystolic arrhythmia and parasystole].

An arrhythmogenic mechanism has been investigated pharmacologically by acute drug tests (ADT) and alternate block of slow calcium channels and fast sodium channels of the cellular membranes. ADT with verapamil reduced extrasystoles by 60% in 44% of patients demonstrating the predominance of calcium-dependent arrhythmogenesis. An antiarrhythmic efficacy of fast sodium channels blockers was ascertained in 33% of the patients. ADT with rhythmilen and verapamil produced a satisfactory antiarrhythmic effect in 18% of the examinees. These patients seem to have parallel activation of both fast sodium and slow calcium channels. They received courses of both drugs. It is stated that a pharmacological analysis of extrasystolic arrhythmia allows inference on pathogenesis of myocytic electrolyte disturbances which serves the basis for adequate corrective therapy.

[Fulguration of extrasystolic ventricular focus].

A case is presented of symptomatic premature ventricular contractions refractory to drug therapy with right bundle branch block QRS morphology and left axis deviation in a 68-year-old female without structural heart disease. Endocardial mapping localized the extrasystolic focus at meso-inferoapical region of the left ventricular septum suggesting an origin from the Purkinje network of the left posterior fascicle. Catheter ablation with direct-current energy abolished extrasystolic complexes, without complications. The patient remained asymptomatic over a follow-up of 3 months.

[Evaluation of the contractile reserve of the myocardium by post-extrasystolic potentiation in aortic insufficiency].

Calculations were made of the indices of ventricular performance during the ejectional phase in 10 patients with a normal left ventricle and in 24 patients with aortic aneurysm, both under basal conditions and in the presence of extrasystoles produced incidentally during left biplanar cineventriculography. The fact that these indices were constantly raised implies the presence of post-extrasystole potentiation associated with an increase in contractility without significant changes in load. This technique, which provides a dynamic study of the behavior of the left ventricle in aortic incompetence allows us to assess the contractile reserve, and thus to give a more accurate prediction of the potential for recovery after operation.

[Treatment of chronic ventricular extrasystole by propafenone (600 mg/d) in 2 or 3 daily doses].

The efficacy of propafenone by oral route in the treatment of chronic ventricular extrasystoles (VES) was investigated in 14 subjects in the context of a multicenter evaluation carried out double blind and using a crossover sequence. The purpose of this study was to compare the antiarrhythmic efficacy of a dose of 600 mg/d of propafenone randomly divided into two or three subdoses. After carrying out two Holter recordings (< 15 days) the patients presenting with chronic (< or = 100 VES/H) and stable (interindividual variability > or = 30%) ventricular extrasystoles were included. The treatment period consisted of two 8-day courses divided by a placebo period and carried out following a crossover mode. The efficacy of treatment was defined as a reduction in the VES by at least 70% relative to the second Holter during the inclusion period which was used as the reference period. Fourteen patients (57.2 +/- 18.2 years) from eight cardiological centers (eight with heart disease) were included. In general, propafenone at a dose of 600 mg/d bid or tid significantly reduced the total number of VES by about 65%: 15,239 +/- 2,663 VES/24 h (baseline) to 5,238 +/- 2,746 VES/24 h (bid) and 5,765 +/- 2683 VES/24 h (tid); p < 0.0001, with no significant difference between the bid and tid treatments. Individually, 8 patients (57%) responded during the bid treatment, 7 patients (50%) during the tid treatment and 6 patients during both treatments.(ABSTRACT TRUNCATED AT 250 WORDS)

[The effect of iodobromide baths on the physical work capacity and extrasystole in patients with ischemic heart disease and stable stenocardia].

68 patients with CHD and stable angina pectoris class 1-2 in the presence of extrasystole were examined (spiro-bicycle ergometry, Holter ECG monitoring) before and after a course of general iodobromide baths (38 patients) or general fresh-water baths (30 patients). General iodobromide baths exerted marked antiarrhythmic effect in coronary heart disease with stable angina pectoris, the occurrence of ventricular, supraventricular extrasystoles reduced at least by half. Painful and silent myocardial ischemia decreased. Due to training effect of iodobromide balneotherapy muscular performance and coronary heart reserves improved.

[Electrocardiography with the use of the orthoclinostatic test in the diagnosis of extrasystolic arrhythmia in childhood].

