The effect of injection of a cholecystokinetic agent, Ceruletide, was compared with that of the fatty meal following a single dose of Biloptin, and Biloptin with Solubiloptin, in regard to efficiency of gall-bladder contraction, cystic and common bile duct visualisation and side effects. In terms of gall-bladder contraction and ductal delineation no advantage was found from giving Ceruletide or Solubiloptin. Gall-bladder contraction occurred more quickly after Ceruletide than after a fatty meal, with corresponding earlier duct delineation, although the gall bladder was completely emptied of contrast medium in a significant proportion of Ceruletide cases. The incidence of abdominal pain was significantly greater after Ceruletide than after a fatty meal. Because Ceruletide requires an intramuscular injection and has no significant advantage over the fatty meal in regard to gall-bladder contraction and ductal delineation, its use in cholecystography is unjustified. A morning dose of Solubiloptin following Biloptin the previous evening offers no advantage over the Biloptin and fatty meal regimen.

The ingestion by normal subjects of 3 g of sodium iopodate, which is widely used in routine oral cholecystography, resulted in significant decreases of serum total and free T3 to a nadir on day 4 which averaged 43% and 40%, respectively, below initial mean values. Total and free rT3 increased markedly to a peak on day 3, 244% and 189%, respectively, above initial mean values. Total and free T4 and free T4 index rose to a maximum on day 4, but these changes were not statistically significant. A marked TSH increase was also seen, most evident on day 3. All these changes reverted to baseline values by day 14 at a time when serum total iodide was still markedly elevated. It is concluded that the changes observed after iopodate were not due to alterations in serum binding proteins nor to an effect on thyroid gland by the large iodine component of iopodate, but were consistent with an effect on the peripheral metabolism of T4. Difficulty in interpreting routine thyroid function tests may occur for up to 14 days after oral cholecystography with iopodate.
Sodium iopodate has recently been advocated for long-term control of hyperthyroidism in Graves' disease. Its advantages over conventional therapy are a rapid fall in thyroid hormones and control of symptoms with a simple dosage regime. We report a case in which severe resistant hyperthyroidism developed during treatment of Graves' disease with sodium iopodate. Sodium iopodate may not be suitable for long-term use in all patients with Graves' disease.

An increasing number of disorders that may cause hyperthyroxinemia without thyrotoxicosis have been recognized in recent years. These include acquired and inherited abnormalities of serum thyroid-hormone-binding proteins, peripheral resistance to thyroid hormones, acute nonthyroidal illness, acute psychiatric illness, and some drug-induced conditions associated with nonthyrotoxic elevations of serum thyroxine. In addition to the laboratory finding of elevated serum thyroxine levels, many of these syndromes are also accompanied by abnormalities in triiodothyronine and free thyroid hormone levels, as well as unresponsiveness of thyroid-stimulating hormone to thyrotropin-releasing hormone, all of which further erroneously indicate a diagnosis of thyrotoxicosis. An awareness of these syndromes and alterations in the results of thyroid function tests that accompany them is important to prevent a misdiagnosis of hyperthyroidism and inappropriate therapy.
Investigations on pharmacologic effects on gallbladder contraction must take into consideration the spontaneous motility of the gallbladder. In 20 healthy subjects, we measured the area of the gallbladder sonographically over 10 h at hourly intervals. The spontaneous motility expressed as a coefficient of variation varied between 1.6 and 13% (mean value, 6.3%). Afterwards, we checked the cholecystokinetic action of ceruletide and fatty meal in eight subjects. Here, an alteration of the area by four times the previously found coefficient of variation was defined as significant. A significant contraction occurred earlier after ceruletide administration than after a fatty meal (10 min as compared to 20 min). This lasted longer (40 min. vs. 25 min) and led to a very much more marked reduction in area (69% as compared to 53%). These differences were significant on a 5% level. The end of the contraction did not differ (50 min as compared to 45 min). We conclude that drugs which affect gallbladder contraction can be compared with consideration of spontaneous motility.

Cholecystography was carried out on 456 consecutive patients using varying dosage schedules of cholebrine (iocetamic acid) tablets and biloptin (sodium iopodate) tablets. The density of gall-bladder gave the impression of a slightly greater opacification with cholebrin tablets. Quality of common duct demonstration was equally good with both media, with those patients having a morning dose of contrast giving the best demonstration.