The differential-diagnostic importance of some ECG criteria and results of orthoclinostatic test in distinguishing functional form organic extrasystolic arrhythmias in childhood is appraised. Among the electrocardiographic indices the coupling interval is of great importance in differential diagnosis. Left-ventricular extrasystole is mostly encountered in rhythm disorders of functional origin. The data of the orthoclinostatic test are highly reliable in distinguishing functional disorders of rhythm from organic disorders.

Ventricular extrasystoles with syncopal episodes, perodactyly, and Robin in sequence in three generations: a new inherited MCA syndrome?

We observed the combination of the Robin sequence with perodactyly (hypoplasia and/or agenesis of the distal phalanx of the toes) and cardiac arrhythmia (ventricular extrasystoles occurring as bigemini or multifocal tachycardia with syncopal episodes) in 6 relatives in 3 generations. This familial association has not been reported before and probably represents a previously unrecognized heritable malformation syndrome.

Maximal postextrasystolic potentiation following multiple extrasystoles: a simple means of myocardial assessment.

To obtain consistent, reproducible postextrasystolic potentiation (PESP), a single closely coupled extrasystole (ES) must be introduced using transvenous pacing. We hypothesized that multiple ES (spontaneous or catheter induced) produce maximal PESP at all coupling intervals (CI). Differences in PESP between single and double ES were assessed in dogs at varying CI by two different methods [systolic time intervals--PEP/LVET, and endocardial markers--ejection fraction (EF)]. Single ES produced variable decreases in PEP/LVET (-8.6 +/- 2.7% (SE) to -22.9 +/- 4.0%) depending on the CI duration. Double ES produced maximal decreases (-26.0 +/- 1.2%) at all CI. A similar relationship was noted for EF following single ES (8.6 +/- 2.7% to 43.0 +/- 4.0%) and double ES (42.9 +/- 1.3%). Double ES produced maximal PESP at all CI. This finding may allow simple myocardial assessment during routine left ventricular catheterization without simultaneous right ventricular catheterization.

Amiodarone treatment in pregnancy for dilatative cardiomyopathy with ventricular malignant extrasystole and normal maternal and neonatal outcome.

Amiodarone treatment in pregnancy might be difficult to handle because of the long half-life of the drug (14-28 days up to 2 months) and because it reduces maternal and neonatal thyroid activity. Although short-term use in pregnancy has been described in cases of fetal supraventricular tachycardia, there are few reports on the chronic use of the drug. In this paper we describe our experience with amiodarone treatment in two pregnant sisters with familial dilatative cardiomyopathy and ventricular malignant extrasystole. Prolonged administration of amiodarone (400-200 mg/die) since the beginning of pregnancy did not have any adverse effects; maternal and neonatal thyroid function was normal, as was the neurological and motor development of the neonates.

Diagnostic and perinatal management of fetal extrasystole.

Fifty fetuses referred to the Polish Mother's Memorial Hospital for fetal echocardiography between January 1, 1991 and June 1, 1995 were evaluated. The mean fetal gestational age at the time of diagnosis of arrhythmia was 34.1 weeks, and the mean gestational age at the time of delivery was 38.7 weeks. Checkup echocardiographic examinations were performed every 10-14 days, for a mean 2.4 studies per fetus. In most cases (48/50, 96%), premature atrial contractions were present during the first echocardiography examination. The fetal heart study was normal in 30 cases; in 7 (14%) there was tricuspid valve regurgitation, in 7 (14%) an atrial septal aneurysm, in 4 congenital heart defects, in 1 myocardial hypertrophy, and in 1 disproportion in the four-chamber view. Of the 50 fetuses, 43 underwent regular echocardiographic monitoring alone; in 7 cases, based on the presence of additional echocardiographic findings, pharmacotherapy was applied (digoxin, verapamil, or both). Three neonates died after delivery owing to malformations in two cases (one critical aortic stenosis, one spina bifida plus hygroma colli) and due to myocarditis in one case. In six of seven newborns treated in utero, myocarditis was diagnosed after birth (including the one with neonatal demise). Most of the newborns were in good condition after birth, their mean Apgar score being 8.6 and the mean birth weight 3259 g. We concluded that most extrasystoles represent an isolated anomaly, not affecting the fetal condition. Their presence should not influence the obstetric care and may require only echocardiographic monitoring. In most of our cases the premature contractions subsided after birth, although sometimes they preceded fetal supraventricular tachycardia or appeared after congenital myocarditis.
